루푸스신염 환자에서 항 SS-A/Ro항체의 다양성과 임상적 의의

전신성 흉반성 루푸스 환자들에서 항핵항체 (antinuclear antibody, ANA)등의 자가항체가 특징적임은 잘 알려져 있다. 이 때 관찰되는 자가항체는 매우 다양하지만 그 중에서도 항 SS-A/Ro항체가 림프구 감소증, 광선 과민성 피부염, 폐질환, 신질환과 관련 있다고 한다. 항 SS-A/Ro항체는 Immunoblot법으로 60kDa과 52kDa의 서로 다른 두 가지 항원에 대해 반응한다는 것이 알려져 있고 이 반응양상에 따라 임상양상이 다를 수 있겠다. 본 연구에서는 루푸스신염 환자에서 항 SS-A/Ro항체 반응의 다양성을 알아보고, 그 반응성에 따라 임상양상의 차이가 있는지를 보고자 하였다.

루푸스신염 환자 19명의 혈청을 효소면역법으로 항 SS-A/Ro 항체의 유무를 검사하였다.

효소면역법으로 항 SS-A/Ro 항체 양성인 환자에서 항체의 다양성을 보기 위해 정상 인림프구와 HeLa 세포의 추출액을 항원으로 sodium dodecyl salfate - polyacrylamide gel electrophoresis (SDS-PAGE)하여 nitrocellulose막에 전사한 것을 환자의 혈청과 반응시켜

검사하였고, 임상양상의 차이를 보기 위해 각 환자의 혈액요소질소 (BUN), 크레아티닌, 크레아티닌 제거율 (creatinine clearance), 24시간 요단백, C3, C4, circulating immune complex (CIC), 감마글로불린 (gamma globulin), IgG, IgA, IgM, 혈색소치, 백혈구수,

혈소판수, rheumatoid factor (RF), C-reactive protein (CRP)등을 조사하였다. 정상 인림프구와 HeLa 세포를 각각 항원으로 사용하여 그 결과를 비교하였고, 그 결과가 일치하지 않는 것으로 나타나 두 항원의 결과를 종합하여 분석하였다.

항 SS-A/Ro항체 반응은 52kDa과 60kDa 항원에 모두 양성인 환자가 10명(53%)으로 가장 많았고 60kDa 항원에만 양성인 환자는 7명 (37%), 52kDa 항원에만 양성인 환자와 두 항원에 대해 모두 음성인 환자는 각각 1명 (5%)씩 이었다.

항 SS-A/Ro항체의 반응성이 60kDa 항원에만 양성인 환자군과 60kDa과 52kDa 항원에 모두 양성인 환자군을 비교했을 때 크레아티닌과 크레아티닌 제거율에는 유의한 차이가 없었고 (p>0.05), 혈액요소질소와 24시간 요단백은 60kDa 항원에만 양성인 환자군이 60kDa과 52kDa 항원에 모두 양성인 환자군보다 높았고 통계적으로 유의한 차이가 있었다 (p<0.05). 그러나, 저보체혈증 (hypocomplementemia), CIC, 고감마글로불린혈증 (hypergammaglobulinemia), 빈혈, 백혈구 감소증, 혈소판 감소증, RF, CRP는 유의한 차이를 보이지 않았다 (p>0.05).

이상의 결과로 immunoblot로 항 SS-A/Ro항체를 검사할 때 최소한 2종류 이상의 항원세포를 사용하는 것이 좋겠고, 항 SS-A/Ro항체 양성인 루푸스신염 환자에서 가장 흔히 검출되는 것은 항 60kDa SS-A/Ro항체이었고, 항 52kDa SS-A/Ro항체는 루푸스신염의 정도와 큰

연관성이 없는 것으로 생각되었다.

[영문]

Autoantibodies to a wide variety of cellular constituents have been reported in systemic lupus erythematosus (SLE). Among these, anti-SS-A/Ro is known to be associated with lymphopenia, photosensitive dermatitis, pulmonary diseases, and renal diseases. When the sera containing anti-SS-A/Ro were analyzed by immunoblot, two bands were detected (at 60kDa and 52kDa). This study was designed to investigate the prevalence of anti-SS-A/Ro subtypes in lupus nephritis and its possible correlations with clinical manifestations.

Patients with lupus nephritis who visited the department of Internal Medicine at Severance Hospital during the past four years were evaluated. The typical lupus nephritis patients were chosen for the presence of anti-SS-A/Ro by ELISA. For immunoblot, extracts of HeLa cells and normal lymphocytes were used for antigen substrates and separated on 10% SDS-polyacrylamide gels and electrotransferred to nitrocellulose sheets.

Then, the nitrocellulose strips were incubated with chosen patients' sera. In order to correlate the immunoblot data with patients' clinical status, blood urea nitrogen (BUN), creatinine, creatinine clearance, 24hr urine protein, C3, C4, circulating immune complex (CIC), gamma globulin, IgG, IgA, IgM, hemoglobin (Hb), WBC count, platelet count and the presence of rheumatoid factor (RF) and C-reactive protein (CRP) at the time of anti-SS-A/Ro positivity were reviewed.

Of the nineteen anti-SS-A/Ro positive sera, ten (53%) were positive for both 60kDa and 52kDa, whereas 7 (37%) were positive for only 60kDa. One patient was positive for only 52kDa. The BUN and 24hr urine protein were significantly higher in only 60kDa positive group than both 60kDa and 52KDa positive group (Mann-Whitney test, p<0.05). But, creatinine, creatinine clearance, C3, C4, CIC, gamma globulin, IgG, IgA, IgM, Hb, WBC count, platelet count and the presence of RF and CRP showed no significant difference between groups (χ**2_test, p>0.05).

The data suggest that 60kDa anti-SS-A/Ro was the most commonly found in anti-SS-A/Ro positive lupus nephritis patients and that the 52kDa antigen might be not associated with severity of lupus nephritis.
