돌봄에서의 신체적 접촉의 의미

신체적 접촉은 인간에게 있어 가장 원초적인 의사소통의 수단이며 이러한 신체적 접촉이 전하는 의미는 인생 경험을 통해 학습된 것이다. 그러므로 인간은 신체적 접촉의 의미에 대한 주관을 갖고 있다. 이러한 주관적 측면으로 인해 신체적 접촉은 의미를 전하는

적극적인 방법임과 동시에 오해 개입의 여지를 아울러 갖고 있다 그러나 간호에 있어서 신체적 접촉은 실무 관계에서 보편적으로 인정된 것이며 필수적 요소로 간호사-환자관계에서 중요한 위치를 차지하고 있다. 따라서 신체적 접촉의 주관적인 의미는 탐구되어져야

한다.

인간은 성장과정을 통해 누군가를 돌보거나 돌봄을 받은 겸험이 있으며 그에 따른 신체적 접촉의 의미를 갖고 있다. 돌봄은 간호의 본질적 요소이며, 돌봄에서의 신체적 접촉의 의미에 대한 연구는 신체적 접촉의 유용한 중재 방향을 제시해 줄 수 있다.

이러한 의미에서 본 연구는 한국의 돌봄에서의 신체적 접촉의 의미를 알아보고, 이를 유형화 하고 유형별 특성을 파악하고 속성을 분석하여 돌봄에서의 신체적 접촉의 특성을 확인함으로 간호 실무와 관련 연구에 기여 하고자 시도되었으며 연구 접근법은 신체적 접촉의 주관적, 개별적 속성을 감안하여 대상자의 자아참조적 진행정의(operant definition)에 의하는 Q-분류법을 이용하였다.

간호사, 환자, 의사, 일반인, 대학생, 간호교육자, paramedicine 종사자인 158명을 대상으로 면담과 질문지법을 통하여 수집한 162개의 자아참조적 진술들(concourse)을 Q-모집단으로 하여 항목신뢰도(item reliability)와 분류신뢰도(sorting reliability)와 검정을 거쳐 확정한 Q-표본은 43개 항목이었다.

연구대상 P-표본은 의료 관계 기관에 입원 중인 환자 10명, 의료 관계 기관 종사자로 간호사 6인, 의사3인, 약사 3인, paramedicine 종사자 3인, 간호학 교육 관련자 3인 그리고 의료 기관에서 현재 치료 받고 있지 않는 성인 남녀 13인으로 총 41인이 었다.

표출된 41명의 대상으로 하여금 43개 각 Q-항목을 각자의 동의 수준에 따라 +4에서 -4까지 9단계로 강제 정상 분포(forced normal distribution)가 되도록 분류하게 하였고, 돌봄에서의 신체적 접촉과 관련된 면담을 하였으며 인구.사회학적 및 건강관련 자료를 구조화 질문지를 통하여 수집하였다.

자료 수집 기간은 1995년 6월 20일부터 1995년 8월 31일까지 73일간 이었다.

분석은 9단계로 분류된 각 Q-항목의 표준점수(Z-score)를 계산하였고, PC-QUANL program을 이용하여 Q-요인분석을 하여 돌봄에서의 신체적 접촉의 의미에 대한 5개의 유형이 추출되었으며, 요인간의 상관 관계와 각 항목이 갖는 평균 점수와 표준 편차를 통하여 돌봄에서의 신체적 접촉의 공유된 의미 특성을 확인하였다.

유형별 신체적 접촉의 의미의 특성은 Q-항목 중 강한 동의 또는 비동의를 나타내 표준점수가 높은(+1.000 이상)항목과 표준접수가 낮은(-1.000 이하)항목 및 타 유형 표준점수와 차이가 큰 (+1.000 이상 또는 -1.000 이하)항목을 분석함으로 이루어졌으며, 유형별 사회.인구학적 특성과 신체적 접촉과 관련된 면담 내용을 분석하여 유형별 특성을 확인하였다.

한국에서의 돌봄에서의 신체적 접촉의 의미의 공유된 특성과 5개의 의미 유형은 아래와 같이 요약된다.

돌봄에서의 신체적 접촉의 의미의 특징은 돌봄이라는 상황적 특성에 따른 의미는 공유되면서도 고유한 특성을 갖는 여러 유형으로 형성이 되는 것이었다. 공유된 의미는 의도가 대상자를 돕고자 하는 것이라는 것과 돌봄의 제공자의 신체적 접촉에 대한 긍정적 평가 그리고 좋은 감정을 전달한다는 측면이었다.

