저자는, 각기 다른 임상적 특징과 진행을 보이는 치주질환시 치은 상피의 표면구조가 질환별로 차이가 있는지 알아보고, 차이가 있다면 이를 질환의 본태와 병인, 진행과정을 규명하고 질환을 진단함에 있어서 현미경적 진단의 새로운 방법으로 응용할 수 있는지의

여부를 알아보기 위하여, 임상적 진단이 비염증성 다이란틴 치은 증식증, 재발성 아프타성 치은 궤양, 박리성 치은염, 진행된 치주염과 치주 농양으로 내려진, Y대학교 치과 대학 병원 치주과에 내원한 환자 25명을 대상으로, 치주 치료를 하지 않은 상태의 병고부위 가운데 각화성 치은 부위를 선택하여 주사 전자 현미경으로 관찰하여 다음과 같은 결론을 얻었다.

1. 세포 형태는 치은 증식증에서는 정상과 유사한 사각형 또는 다각형, 재발성 아프타성 치은궤양에서는 세포가 탈락되는 듯하나 세포 주위 경계가 편평한 장방형, 박리성 치은염에서는 방추형의 다각형 또는 마름모 모양을 이루어 정상과 유사하게 편평하였고, 진

행된 치주염에서는 세포간 경계가 불분명한 볼록한 형태의 타원형 또는 다각형, 치주 농양에서는 세포주위 경계가 말려 올라가며 탈락되는 장방형을 이루어 불규칙한 표면구조가 관찰되었다.

2. 세포의 크기는 한변의 길이가 정상은 10∼3Oμ, 진행된 치주염은 18∼23μ, 치은 증식증은 18∼29μ, 박리성 치은염은 15∼4Oμ, 치주 농양은 27∼40μ이며, 재발성 아프타성 치은궤양은 40∼42μ으로 관찰되었다.

3. 세포의 표면 형태는 정상에서 미세융기가 pitted appearance를 이루어 벌집모양과 같은 양상을 이루고, 치은 증식증은 정상과 거의 유사하나 일부에서는 지 문 형태를 나타내 는 것으로 보였다. 진행된 치주염은 미세융기가 서로 엉겨 또다른 커다란 형태의 벌집모양을 이루고, 재발성 아프타성 치은궤양은 분화구모양의 함몰 형태가 많이 관찰되었으며, 치주 농양에서는 미세융기가 분해되어 microvilli 모양으로 불규칙하게 세포 표면에 솟아 있는 것이 관찰되었다.

4. 박리성 치은염에서의 세포 표면 형태는 세포의 일부중, 미세융기가 분해되어 물집모양(bleb) 또는 microprocess의 형태를 이룬 세포들이 특징적으로 많이 관찰되었다.

5. 치주 농양과 진행된 치주염에서는 다양한 형태의 적혈구, 섬유소 잔사, 세균 및 염증세포가보였으며, 특히 치주 농양의 누공 부위에서는 결체조직이 노출되어 있음이 관찰되었다.

[영문]

The purpose of this study was to evaluate the differences of the gingival surface characteristics among the several types of periodontal disease.

Twenty-five individuals clinically diagnosed as normal gingiva, dilantin hyperplasia, recurrent aphthous ulcer, desquamative gingivitis, advanced periodontitis and periodontal abscess were selected for the study.

Mainly keratinized gingival surface lesions were examined in scanning electron microscopy.

The results were as follows:

While in normal gingiva, gingival epithelia were square or polygonal shape and divided markedly by raised intercellular border, in dilantin hyperplasia, gingival epithelia were square or polygonal shape similar to normal gingiva, in recurrent

aphthous ulcer, rectangular shape with exfoliating and flat cell periphery and in desquamative gingivitis, spindle-like polygonal or diamond shape. Gingival epithelia of the above had even surface similar to normal gingiva.

In advanced periodontitis, gingival epithelia were convex oval or polygonal shape and had obscure intercellular border and in periodontal abscess, rectangular shape with exfoliating and roiling cell periphery like fallen leaves. Gingival epithelia

of the above had not even surface.

2. While in normal gingiva. gingival epithelia were 10-30μ in length, in advanced periodontitis, gingival epithelia were 19-23μ in length, in dilantin hyperplasia, 18-29μ in length, in desquamative gingivitis, 15-40μ in length, in periodontal abscess, 27-40μ in length and in recurrent aphthous ulcer. 40-42μ in length.

3. While in normal gingiva, gingival epithelia had a honeycomb appearance or pitted appearance and microridges were observed apparently, in dilantin hyperplasia, some of gingival epithelia were similar to normal gingiva and some had finger print appearance, in advanced periodontitis, gingival epithelia had another larger honeycomb appearance than normal gingiva and microridges fused with each other in recurrent aphthous ulcer, gingival epithelia had many crater-like depressions and in periodontal abscess, gingival epithelia had irregularly grown up

microvilli due to disintegration of microridges.

4. In desquamative gingivitis some of gingival epithelia had characteristic blebs or microprocess due to disintegration of microridges.

5. In periodontal abscess and advanced periodontitis, variable forms of erythrocytes, fibrin remnants bacteria and inflammatory cells were observed and especially connective tissues were observed in periodontal abscess.
