혈액배양에서 분리되는 균종과 항균제 감수성 양상을 파악하는 일은 환자의 경험적 치료를 위한 중요한 근거가 된다. 혈액배양에서 분리되는 균종과 항균제 감수성 양상은 시대, 지역 및 대상환자 등에 따라 달라지게 된다.

본 연구에서는 1984-1993년간의 세브란스병뭔 입원 및 외래환자의 혈액배양 결과와 1988년 5월부터의 항균제 감수성 결과를 후향적으로 분석하였다. 이 기간동안 207,877검체의 혈액이 배양되어 17,407검체 (8.4%)가 배양양성이었으며 양성 환자수는 10,021명이었다.

분리된 균종 중 호기성 세균이 91.6%, 혐기성 세균이 6.4%, 진균이 2.0%이었다. 흔히 분리된 균종으로는 Escherichia coli, Staphylococcus aureus, α-hemolytic Streptococcus, Enterococcus, Klebsiella, pneumoniae, Enterobacter, Salmonella typhi 및 Pseudomon as aeruginosa 순이었다.

연도별 분리율은 S. typhi 의 경우는 1984년 7.4%에서 1993년 1.9%로 현저히 감소하였으며, Enterococcus는 1984년 2.6%에서 1993년 5.3%로 증가하였고, Enterobacteriaceae의 분리율은 변화가 없었다. 2세 미만 군에서는 α-hemolytic Streptococcus, 20-59세 군에서는 S. typhi, 50세 이상 군에서는 Enterobacteriaceae와 포도당 비발효 그람음성 간균의 분리가 많았다. 다균성 균혈증은 전체 배양 양성 환자중 3.9%였고, 흔히 분리된 세균은 E. coli, K. pneumoniae, Enterobacter 및 S. aureus이었다.

항균제 감수성 시험 결과에서 S. aureus의 methici1lin에 대한 내성율과 Enterococcus 의 ampicillin에 대한 내성율은 현저히 증가하였고, K.pneumoniae는 3세대 cephalosporin에 대한 내성율이 증가하였다.

이상의 결과에서 균혈증 환자에서 가장 흔히 분리되는 것은 E. coli이며, S. aureus, α-hemo1ytic Streptococcus, K. pneumoniae가 변함없이 흔히 분리되는 균혈증의 뭔인균이며, S. typhi의 분리가 현저히 감소하고, Enterococcus가 균혈증의 중요한 병인균으로

대두되고, 혐기성 세균 및 진균에 의한 균혈증도 증가하고 있으며, 또한 methicillin 내성 S. aureus와 3세대 cephalosporin에 내성인 K. pneumoniae가 증가하고 있다는 결론을 얻었다.

Blood culture is an important procedure for the determination of the etiologic agent of bacteremia. Analysis of blood culture results and antimicrobial susceptibility trend can provide the clinicians with relevant informations for the empirical treatment of patients. In this study, the results of both the species and

antimicrobial susceptibility of the isolates at the Severance Hospital during the years 1984-1993 were analysed.

During the study period, a total of 207,877 blood specimens were cultured, and 17,407 positive specimens were obtained from 10,021 patients. Among the isolates, 91.6% were aerobic or facultative anaerobic bacteria, 6.4% anaerobes, and 2.0%

fungi. E. coli was isolated most frequently, followd by S. aureus, α-hemolytic Streptococcus, Enterococcus, K. pneumoniae, Enterobacter, S. typhi and P. aeruginosa. The proportion of patients with Enterococcus increased from 2.6% in 1984 to 5.3% in 1993. S. typhi decreased from 7.4% in 1984 to 1.9% in 1993. There

was no significant change in the other species of Enterobacteriaceae. α-hemolytic Streptococcus was frequently isolated from the age group of less than 2 years, Enterobacteriaceae and glucose-nonfermenting gram-negative baci11i from the age group of more than 50 years, and S. typhi from the age group of 20-59 years. The isolation rate of S. typhi was higher in April-March, but no obvious seasonal trend was observed in most of the other organisms. Polymicroblal bacteremia were found in 3.9% of patients with positive blood cultures. The organisms frequently involved were E. coli, K. pneumoniae, Enterobacter and S. aureus. Methicillin-resistant S. aureus, ampici11in-resistant Enterococcus and cefotaxime-resistant K. pneumoniae increased significantly during the study period.

It is concluded from the study that E. coli is the most common cause of bacteremia and that S. aureus, α-hemolytic Streptococcus and K. pneumoniae remains to be frequently isolated pathogens. While the isolution of S. typhi has decreased over the study period, the bacteremia due to Enterococcus has Increased. More frequent resistance of S.aureus, Enterococcus and k. pneumoniae become problems In the selection of antimicrobial agent.