유형별 특성은 유형 Ⅰ은 기의 교류형으로 돌봄에서의 신체적 접촉은 치료적인 의미를 지닌다고 생각한다. 그러므로 오랜 세월 동안 신체적 접촉이 치료의 한 방법이었던 동양 의학적 관점에서 기의 흐름을 원활히 하는 기능이 신체적 접촉을 통하여 이루어 진다고 믿는다.

유형 Ⅱ는 영적 교감형으로 아플 때 신체적 접촉에서 도와 주는 사람이 있다는 느낌을 가지며 약한 입장에서 상대방을 의지하는 마음을 갖는다. 아픔을 통해서 스스로를 돌아보며 그때의 종교적 기원을 담은 신체적 접촉에서 절대자의 힘이 대리하여 흐를 수 있다고

생각하고 있으며 절대자의 힘으로 영적인 면이 충만되면 병이 나으리라 생각한다.

유형 Ⅲ은 심신 안위형으로 돌봄에서의 신체적 접촉은 용기를 주고 안스러움을 전달함으로 정서적 안정을 도모하여 신체적 고통을 경감시키는 것이라고 생각한다.

유형 Ⅳ는 정서적 공감형으로 돌봄에서의 신체적 접촉은 대상자에 대한 사랑과 관심의 방법이라고 생각한다. 신체적 접촉을 할 때 대상자와의 교감의 형성으로 감정이 순화됨을 느낀다. 즉 신체적 접촉은 어려움에 대한 이해의 교감을 형성하는 관심 표현과 서로의 공유의 기회를 확인할 수 있는 방법이라고 생각한다.

유형 Ⅴ는 역할 기능형으로 돌봄에서의 신체적 접촉은 돌보는 자와 돌봄을 받는자의 양자의 역할에 따른 관계 형성을 세우는 과정에서 효과적으로 기능한다고 보는 입장이다.

따라서 신체적 접촉은 치료자는 치료의 입장에서 상태의 확인을 위한 직업상 필요한 행위이며 보편적인 행위라고 생각한다.

Physical touch is one of the primitive means of communication, and the meaning of physical touch is assumed to be learned through each individual life experience, thus, the factors influencing on the development of meaning of physical touch are

multiple in nature. Caring is an essential phenomena between human interaction ; every individual is ether care giver or care recipient all through one's life. And caring is the core of nursing phenomena. Physical touch is an universal and essential part of nursing practice.

This study was designed and carried out to identify, describe and classify the meaning of physical touch in caring and to identify factors related to the meaning of physical touch.

In an atempt to measure the subjective meaning, the unstructured Q-methodology

was used. For the Q-population, 162 self-referent descriptions on meaning of physical touch in caring were gathered from 158 Korean adult in caring situation; nurses, doctors, patients, nurses educators and university students, unspecified healthy adults.

Among the Q-population, a total of 43 Q-items were sampled and validated through item-reliability and sorting-reliability tests.

The P-sample was consisted of 41 Korean adult: 10 patients, 6 nurses, 3 doctors, 3 paramedical practitioners, 3 nurses educators, 13 adults.

Sortings of Q-items according to the level of personal agreement were carried out by the P-samples and a forced normal distiibution of the 43 Q-items into 9 levels.

The Z-scores of Q-sort data were computed and principal components factor analysis were carried out by the use of PC-QUANL Program. The demographic, socio-cultural, health-related attributes and interview data about physical touch in caring were

analysed to identify the property of types.

5 types of meaning of physical touch in caring were identified;

Type Ⅰ consisted of 10 P-samples, named the type of exchange of Qi .For them, the meaning of physical touch in caring is mode of therapy reflecting perspectives of oriental medicine. Physical touch in caring means to fuction in facillitating the flow of Qi throughout the body.

Type Ⅱ consisted of 5 P-samples, named the type of sharing spirituality. For them, the meaning of physical touch in caring is to convey religious plea to the one to be recoverd from illness, to convey blessing of God and sharing love of God.

Type Ⅲ consisted of 9 P-samples, named the type of psycho-somatic comfort.

For them, the meaning of physical touch in caring is to achieve physical comfort through promoting of emotional equibrilium. So they believe that the physical touch in caring relieve the pain.

Type Ⅳ consisted of 10 p-samples, named the type of empathic understanding. For them, the meaning of physical touch in caring is a means to share love, concern and understanding of difficulty.

Type Ⅴ consisted of 7 p-samples, named the type of role-function, For them, the meaning of touch in caring is function of process to establish relationship according to personal role.

By additional analysis, good intention, positive attitude toward caregiver and sharing goodness were identified to be shared by all P-samples in the study.
