사회적지지가 당뇨환자의 역할행위 이행에 미치는 영향에 관한 연구

dc.description.abstract[한글] 대부분의 만성질환자는 질병의 조절은 가능하나 완치되지는 않는다는 점과 증상이 뚜렷하지 않아도 평생동안 치료와 자가간호를 계속해야 하는 난문제를 갖고 있기 때문에 환자로서 지켜야 할 행위를 이행하지 않아 회복을 지연시키거나 질병을 악화시키게 된다. 특히 당뇨병은 만성 대사성 질환으로 적당한 식이요법, 운동요법, 약물요법을 꾸준히 시행하면 건강인과 같은 사회생활을 유지할 수 있는 반면에 치료를 소홀히 하면 혼수상태에 빠지거나 생명이 위험한 합병증을 일으키는 무서운 질병임에도 불구하고 너무 가볍게 생각하는 경향이 있다. 이러한 만성질환이나 퇴행성 질환의 건강상태를 변화시키는데는 어떤 물리적 환경을 변화시키는 것 보다는 인간의 건강에 대한 신념이나 행위를 변화시키는 일이 더 중요한 일이라 할 수 있다. 그러므로 본 연구는 당뇨환자의 건강신념을 변화시키고 환자역할행위 이행을 증가시키기 위한 간호중재로서 사회적 지지에 관심을 갖고 환자와 그의 가족에게 당뇨병에 대한 정보제공, 상담과 조언, 그리고 자가간호 지침인 소책자 등의 물질적인 조력을 제공함으 로써 당뇨환자의 건강신념을 변화시키고 환자역할행위 이행을 증진시키기 위한 목적으로 시도되었다. 연구 목적을 달성하기 위하여 첫째, 사회적 지지가 환자의 역할행위 이행에 미치는 영향을 규명하고, 둘째 사회적 지지가 환자의 건강신념에 미치는 영향, 셋째 사회적 지지가 당뇨병에 대한 지식을 증가시킬 수 있는가에 대한 영향을 분석하며, 넷째 건강신념과 환자역할행위 이행과의 관계를 규명하고, 다섯째 당뇨병에 대한 지식과 건강에 대한 신념과의 관계를 규명하고, 여섯째 당뇨병에 대한 지식과 환자역할행위 이행과의 관계를 규명하였다. 연구 방법은 유사 실험으로서 1982년 1월 1일부터 1984년 4월 31일까지 만 2년5개월간 C대학 부속병원 내과외래와 기타 병원에서 당뇨병으로 진단을 받아 통원치료 받도록 지시된 환자 172명을 대상으로 실험군과 대조군으로 임의 배치한 후 가정방문을 통한 면접과 혈당 및 뇨당 측정으로 자료를 수집하였다. 연구 도구는 저자에 의해 개발된 설문지와 독일 B.M회사제작의 혈당기 Flulolux, 그리고 미국 Ames회사 제작의 뇨당검사 테이프인 Clinistix를 사용하였다. 설문지는 신뢰도와 타당도를 검증한 이행행위 측정도구, 건강신념 측정도구, 당뇨병에 대한 지식측정도구, 인지된 가족지지 측정도구였다. 수집된 자료의 분석은 사회적 지지 중재의 효과분석을 위한 가설 검정에서 Paired-t 검정과 단순 상관관계 분석, 그리고 환자역할행위 이행과 건강신념에 영향을 미치는 인구학적, 구조적 변수 확인을 위해서는 t-test와 ANOVA로 분석하였다. 환자역할행위 이행에 영향을 미치는 예측인자확인을 위해서는 단순상관관계 분석과 단계적 중회귀 분석을 하였다. 연구 결과를 요약하면 다음과 같다. 1 .사회적 지지 중재의 효과분석을 위한 가설 검정 제1가설 : "사회적 지지는 환자역할행위 이행을 증가시킬 것이다"는 인지된 가족지지 ( t=6.59, p<.001)와 이행행위 (t=15.55, p<001)가 유의하게 증가하고 혈당(t=-1.99, p<.05)과 뇨당(t=-3.57, p<.005)이 유의하게 저하됨으로써 제1가설은 지지되었다. 제2가설 : "사회적 지지는 환자의 건강신념을 변화시킬 것이다" 는 t=6.80, p<.001로 지지되었다. 제3가설 : "사회적 지지는 환자의 질병에 대한 지식을 증가시킬 것이다"는 t=9.36, p<.001로 지지되었다. 제4가설 : "건강에 대한 신념이 높을수록 환자역할행위 이행도 높을 것이다"는 건강신념과 주관적 이행과의 상관계수 r:.5625, p<.001로 지지되었으나 객관적 이행인 혈당치와의 상관계수 r=-.0320, p>.05, 뇨당치와의 상관계수 r=-.0623 , p>.05로 기각되어 제4가설은 부분적으로 지지되었다. 제5가설 : "당뇨병에 대한 지식이 높을수록 건강에 대한 신념도 높을 것이다"는 r=.2629,p<.005로 지지되었다. 제6가설 : "당뇨병에 대한 지식이 높을수록 환자역할행위 이행도 높을 것이다"는 당뇨병에 대한 지식과 주관적 이행과의 상관계수 r=.5560, p<.001로 지지되었으나 객관적 이행인 혈당치와의 상관계수 r=-.1266, p>.05, 뇨당치와의 상관계수 r=-.0790, p>.05로 기각되어 제6가설은 부분적으로 지지되었다. 2. 건강신념에 영향을 미치는 요인은 당뇨병에 대한 지식 ( r=.2629, P<,005), 인지된 가족지지 ( r=.2250, p<.05), 당뇨로 인한 입원경험 ( t=5.46, p<.05), 경제상태 (F=3.58, p<.01), 교육수준 (F=4.04, p<.01), 직업 ( F=2.88, p<.05)였다. 3 .환자역할행위 이행의 실태와 이에 영향을 미치는 요인. 의사의 지시대로 2주에 1회 정기적인 병원방문약속을 이행하는 사람은 27.5 %였고 식품교환표를 이해하고 그에 준하여 식이요법을 잘 이행하는 사람은 3.9%였으며 거의 이행하지 않는 사람이 65.7%였다. 환자역할행위의 주관적 이행에 영향을 미치는 요인은 구조적 변수 중에서는 치료의 유형 (F=11.51, p<.001), 당뇨교육경험 (t=11.60, p<.005), 당뇨로 인한 입원경험 (t=8.78, p<.005), 의료보험 가입상태 (F=4.23, p<.05), 당뇨병력 기간 (F=3.51, p<.05 )이었고, 인구학적 변수 중에서는 유일하게 직업( F=3.04, p<.05)이 확인되었다. 혈당에 미치는 요인은 의료보험 가입상태 하나로서 의료보험에 가입된 사람이 가입되지 않은 사람보다 유의하게 혈당이 낮았다.(F=3.80, p<.05). 뇨당에 미치는 요인은 당료병력기간 하나로서 당뇨병력기간이 길수록 뇨당치가 높게 나타났다. (F=3.739, p<.05). 환자역할행위의 주관적 이행에 영창을 미치는 예측인자를 확인하기 위한 단계적 중회귀 분석 결과 가장 중요한 인자는 건강신념이었고 다음이 당뇨병 지식, 그리고 인지된 가족지지의 순으로 확인되었다. 결론적으로 간호원과 가족에 의한 사회적 지지는 환자의 건강신념 변화와 역할행위 이행을 증진시킬 수 있었다. 