조종사의 건강은 비행안전과 직결되므로 조종사에 대한 건강관리는 예방위주의 포괄적인 건강관리 프로그램을 실시하여 그들의 건강상태를 최상으로 유지하도록 하여 건강장애로 인한 조종기능의 상실로 비행안전을 위협하게 되는 것을 극소화하고, 유효한 조종수명을 최대한 연장할 수 있도록 해야한다.

따라서 조종사를 위한 건강관리 프로그램의 효과 여부는, 조종사의 장기 불구율, 의학적 영구도태율, 실제 비행 중에 건강장애로 인한 조종기능 상실( Inflight incapacitation) 정도로 평가되므로 이에 대한 기초 자료가 필요하고, 앞으로 보다 효과적인 예방위주의 건강관리 프로그램의 개발을 위하여 그동안 20여년간 지속적으로 실시해 온 건강 기록을 정리할 필요가 있다고 생각하여 이에 본 연구를 시도하였다.

본 연구는 우리나라 민항공 조종사의 건강장애를 파악하기 위하여 1970년부터 1988년까지 19년간의 A. 항공사의 건강 기록지를 중심으로 분석한 후향적 서술연구( Retrospective descriptive study)이다.

연구 대상자는 1970년 1월부터 1988년 12월사이에 A 항공사에 근무한 조종사로서 1988넌 12월 현재 근무중인 744명과 사직, 전직, 사망한 176명을 포함한 920명을 조사하었다.

조종사의 건강장애를 파악하기위해 19년동안의 의학적 영구도태, 병휴, 조건부 근무 허가 자료를 분석하였고, 이를위해 출생년도, 입사년도를 포함하여 의학적 영구도태에 관한 변수 3개, 조건부 근무 허가에 관한 변수 7개, 병휴에 관한 변수 49개 등 총 61개의 변수를 선정하여 본 연구자가 작성한 마스터 코드(Master code)에 의하여 부호화 하였다.

병휴의 원인은 한국 질병 사인 분류( Korean Classification of Diseases, 1972)에 근거하여 작성한 항공사 의무부서의 질병코드에 따라 분류하였다.

자료 분석을 위해 연구에 사용된 모든 자료는 전산조직에 입력하여 SPSS-X 통계 프로그램을 이용하였다.

병휴분석은 U.S.P.H.S( United States Public Health Servicea )에서 병휴 분석시 사용하는 빈도율, 불구율, 강도율을 구하였고, 장기 병휴자(강도율 90이상과 의학적 영구 도태 포함 )를 중심으로 조종사의 장기 불구율과 원인 질환, 그 질환의 발생 시기를 파악하였다.

의학적 영구 도태율과 조건부 근무 허가율, 장기 불구율은 연령을 조정하여(예: 29이하, 30-34등 ), 1000인년비로 나타내었다.

본 연구의 결과는 다음과 같다.

(1) 19년간 건강장애로 영구 도태한 조종사는 38명이었고 ( 질병으로 사망한 9명 포함 ), 의학적 영구도태의 원인중 가장 많은 것은 심맥관계 질환으로 전체 원인의 39.5%를 차지하였고, 그 다음은 악성종양으로 21.0%, 안과문제가 13.2%, 간질환이 10.5%를 차지하였다.

사망한 9명의 원인은 악성종양(7), 간 경화증(1), 심근 경색증(1) 등 이었다.

(2) 조건부 근무는 총 214명의 조종사에게 250건을 허가하였다. 원인별로는 순음청력 기준 미달이 156건으로 62.4%를 차지하여 가장 많았고, 두번째로는 고혈압(인가된 혈압강하제 복용 )으로 38건(15.2%), 그 다음은 백내장 수술후 인공수정체 삽입상테, 시력 기준 미달, 수정체 혼탁(lens opacity)등의 안과 문제가 24건으로 9.6%를 차지하였다. 연령별 조건부 근무 허가율(age-specific medical waiver rate )은 30-34세의 4.45/1000인년에 비해 연령이 증가함에 따라 점차 증가하여 45-49세는 32.98/1000인년, 50-54세는 79.21/1

000인년, 55세이상은 89.74/1000인년으로 최고에 달했고. 조건부 근무를 허가받은 조종사의 75.6%가 45세이상 이었다. 그것은 45세이후에 청력 손실, 고혈압, 백내장 등으로 조건부 근무를 허가받은 율이 늘었기 때문이었다.

