악교정 수술시 골편의 술전 예측치와 술후 설제치에 관한 연구

악안면 기형환자를 술후에 평가함에있어 술전에 치료 결과를 예측한 자료를 기초로 수행한 술후결과를 술전예측치와 실제 비교 ·연구한 발표가 드물어 저자는 1990년 1월부터 1994년 12월까지 연세대학교 치과대학 구강악안면외과에서 악안면 기형으로 악교정수술을 시행받은 총 254예중 술전 ·술후 촬영된 사진상에서 S-N plane의 중첩시 차이가 1㎜ 이내이며, Le-Fort I 골절단술과 시상분할 골절단술(Sagittal split ramus osteotomy : SSRO) 및 구내 수직골 절단술 (Intraoral Vertical Ramus Osteotomy : IVRO)을 시행 받은

39증례를 대상으로 술전 lateral cephalogram 상에서의 술후의 예측치와 술후 즉시(3일이내) 촬영된 사진에서의 술후 실제치와의 차이를 일정기준선 위에서 측정하여 분석한 결과 다음과 같은 결과를 얻었다.

1. 상하악 동시 수술군이 하악 단독 수술군에 비하여 큰 차이와 표준편차를 보였다.

2. 상하악 동시 수술군중 상악에서보다는 하악계측치에서의 차이가 크게 나타났으나 Coronoid(Cor)를 제외한 모든 측정치에서 유의성은 관찰되지 않았다.(P<0.05)

3. 상하악 동시수술군과 하악만 수술한군 모두에서 Upper Incisal Tip(U 1)에서의 차이가 가장 작았으며 Mention(Me)과 Antegonial Notch(AgN)에서의 차이가 비교적 크게 나타났다.

4, 상하악 동시수술군중 상악중절치의 이동방향에 따른 상악골의 술전예측치와 술후실제치간의 차이는 절치단을 이동하지 않은 group과 하방이동한 group에서 적은 차이를 보였으며 전방으로 이동한 group과 상방으로 이동한 group에서 큰 차이가 관찰되었다.

5. 상하악 동시수술군중 상악 제1대구치의 상방이동량에 따른 상악골의 술전예측치와 술후실제 치간의 차이는 계측점 Upper Incisal Tip(U 1) 및 AnteriorNasal Spine(ANS)에서는 두 group간에 비슷한 값을 나타냈으나 PosteriorNasal Spine(PNS)은 두 group간에 의미있는 차이를 보였다 (p<0.05).

[영문]

It is of interest and also a responsibility of the operator to make a proper prognosis of the outcome of a surgery on maxillofacial deformity and perform retrospective studies of preoperative plans on the basis of postoperative results.

In order to assess the accuracy of a planned surgery according to preoperative preparations and operative procedures, 254 patients were followed-up who had visited the department of oral and maxillofacial surgery at the Yonsei university for maxillofacial deformity since 1990 to 1994.

The deformities of the maxilla were treated with LeFort I osteotomy and those of the mandible with sagittal split ramps osteotomy or intraoral vertical ramus osteotomy.

39 cases, in which the superimposition deviation of the S-N plane on preoperative and postoperative radiographs were within 1mm were selected for this study.

A comparative analysis was performed on a predetermined standard line of estimated values on preoperative lateral cephalograms and of postoperative measured values on radiographs taken within 3 days after surgery.

The following results were obtained.

1. Cases with simultaneous maxillary and mandibular surgery showed a great difference and standard deviation than cases in which surgery was performed on the mandible alone.

2. Of the cases in which surgery was performed on both jaws, the measured values of the mandible showed a greater difference but, except for Coronoid(Cor) value, there was no statistical significance in all measuring points.(p<0.05)

3. Simultaneous maxillo-mandibular surgery group and only mandibular surgery group showed the least difference in Upper Incisor TiP(U 1) value, but Menton(Me) and Antegonial Notch(AgN) values exhibited a relatively great difference.

4. Of the cases in which surgery was performed on both jaws, the mean and standard deviation of immobilized group and downward movement group according to the difference of incisal movement direction showed a relatively small difference but a great difference was noted in forward and upward movement groups.

5. Of the cases in which surgery was performed on both jaws, the difference of upper Incisal Tip(U 1) and Anterior Nasal Spine(ANS) values in both groups according to the amount of upward movement of the upper first molar were similar but the Posterior Nasal Spine(PNS) value was significantly different in both groups.(p<0.05)
