류마티스 관절염환자의 우울에 대한 사회적 지지기능

dc.description.abstract[한글] 류마티스 관절염환자는 지속적인 통증과 관절의 부종 및 강직으로 인해 신체적기능이 제한되어 일상적인 활동이나 사회적 관계에 영향을 받게 되고 사회적으로 고립되면서 흔히 우울을 경험하게 된다. 이 환자들의 우울은 증상의 악화를 초래하므로 효과적인 치료를 위해서는 먼저 우울의 문제가 해결되어야 하는데 사회적 지지가 우울에 대한 치료의 중요한 측면으로 부각되고 있다. 이에 본 연구는 류마티스 관절염환자의 우울에 미치는 사회적 지지의 직접효과와 생활사건, 통증 및 신체적 기능장애를 완화시키는 완충효과를 확인하기 위해 설계된 서술적 상관관계 연구이다 연구의 이론적 기틀은 Selye(1976)의 스트레스 반응모형과 사회적 지지모형을 기초로 도출하였다. 자료수집은 1995년 9월 4일부터 9월 16일까지 서울시 소재 일개 대학부속병원 류마티스 클리닉에 치료를 받기 위해 내원한 류마티스 관절염환자 214명을 대상으로 하였다. 연구도구는 종속변수인 우울을 측정하기 위해 Radloff(1977)가 개발한 우울증상들로 구성된 도구(CES-D)를 연구자가 번역하여 사용하였다. 우울에 영향을 미치는 예측변인을 측정하기 위해 Cohen, Hoberman(1983)이 개발하고 서문자(1987)가 수정보완한 대인관계적 지지의 지각척도인 사회적 지지측정도구, 선행문헌(Holmes & Rahe, 1967; Paykel, 1980; 이평숙, 1984)을 기초로 류마티스 관절염의 특성을 고려하여 연구자가 작성한 생활사건 질문지, AIMS의 통증측정도구 그리고 Fries 등(1980)이 개발하고 Kirwan, Reeback(1986)이 수정한 일상활동상의 어려움의 정도를 측정하는 HAQ를 번역한 뒤 수정보완한 신체적 기능장애측정도구가 사용되었다. 자료분석은 SAS를 이용하였으며, 대상자의 일반적 특성과 연구변수에 대한 서술적 통계를 산출했다. 일반적 특성과 우울의 관계 그리고 연구 변수들간의 관계를 확인하기 위해서 t-test, One-way ANOVA 및 Pearson Correlation Coefficient로 분석하였고 가설검증을 위한 제 변수들간의 관계는 Pearson Correlation Coefficient및 Stepwise Multiple Regression으로 분석하였다. 연구결과는 다음과 같다 1. 제 1가설: "류마티스 관절염환자의 생활사건정도가 높을수록 우울정도는 높을 것이다"는 지지되었다(r=.49, p=.0001). 2. 제 2가설: "류마티스 관절염환자의 통증정도가 높을수록 우울정도는 높을 것이다"는 지지되었다(r=.44, p=.0001). 3. 제 3가설: "류마티스 관절염환자의 신체적 기능장애정도가 높을수륵 우울정도는 높을 것이다"는 지지되었다(r=.46, p=.0001). 4. 제 4가설: "류마티스 관절염환자의 사회적 지지정도가 높을수록 우울 정도는 낮을 것이다"는 지지되었다(F=84.52, p=.0001). 5, 제 5가설: "류마티스 관절염환자의 생활사건과 우울정도의 관계는 사회적 지지정도에 따라 차이가 있을 것이다"는 기각되었다(F=.29, p=.5928). 6. 제 6가설. "류마티스 관절염환자의 통중과 우울정도의 관계는 사회적 지지정도에 따라 차이가 있을 것이다"는 지지되었다(F=3.19, p=.0755). 7. 제 7가설: "류마티스 관절염환자의 신체적 기능장애와 우울 정도의 관계는 사회적 지지정도에 따라 차이가 있을 것이다"는 지지되었다(F=5.69, p=.0180). 8. 류마티스 관절염환자의 우울에 영향을 미치는 예측변인은 사회적 지지, 생활사건, 통증 및 신체적 기능장애였다. 9. 류마티스 관절염환자의 우울은 사회적 지지와 역상관 관계(r=-.56, p=.0001)였고 사회적 지지는 생활사건(r=-.22, p=.0007), 통증(r=-.18, p=.0069), 신체적 기능장애(r=-.15, p=.0293)와 각각 역상관 관계를 나타냈다. 10. 대상자의 일반적 특성과 우울정도의 관계분석에서는 우울정도가 여성이 남성보다 더 높았으며(t=2.26, p= .025), 학력수준(r=.189, p=.006)이나 현재 치료방법수(r=.201, p=.003)와 각각 정상관 관계를 나타냈다. 본 연구결과를 통해서 류마티스 관절영환자의 생활사건, 통증 및 신체적 기능장애에 의해 유발된 우울에 미치는 사회적 지지의 직접효과 그리고 통증이나 신체적기능장애가 우울에 미치는 직접적인 영향을 사회적 지지가 완화시킴으로써 우울을 감소시키는 완충효과 가 확인되었으므로 이는 간호중재상황의 예측에 적용될 수 있다고 보며, 사회적 지지의 제공이 이 환자들의 우울감소를 위한 바람직한 간호중재임을 알 수 있었다. [영문] The patients with rheumatoid arthritis have depression because the cardinal characteristics of the disease, persistent pain, swelling and stiffness in joints, limit functional ability and affect activities of daily living, social relationships, and overall quality of life. Social support is emphasized as an important and useful factor to prevent the occurrence of depression and to reduce the depression of patients with rheumatoid arthritis. This study has been done for the purpose of determining whether the positive association between social support and depression is attributable to an