결론적으로 연령이 증가함에따라 건강장애로 인한 영구도태율, 장기불구율, 조건부 근무를 허가받은 정도가 증가하므로 40세이상의 조종사에 대해 각 건강장애, 특히 심맥관계 질환이 예방과 조기발견을 위한 건강관리 프로그램을 강화하고, 건강장애별로 건강위험요인 profile(Health risk prefile)를 개발, 시행하여 kealth risk reduction program을 활성화하여야 할 것이다.



[영문]

This retrospective descriptive study was undertaken to identify the health problems influencing medical fitness of airline pilots in Korea by health record analysis.

This study detai1s the 19 years experience for permanent medical retirements, medical waivers ( Statements of Demonstrated Ability ), and sickness adsenteeism in a Korean airline pilot population from Jan.1, 1970 through Dec. 31, 1988.

The detai1s pertaining to all 920 pilots who worked for the Airline during above period were obtained.

The tools used for this study were health records of the Airline pilots and Mester code developed by the researcher.

The Master code contains 61 variab1es in total including 3 for permanent medical retirements, 7 for medical waivers and 49 for sikness absenteeism.

The following data were extracted from the health recorda for this study : birth year, the year of entrance to airline, date and reason for termination of employment, number of waivers granted per pilot, onset and types of health problems requiring waivers, frequency of sickness absenteeism for each pilot, onset of each sickness, causes and duration of each absence, and the chtracteristics of injury( industrial or nonindustria1 ).

The date presented were coded by Master code, then compiled and ana1yzed, using an SPSS- X program.

Analysis of data was done in the following manner:

Permanent medical retirements and medica1 waivers were reported as individual cases and their causes were expressed in percentage of the total. Ages were grouped in 5 year increments, - i.e. ages 30-34, 35-39, etc. - in this study.

Permenent medical retirement, medical waiver and long term disability rates expressed as per 1000 personn-years.

The causes for absences were classified according to Coding System used in the Airline medical department and modified based upon Korean Classification of Diseases(1972).

The criteria chosen for analysis of the Airline sickness absenteeism date are those used by the U.S.P.H.S( United States Public Health Servicas ), i .e., frequency, disabi1ity, and severity rates.

They are as fo1low : (1) frequency rate or the average number of absences per 1000 pilots, (2) disbility rate or the average number of days lost per pilot per year, and (3) severity rate or the average number of work days lost per absence.

The results were as follows:

1. During 19 years, 38 pilots have retired because of medical reason, (including 9 deaths), primarily cardiovascular( 39.5%), followed by malignancy (21.6% ), opthalmological problem( 13.2% ), liver disease(10.5%) in sequence.

The causes of 9 deaths were malignancy(7), liver cirrhosis(1), myocardial infarction(1).

2. Age specific permanent medical retirement rates increased rapidly after age 45 years, with cardiovascular disease responsible for more than 39.5% of these medical retirements.

3. Two hundered fifty medica1 waivers were granted for 214 pilots. The leading grounds for waivers were hearing loss( 156cases, 62.4% ), hypertension under contro1 with authorized antilhypertensive drugs by PAA coming in second(38 cases, 15.2%), and opthalmological causes(24 cases, 9.6%) being the third.

Approximately 75.6% of the waivera were granted to pilots aged over 45, and age specific waiver rate increased rapidly above this age with hearing loss, hypertension and cataract being predominant causes.

4. A total of 1189 cases of sickness absenteeism occured during the 19 years. The most prevalent disease observed were upper respiratory infection,( 9.4% of all disease ), hypertension( 8.8% ), aerotitis( 4.9% ), gostritis( 4.5% ), conjunctivitis( 4.0% ).

5. The leading causes of long term disability ( including severity rate above 90 and medical retirement ) were digestive disorder( 18.3% ), opthalmological problem( 15.2% ) and cardiovascular disease( 15% ). Hepatitis, pulmonary tuberculosis, and cataract were the greatest contributor to the total long term disability of pilots.

6. The age-adjusted rate of long term disability resulting from sickness is reletively low between the ages of 30-44, but beginning about age 45 the rate of long term disability rises sharply, and it rises to 19.80/1000person-years for 50-54 age group with cardiovascular disease and cataract being the major

contributing factors.