그리고 당뇨환자의 역할행위이행에 가장 영향을 미치는 요인은 건강신념이었으며 가족의 지지와 당뇨지식은 환자역할행위 이행 및 건강신념에 영향을 미치는 요인으로 확인되었다. [영문] This study examined the effect of social support on compliance with sick role behavior in diabetic patients. The purpose was to improve patient compliance, health beliefs and knowledge about diabetes The study objectives were to determine factors influencing compliance with sick role behavior and health beliefs, and the effect of social support on compliance with sick role behavior and on health beliefs of diabetic patients. The subjects, consisting of 102 diabetic patients diagnosed in the C. and other Hospitals in K. City, were divided into experimental and control groups. Data were gathered from July to August, 1984 through interviews by questionnaires, measurment of blood sugar level by Flulolux and Urine sugar level by clinistix during home visits. Social support was defined as informational, emotional, material and active support given by the nurse and family. A booklet representing supportive nursing and questionnaires were developed by the investigator, and were tested for content validity by 5 nursing professors, 2 nurses, 2 physicians and 1 diabetic patient, and reliability by Item Analysis : Cronbach's α for any instrument to measure variables was patient compliance .86, perceived family support .81, diabetic knowledge .94, health beliefs : sensitivity .99, severity .98, benefit .99, and barrier .95. Analysis of data were done by t-test, ANOVA, Pearson correlation coefficients, Stepwise Multiple Regression Analysis and Paired t-test The results of the study may be summarized as follows : 1. The effect of social support on Compliance with sick role behovior, on health beliefs, perceived family support and diabetic knowledge was confirmed by significant differences for all the variables between the experimental and the control group before and after the experiment. The first hypothesis that social support will improve compliance with sick role behavior in diabetic patients was supported by increased perceived family support, and decreased blood sugar and urine sugar levels. The second hypothesis that social support will improve health beliefs in diabetic patients was supported. The third hypothesis that social support will increase diabetic knowledge in diabetic patients was supported. The fourth hypothesis that the higher the health beliefs, the higher the compliance with sick role behavior in diabetic patients was only partially supported : health beliefs showed a significant positive relation ship with perceived compliance but not with the blood sugar and urine sugar levels. The fifth hypothesis that the higher the diabetic knowledge, the higher the health belief in diabetic patients, was supported. The sixth hypothesis that the higher the diebetic knowledge, the higher the compliance with sick role behavior in diabetic patients was only partiallysupported : diabetic knowledge showed a significant positive relationship with perceived compliance but not with the blood sugar and urine sugar levels. 2. Factors influencing health beliefs were diabetic knowledge, perceived family support, and the experience of hospitalization for diabetes mellitus. 3. Factors influencing perceived compliance were health beliefs, diabetic knowledge perceived family support, type of medical treatment experience of diabetic education, experience of hospitalization for diabetes, medical insurance, duration of diabetic illness and occupation. The only factor influencing blood sugar level was medical insurance. The only factor influencing urine sugar level was duration of diabetes. The longer the duration of diabetic illness, the higher the urine sugar level. 4. The average of compliance with sick role behavior in the total subject was 48.6 of a possible total of 75 points, Blood sugar level averaged 221.2mg/100cc blood and urine sugar level 1.8. Of the total subject, 27.5 percent Kept appointments with the hospital every 2 weeks. Only 3.9 percent maintained a careful dietary practice.-
