척수마비환자를 위한 간호사정도구 개발

과학적인 사정은 환자에 대한 이해를 높이고 적절한 간호를 제공할 수 있게하기 때문에 환자의 빠른 회복에 도움을 준다. 그리고 사정한 자료를 일관되게 기록하는 것은 건강관리요원들간의 의사소통을 높이고 반복적 사정을 예방할 수 있다. 본 연구는 척수마비 환자의 독특한 건강문제를 적절히 사정하고 기록할 수 있게하며 건강관리요원간의 긴밀한 의사소통으로 환자에 대한 이해를 높이고 더 높은 질의 간호를 제공하기 위한 사정도구의 개발을 그 목적으로 한다. 척수마비는 중추신경계의 기능상실로 신체, 정신, 사회적 모

든 부분에 심각한 영향을 미치게 한다. 척수의 마비수준과 정도는 환자의 남은 전체 삶의 기능을 결정한다. 그러므로 척수마비환자의 간호사정은 환자의 모든 국면을 총체적으로 이해할 수 있는 것이어야 한다. 따라서 이 도구가 포함한 내용은 문헌고찰을 통해 척수마비의 증상과 증후, 합병증을 검토하여 선정되었는데 여러 신경계 검진에서 제시된 일반적 사항들 즉, 인구통계학적 요소들과 주호소, 질환의 발병시기와 경위, 환자의 건강, 특히 신경계에 영향을 미칠 과거력과 가족력을 포함하였다. 구체적인 척수마비환자의 문제로는 호흡, 순환, 영양, 통증, 수면, 배뇨와 배변, 피부, 자율신경계 문제, 운동과 감각, 정서사회적 요소, 성기능과 역할 그리고 대처와 대응들을 그 내용으로 한다.

각 부문들은 다시 소항목들로 구성되어 있는데 이 작은 항목들은 큰 제목을 구체적으로 사정하기 위한 변수들이다. 이 도구는 사정하는 간호사들은 동일한 준거를 공유하게 하기 위하여 사정을 위한 지침과 기준을 함께 제시하고 있다. 특히 척수마비환자에게 있어 지속적으로 변화하는 운동, 감각 기능과 반사 그리고 호흡능력을 보는 신경계 검사는 매일매일 사정할 수 있게 흐름표로 구성되어 있다. 작성된 예비도구는 동일한 환자에 대한 두 측정자간의 차이를 측정하는 채점자간신뢰도를 검증하기 위해 두명씩 짝을 이룬 5개조 간호사들에 의해 척수마비환자들에게 사용되어졌다. 이 채점자간신뢰도는 전체 동의백분율점수로 표기되는데, 이 연구에서는 87.5%로써 신뢰성이 있다고 본다. 내용타당도 검사는 척추센터 수간호사 4인과 대학에서 신경계를 가르치는 교수 2인으로 구성된6명의 전문가 집단에게 의뢰되었으며 4점 척도로 이루어진 설문지가 배포되어 각각의 문항에 대한 타당도 점수가 주어졌다. 이렇게 선정된 항목들중 전문가집단 6인 중 5인 이상이 3점 이상을 준 항목들을 최종 선정하였는데 이는CVI(Index of content validity)가 83%(p <.05)이상인 것들이다.

연구결과를 요약하면 다음과 같다.

1. 신뢰도 검정을 위해 채점자간 신뢰도점수를 적용했는데 도구의 전체 동의백분율점수는 87.5%였다.

2. 내용타당도를 위해 전문가 집단 6인중 83% 이상의 전문가가 4점 척도를 사용해 3점 이상의 타당도 점수를 준 문항을 선정하였다. 이렇게 선정된 항목은 전체 126개 항목 중 114개 였다.

위의 내용으로 볼때 척수마비환자를 위한 본 간호사정도구는 신뢰도와 타당도가 매우 높은 도구로써 척수마비 환자의 총체적 사정에 적합한 도구라고 평가할 수 있다.

Systematic assessment helps nurses to gain a better understanding of patients. Therefore, patients' recovery may be facilitated by the use of these skills. Consistent documentation of the assessed data may improve the communication among the health care members decreasing the repetition of assessment and improving the quality of care.

Spinal plegia affects all the aspects of a patient's life because of the loss of functions related to impairment of the central nervous system. The level and the degree of spinal plegia determines the patient's functional status for the rest of

his life. The literature review resulted in the co]]ection of sign, symptom and complication of the spinal plegia. The new assessment tool included : the general assessment which is made up of the demographic factors, the chief complaints, the history of the chief complaints, the general past history including especially the nervous system and the family history. The specific problem areas in the assessment are : respiratory system, circulatory system, nutrition, pain, sleep pattern, bowel

and bladder, skin, autonomic nervous system, motor and sensory function, sexuality and psycosocial elements and coping. Preliminary tool was tested by 5 group nurses, organized into pairs, with one pair being a group. Each of the two nurses from the one group applied the tool to same patient. Differences between raters were tested for interrater reliability. The items that scored above 80% of total percent agreement were accepted as reliable. For the validity, the tool was evaluated by a panel of experts which consisted of experts. To test content validity, a 4 point likert scale questionnaire was given to the experts and the experts rated each of the items as to their validity. From those selected items, items which had 83% CVI were finally included on the new assessment tool.

The results of the study were summarized as follows. :

1. To test reliability, interrater reliability was applied. The total percent agreement of the tool was 87.5%(an average of 70% is necessary, 80% is adequate and 90% is good).

2. To test validity, content validity was applied. On the 4 point likert scale, the items that gained 3 or 4 points were selected, and among them the items that gained above 83% agreement of experts were finally selected.