overall beneficial effect of support(direct effect) or to a process of support protecting persons from adverse effects of stressors such as life events, pain or physical disability (buffering effect) . The study design was a descriptive correlational study. The theoretical framework for this study was derived from the stress theory developed by selye(1976) and the direct effect model and the buffering effect model of social support theory. The sample consisted of 214 patients who were identified as the rheumatoid arthritis. The instruments used in this study were Depression Scale(CES -D), Perceived Social Support Scale, Life Events Questionnaire, AIMS Pain Scale, and Physical Disability Scale. The data were analysed by the use of t-test, ANOVA, Pearson Correlation Coefficient and Stepwise Multiple Regression. The results of this study are summerized as follows: 1. The lst hypothesis, "The higher the life events degree, the higher the depression degree" was supported(r=.49, P=.0001). 2. The 2nd hypothesis, "The higher the pain degree, the higher the depression degree" was supported(r=.44, P= .0001). 3. The 3rd hypothesis, "The higher the physical disability degree, the higher the depression degree" was supported(r=.46, P=.0001). 4. The 4th hypothesis, "The higher the social support degree, the lower the depression degree" was supported(F=84.52, P=.0001). 5. The 5th hypothesis, "There will be different in the relationship between the degrees of life events and depression according to social support degree" was rejected(F=.29, P= .5928) 6. The 6th hypothesis, "There will be different in the relationship between the degrees of pain and depression according to social support degree" was supported(F=3.19, P=.0755). 7. The 7th hypothesis, "There will be different in the relationship between the degrees of physical disability and depression according to social support degree" was supported (F=5.69, P=.018). 8. The predictive variables for depression were the degrees for social support, life events, pain, and physical disability. 9. The depression degree showed a inverse correlation with social support degree(r=-.56, p=.0001). The social support degree showed a inverse correlation with the degrees of life events(r=-.22, p=.0007), pain(r= -.18, p=.0069) and phisical disability(r= -.15, p=.0293). 10. The depression degree showed significant differences in the variables of sex(t=2.26, p=.025), educational level(r= .189, P=.006) and the number of treatment method(r= .201, P=.003). In conclusion, it was found that social support had the direct effect on depression and the buffering effect in each relationship between degrees of pain or physical disability and depression in patients with rheumatoid arthritis So the researcher thinks that it is effective that nurses should provide these patients with social support to redueedepression in cases of having severe pain or physical disability.-
