[한글]

흡연은 현재 예방할 수 있는 공중보건사업중 가장 건강에 위협을 주고 있는 요인중의 하나이다. 그럼에도 불구하고, 많은 수의 장병들에서 군입대후 흡연량이 늘고 있으며 이에 대한 정책적 고려도 없는 실정이다. 장병의 건강을 책임지고 있는 군의무요원들은 이러한 예방할 수 있는 건강문제에 대해 문제의 제기와 아울러 해결방안을 위한 노력을 계속해야 한다고 본다.

이에 본 연구는 교육을 통하여 병사들의 흡연에 대한 지식. 건강신념, 태도를 변화시켜 궁극적으로 흡연행위에 변화를 가져올 수 있는지를 규명하여, 군보건교육사업을 위한 근거자료를 마련하기 위하여 시도된 유사실험 연구이다.

연구방법은 서울근교지역에 위치한 독립된 세 부대를 실험군 두 군과 대조군 한 군에 각각 할당하여 제1실험군 50명, 제2실험군 50명, 대조군 50명을 연구의 대상으로 선정한 후에 제 1실험군에는 시청각교육 및 집단결정의 방법을 제공하고, 제2실험군에는 시청각 교육방법만을 제공하였으며 대조군을 중재를 가하지 않았다. 실험군 두 군과 대조군 모두에게 본 연구자가 작성한 도구를 사용하여 흡연에 대한 지식, 건강신념, 태도. 행위를 측정하는 사전조사를 실시하였고, 교육제공후 4주후에 같은 측정도구를 사용하여 사후조사를 실시하였다.

자료수집은 1988년10월31일부터 11월30일 사이에 이루어졌다. 수집된 자료중 사후조사시 탈락 및 기재가 불충분한 질문지를 제외한 각 군 각 37명의 사전 사후조사지 총222부가 분석에 사용되었다. 자료분석을 위해 일반적 특성은 실수와 백분율을 구하였으며, 실험군, 대조군간의 유사성 검증을 위해 x**2 -test를 사용하였고, 대상자의 일반적 특성 및 흡연관련 특성파 본 연구의 측정변수와의 유의성 검증을 위해 t-test와 ANOVA를 이용하였다. 가설검증을 위해서는 t-test와 ANOVA , Student-Newman-Keuls test, Pear-son 적률 상관계수를 사용하였으며, 추가분석으로, 흡연행위 변화에 가장 큰 영향을 미치는 요인을 확인하기 위해 단계적 중희귀분석(Stepwise Multiple Regression)을 하였다.

본 연구의 결과는 다음과 같다.

1. 제1, 제2실험군과 대조군의 일반적 특성은 유의한 차이를 보였으나. 횹연행위와 관련된 주요변수인 흡연에 대한 지식, 건강신념, 태도점수에는 세 집단에서 유의한 차이가 없었다.

2 ."교육을 받은군이 받지않은 대조군보다 흡연행위의 변화가 많을 것이다"는 제 1가설은 교육이 흡연량을 줄이는데 효과가 있음으로 나타나고(F=22.5182,p<.001) ,또한 금연이행자의수도 제1, 제2실험군에서 대조군에 비해 유의하게 많은 결과를 보임으로써(x**2 =9.51429, p<.01) 지지되었다. 그러나 "시청각교육 및 집단결정의 방법을 받은군이 시청각교육만을 받은군보다 흡연행위의 변화가 많을 것이다"는 제2가설은 두 교육방법간에 유의한 차이를 보이지 않음으로써 기각되었다(t=-1.38, p>.05).

3. 교육이 흡연에 대한 지식, 건강신념, 태도에 미치는 영향에 대한 검증에서는 먼저 "교육을 받은 군은 대조군보다 지식점수가 높을 것이다"는 제3가설은 지지되었다(F=7.4538, p<.01). 그러나 "교육을 받은군은 대조군보다 건강신념의 점수가 높을 것이다"는 제4가설은 기각되었으며(민감성: F=1.1104, p>.05 심각성: F=.8053, p>.05 장애성에 대한 유익성: F=1.0433, p>.05), "교육을 받은군은 대조군보다 흡연행위에 대해 더 부정적인 태도를 나타낼 것이다"는 제5가설은 제2실험군에서만 대조군에 비해 더 부정적인 태도변화를

나타냄으로써(F= 7.2677, p<.Ol), 부분적으로 지지되었다.

4. 지식, 건강신념, 태도의 변화가 흡연행위 변화에 미치는 영향에 대하여 검증한 결과, "흡연에 대한 지식정도가 높아지면 흡연행위에 더 많은 변화를 나타낼 것이다"는 제6가설은 지지되었고(r=-.3249, p<.001)," 흡연에 대한 건강신념이 높아지면 흡연행위에 더 많은 변화를 나타낼 것이다"는 제7가설은 민감성과 심각성에서만 지지되었다(민감성: r=-.2823 p<.01 심각성: r=-.1955, p<.05 장애성에 대한 유익성 r=-.1220, p>.05). 또한 "흡연에 대해 부정적인 태도를 나타낼수록 흡연행위에 더 많은 변화를 나타낼 것이다"는 제8가설은 지지되었다(r=-.2150, p<.05).

