군 병원 입원환자의 만족도 결정요인 분석

군 병원 입원환자 만족도 및 재이용의사의 영향요인을 알아보기 위하여 군단지원 3개 병원 선정 입원환자 대상으로 자기기입식 설문조사를 실시하였다. 설문내용 구성은 총 70문항으로 복지 및 편의시설(15항목), 군의관(9항목), 간호장교(8항목), 의무병(4항목), 병실생활 환경(8항목), 보급수준(4항목), 복지 및 편의시설(12항목), 입원 및 진료(6항목)로 구성 측정하였다. 그 밖에 시급히 개선해야 될 사항, 가장 만족한 사항, 대상자의 일반특성이 포함되었다.또한 연구대상 병원은 아니지만 첨단의료정보시스템을 도입 활용하고 하고 있는 국군서울지구병원 사례를 분석하여 포함하였다.연구결과 군병원 입원환자의 평균 연령은 21.3세였고 대학재학 이상이 대부분인 고학력이었다. 입원경로별로는 응급후송이 36.0%였으며, 진료과는 내과가 69.5%로 나타났다.입원 환자의 전반적 만족도 및 재이용의사 결정에 영향을 미치는 중요한 변수는 건강회복정도, 군의관요인, 입원 및 이용절차 요인으로 나타났다. 이는 군병원에 대한 과감한 투자를 통해 시설과 장비의 현대화, 군의관의 엄격한 선발과 관리, 입원 및 이용절차의 전산화 및 효율성 강화 등의 노력이 시급함을 시사하고 있다따라서 향후 군 병원의 입원의료 서비스 수준을 민간 대학병원 수준과 비교하여 부족한 점이 없도록 중․장기적인 발전계획에 의거 지속적인 개선을 추구해 나가야 할 것이다.비록 군 병원이 수익성을 최고 목표로 하는 민간병원과는 다른 성격을 가지고 있기는 하지만, 최근 환자중심의 병원과 운영 효율성을 강조하는 사회 전반적인 흐름을 인식할 필요성이 있다. 이제는 군병원 운영에 있어 관료적인 틀에서 과감하게 탈피하여 운영의 전문성과 효율성이 강조되어야 하며, 경쟁력 강화를 위해 군인뿐만 아니라 민간인 진료체계로의 발전 등 혁신적인 개선방안들이 적극적으로 논의되어야 할 시점이다. 본 연구의 결과는 이러한 발전적인 논의가 군병원에서 적극적으로 이루어지는데 중요한 기초 자료를 제공할 것으로 기대된다.

[영문]This study purports to investigate the determinants of inpatients'' satisfaction at 3 military hospitals in Korea. A total of 8 independent variables were selected from literature review. They are the level of satisfaction with physicians, nurses, ancillary staff, quality of ward services, material supply, welfare and convenience, administrative procedure, and health recovery. Each variable contains 4 to 13 items measuring the level of satisfaction with various aspects of the variable.The sample used in this study consisted of 400 inpatients. Data were collected with self-administered questionnaire survey and analyzed using multiple regression analysis.The major findings of the study are as follows:1) The following variables, listed in order of size, have significant positive effect on the inpatients'' satisfaction : health recovery, satisfaction with physicians, administrative procedures, status, age, and admission route.2) The following variables, listed in order of size, have significant positive effect on the intent to revisit of inpatients : health recovery, satisfaction with physicians, administrative procedure, and age..The results of the study indicate that Korean military hospitals should make an effort to improve the satisfactory level of patients with clinical outcome, services provided by physicians and ancillary staff, and the quality of administrative procedure. In order to achieve this goal, such efforts as modernization of facility and equipment, selection and training of good physicians, computerization of administrative procedures are required through the large investment to military hospitals.
