High sucrose diet 백서의 상피부착부 변화에 대한 주사 및 투과전자현미경적 연구

본 연구는 치주질환의 진행에 따른 치아와 치은을 연결하고 있는 상피부착부의 반교소체와 기저층의 변화와 치아와 접하고 있는 부착상피의 표면형태변화를 시간경과에 따라 관찰하기 위하여 백서 50마리를 각각 대조군과 실험군으로 나누어 일상사료와 고당식을

투여한후 각각 3,7, 14, 21, 35일에 희생시켜 투과 및 주사전자현미경관찰을 위한 표본을 제작한 후 시간경과에 따른 상피부착부를 관찰한 결과 다음과 같은 결론을 얻었다.

1. 대조군에서 전실험기간을 통해서 부착상피는 치아와 반교소체와 기저층으로 연결되었으며 상부와 하부는 비교적 중간부위보다 반교소체수가 적었으며 최상부세포는 다소 차이는 있었으나 변성변화를 보이고 있었다. 주사전자현미경상에서는 세포표면은 피막양물질로 덮혀있었으며 미세융기는 관찰되지 않았다.

2. 실험3일후 상피부착부의 형태는 대조군과 특이한 차이는 없었으나 부착상피 세포간극내에 초기염증에서 볼 수 있는 다형핵백혈구가 증가하였다.

3. 실험7일후 투과전자현미경상에서 반교소체의 형태변화가 관찰됐으며 기저층은 상부에서 하부까지 존재하였다. 주사전자현미경상에서 세포간극의 확장이 관찰되었고 최상부에서는 다형핵백혈구가 많이 유출되고 있었으며 세균탐식도 관찰되었다.

4. 실험21일후 최상부에서는 계속 변성변화를 보이고 있었으며 다형핵백혈구들이 열구내로 유출되는 양상을 보였다. 기저층도 불분명하거나 소실되었고 반교소체도 볼수 없었으며 치아에 인접한 상피세포막이 파괴되고 세포질병변도 관찰되었다. 주사건자현미경소

견은 상피세포들이 서로 평면을 이루었던 구조적 특성을 잃고 분해되었고 곳곳에 염증세포들의 유출로 보이는 작은 구멍이 관찰되었다.

5. 실험35일에 최상부의 상피층은 일부 궤양화되었고 세균들이 이따끔 발견되었으며 기저층과 반교소체는 거의 소실되었고 염증세포들이 유출되는 곳에 상피세포의 파괴가 보였다. 주사전자헌미경 소견은 상피세포들의 고유표면특성을 잃고 치주낭상피로 변형되었다.

[영문]

The purpose of this study was to observe, the alterations of the hcmidesmosomes and basal-laminas on the epithelial attachment by which the tooth is connected with the gingiva, and the alterations of surface features of the juncti onal epitheliums

attached to the tooth, in regards to the elapsing of experimental time.

In this experiment,50 white rats were used. They were divided into two groups - CONTROL and EXPERIMENTAL, control group was fed conventional diet, experimental group was fed high sucrose diet.

4 rats from the control group and 6 from the experimental group were sacrificed on the 3rd,7th,14th,21st,35th experimental days.

Each specimen was prepared for Scanning and Transmission electron microscopic observation. The results of this study were as follows;

1. On the control group, the tooth and the junctional epithelium (J.E.) were connected by the basal lamina (B.L.) and the hemidesmosome (H.D.). The most coronal portion was showed the alteration with little differences. On the S.E.M.

observation, the surfaces of the epithelial cells seemed to be covered with a thin memberane, but not the microridge.

2. On the 3rd experimental day there weren't significant differences between the two groups in regard to the J.E., but there was an increase in the number of polymorphonuclear leukocyte(P.M.N.) within the epithelial cell of the J.E.

3. After one week, on the T.E.M. observation, there was an alteration of the H.D., B.L. was observed from the coronal to the apical portion. The S.E.M. observation skewed widening of the intercellular space within the most coronal portion and the emigration of numorous P.M.N and their phagocytosis.

4. On the 21st day, the most coronal portion has resealed alteration. The emigration of P.M.N. through the epithelial cell of the J.E. showed, the B.L. lost its significant characteristics, could not almost be seen, the H.D. was only slightly detectable. The membrane of the epithelial cell clone to the tooth surface was disrupted, and a degeneration of its cytoplasm was observed. On the S.E.M. study, the epithelial cell lost the flattened structure characteristic and was exfoliated and disrupted everywhere on the surface of the J.E.. Holes of different sizes through which inflammatory cells emigrated could be seen.

5. On the 35th day, almost all of coronal cells were almost ulcerated, and bacterial cells were sometimes seen within the epithelium, the B.L. and H.D. were almost non existent and epithelial cells were destroyed by contact as the inflammatory cells emigrated. On the S.E.M. observation, cell morphology was

completely disintegrated, even microridges which were peculiar surface characteristics of most of the epithelial cells, were lost. The J.E. was transformed into the pocket epithelium.