5. 흡연행위 변화에 영향을 미치는 예측인자를 확인하기 위한 단계적 중희귀분석 결과 가장 중요한 예측인자는 지식의 변화였으며 그 다음이 민감성 변화였고 이 2개의 주요변수에 의해 흡연행위 변화를 15.23% 설명할 수 있는것으로 나타났다.

결론적으로, 금연을 위한 시청각교육이 군인의 흡연행위를 변화시킬 수 있는것으로 나타났으며, 지식의 변화가 흡연행위를 변화시키는데 가장 주요한 변수임이 확인되었으므로, 이러한 결과를 토대로 군의무요원들은 장병들의 예방적 건강행위를 위한 효과적인 군보건교육사업을 실천해야 할것이다.

This quasi-experimental study was undertaken to test the ets of two educational approaches to smoking cessation he change of smoking behavior in men in the military and to idea foundation for effective health education service he military.

The subjects for this study consisted of 150 military men serving at three separate military units located in the vicinity of Seoul.

The samples were selected according to a pre-established criteria; 50 enlisted men were enrolled in the first experimental group, 50 enlisted men were enrolled in the second experimental group and 50 enlisted men were enrolled in the control group.

For the first experimental group, the audio-visual education program was followed by a group discussion for decision making. The second group received only the audio-visual education program. The control group dud not receive either of these formal programs.

The tools used for this study were a knowledge scale, an attitude scale and a health belief scale about smoking that had been developed by the researcher. This was given to all three groups as a pre-test. This was then followed by the education intervention programs for the two experimental groups as outlined above. Four weeks after the intervention was given, a post-test using these same scales was done. Smoking behavior was measured by reported number of cigarettes smoked per day.

Data for this study were gathered between October 31st to November 30th 1988 by questionnaire. Subjects who were not available for the post-test or who dud not completely filled in the questionnaire were excluded from the data analysis.

Data were analyzed on a total of 222 questionnaires in which there were a complete pre-test and post-test questionnaire, that is for 37 subjects tn each group.

Analysis of data was done by use of the following:

Frequencies, percentages were used to examine general characteristics of the subjects.

x**2 -test was used to determine similarity among the three groups. t-test, ANOVA, Student-Newman-Keuls test and Pearson Correlation Coefficients were used to test the formulated hypotheses, and Stepwise Multiple Regression was used to find out the most important predictive factor for changing smoking behavior.

The results of this study were as follows:

1. There was a difference between the two experiment groups and the control group on general characteristics including age, military service months, religion, education level, but there was no difference between the three groups on important smoking related variables including knowledge, health beliefs and attitudes.

2. The first hypothesis "that the military men who received education intervention will reveal more changes in smoking behavior than the control group" was supported(F=22.5182, p<.001). The experimental groups showed a reduced number of cigarettes smoked per day and more quitters than the control group but the second hypothesis "that the military men who participated in the audio-visual education and group decision will reveal more changes in smoking behavior than those who received only the audio-visual education program" was not supported(t=-1.38, p>.05). There was no significant difference between the two educational approaches in changes in smoking behavior.

3. The third, the fourth and the fifth hypotheses: "that the military men who received education intervention will have a higher knowledge scores than the control group" was supported(F=7.4538, p<.01).

"That the military men who received education intervention will have higher health belief scores than the control group" was not supported (susceptibility: F=1.1104, p>.05 severity: F=.8053, p>.05 benefits compared to barriers: F=1.0433, p>.05).

"That the military men who received education intervention will reveal a more negative attitude toward smoking" was supported at the second experimental group only (F=7.2677,p<.01).

4. The sixth, the seventh and the eighth hypotheses: "that the higher the level of knowledge change about smoking, the more changes there will be in smoking behavior" was supported(r=-.3249, p<.001).

"that the higher the level of health belief change about smoking, the more changes there will be in smoking behavior" was supported at susceptibility and severity only (susceptibility: r=-.2823, p<.01 severity: r=-.1955, p<.05 benefits compared to barriers: r=-.1220, p>.05).

"that the higher the level of attitude change toward smoking, the more changes there will be in smoking behavior" was supported (r=-.2150, p<.05).

5. The result of the stepwise multiple regression for the purpose of finding the predictive factors for changing smoking behavior showed that the most important one was a change in the level of knowledge, the second one was the change in the level of susceptibility.

These together explained 15.23% of the change in smoking behavior.

In conclusion, this study revealed that the audio-visual education for smoking cessation is an effective educational method and knowledge about smoking is an important factor in changing the smoking behavior of military men.
