비 소세포성 폐암의 예후인자로서 flow cytometric nuclear DNA content와 S-phase fraction의 의의

dc.description.abstract[한글] 폐암환자에 있어서 병리학적 병기가 동일하여도 환자에 따라 수술후의 생존율에는 큰 차이가 있는데, 이러한 사실은 현재까지 인정되고 있는 예후인자, 즉 병리학적 병기, 조직 세포형, 조직학적 분화도 및 수행상태 이외에도 또 다른 예후인자가 있음을 의미한다. 따라서 폐암환자에서 현재 이용되고 있는 예후인자만으로는 다양한 폐암환자의 예후를 정확히 예측하기에 부족하다. 이에 본 연구자는 비 소세포성 폐암으로 진단받고 수술적 치료를 받은 환자에 있어서 유식세포 분석기(flow cytometry)를 이용하여 종양세포의 DNA 함량 및 S-phase 분획(5-phage fraction, 이하 SPF으로 약함)을 분석하여 이들이 폐암환자의 수술 후 예후를 예측할 수 있는 "새로운 예후인자"로서 유용한 지를 알아보고, 또한 유의성의 정도를 평가하여 기존의 예후인자와 예측도를 비교하고자 하였다. 1987년 1월부터 1992년 10월까지 연세 의대 세브란스 병원 및 연세 암센타에서 근치적 절제술을 시행받은 병기 Ⅰ,Ⅱ, Ⅲa의 비 소세포성 폐암환자로 수술 후 보조 항암요법 및 방사선 치료를 받고 1995년 6월말까지 추적관찰이 가능하였던 81예를 대상으로 하였다. 1. 대상환자의 성별분포는 남자 62예, 여자 19예였으며, 연령분포는 25세부터 79세였고 평균연령은 57세였다. 2. 조직 세포형에 따른 분포는 각각 선암 26예(32.1%), 편평상피암 46예(56.8%),선편평상피암 6예(7.4%), 미분화암 1예(1.2%), 대세포암 2예(2.5%)이었다. 병기별로는 병기 Ⅰ 15예(18.5%), 병기 Ⅱ 20예(24.7%), 병기 Ⅲa 46예(56.8%)였다. 3. 대상환자중 59예에서 DNA 함량을 측정하였는데, 비배수성(aneuploidy)이 45예, 배수성(diploidy)이 14예로 비배수성이 76.3%를 차지하였다. 조직 세포형및 병기진행에 따른 비배수성의 비율에는 유의한 차이가 없었다. 4. 대상환자중 36예에서 SPF을 측정하였는데, 평균 SPF은 19.2(±12.62)%였고, 병기의 진행에 따른 SPF의 증가는 없었다. 5. SPF 10%를 기준으로 구분한 저 SPF(low SPF)군과 고 SPF(high SPF)군에서고 SPF군이 차지하는 비율은 75%였고, 병기가 진행할수록 고 SPF군의 비율이 높아짐을 알 수 있었다. 6. 저 SPF군과 고 SPF군의 중앙생존기간(median survival time)은 각각 32개월, 12개월로서 유의한 통계학적 차이가 있었다(p=0.015). 비배수성군과 배수성군간에는 각각 19개월, 34개월로서 유의한 차이가 없었다. 7. 저 SPF군과 고 SPF군의 무병중앙생존기간(disease free median survival time)은 각각 5개월, 19개월로서 유의한 통계학적 차이가 있었으나(p=0.034), 비배수성군과 배수성군간에는 각각 12개월, 34개월로서 유의한 차이가 없었다. 8. 칵스 회귀 모형을 이용한 다변이 생존분석에서 병기 및 고 SPF이 비 소세포성 폐암환자에서 수술 후 유용한 예후인자였다(p=0.009, p=0.043). 이상의 결과로 비 소세포성 폐암환자에 있어 근치적 절제술후에 환자의 TNM 병기와 10%를 기준으로 한 SPF의 결과가 환자의 예후 예측지표로서 유의하였다. 따라서 병기의 진행여부외에 고 SPF을 보이는 경우의 환자에 대해서는 이들 환자의 예후개선을 위한 치료모 델의 정립이 필요할 것으로 생각된다. [영문] In surgically treated non-small cell lung cancer, patients have a wide difference in prognosis even though they may be in the same stage. This means that there are another prognostic factors besides stage, cell type, differentiation and performance status, which have all been used as prognostic factors. Therefore it is difficult to establish the prognosis far individual lung cancer patients. In this study, by using flow cytometric analysis of nuclear DNA content and S-phase fraction(SPF) of surgically treated non-small cell lung cancer patients, we proposed to establish other prognostic factors and their validity in comparison with the existing ones. Specimens from 81 surgically treated patients, diagnosed with non-small cell lung cancer ranging from stage Ⅰ to stage Ⅲa, were analyzed by flow cytometrically determined nuclear DNA content and S-phase fraction between January 1987 and October 1992. These patients has been followed up until Jun, 1995 1. These patients consisted of 62 men and 19 women with an age range of 25 to 79 years(mean 57 years). 2. According to cell type, there were 26(32.1%) adenocarcinoma, 46(56.8%) squamous cell carcinoma, 6(7.4%) adenosquamous cell carcinoma, 1(1.2%) undifferentiatied carcinoma and 2(2.5%) large cell carcinoma respectively. According to staging, 15 of 81(18.5%) patients were evaluated as stage Ⅰ, 20 of 81(24.7%) as stage Ⅱ, 46 of 81(56.8%) as stage Ⅲa. 3. DNA content analysis was carried out for 59 of 81 patients. Of the 59 patiens who were investigated by flow cytometry, 45(76.3%) of the tumors were DNA aneuploidy and 14(23.7%) were DNA diploidy. The proportion of DNA aneuploidy tumors showed no significant difference between cell types or stage. 4. S-phase fraction was evaluated for 36 of 81 patients. Mean value of SPF was 19.2%(±12.62)% The value of SPF had nothing to do with stage. 5. The proportion of the high SPF group(more than 10% of cell proliferation cycle) was 75%. With advance staging, the proportion of the high SPF group increased. 6. Significant difference in the median survival time was observed between the low SPF group and the high SPF group(32 months in low SPF, 12 months in high SPF)(p=0.015). No significant difference in the median survival time was observed between the aneuploidy group and the diploidy group(19 months in aneuploidy, 34 months in diploidy). 7. Significant difference in the disease free median survival time was observed between the low SPF group and the high SPF group(5 months in low SPF, 19 months in high SPF)(p=0.034). No significant difference in the disease free median survival time was observed between the aneuploidy group and the diploidy group(12 months in aneuploidy, 34 months in diploidy). 8. Upon multivariate analysis, including SPF, stage, cell type, DNA content etc, stage and high SPF(more than 10% of cell proliferation cycle) were significant prognostic factors in surgically treated non-small cell lung cancer patients(p=0.009, p=0.043 respectively). In conclusion, the TNM stage and high SPF were significant as prognostic factors in surgically treated non-small cell lung cancer patients. Therefore new treatment plan should be needed in the patients who have high SPF.-
