만성치은염 및 치주염 환자에 Modified Keyes 방법과 치은연하소파술의 효과에 대한 비교연구

저자는 만성치은염 및 치준염환자에 대해 치은연하소파술과 Keyes 방법간의 치유효과를 비교, 연구하기 위하여 23∼44세의 치주과 내원환자 22명을 선택하였다.

이들을 치주낭 깊이에 따라 3군으로 나누고 (제Ⅰ군: 치은염, 치주낭깊이 3mm이내, 제Ⅱ: 치주염, 치주낭깊이 3∼5mm, 제Ⅲ군: 치주염, 치주낭깊이 5mm이상), 치석제거한 후 우측치은연하소파술을 행하고, 좌측은 Keyes방법을 시행하여 치석제거전, 치료전, 1주, 2

주, 3주, 4주에 각기 치은염지수와 치태지수, Sulcus Bleeding Index (SBI), 암시야현미경의 치은연하치태의 세균분포를 비교분석하여 다음과 같은 결론을 얻었다.

1. Ⅰ군에서 세균분포비율, SBI의 변화도는 두방법 유사하였으며, 치태지수와 치은염지수는 Keyes 방법을 한 측이 높은 감소율을 보였다.

2. Ⅱ군에서 비운동성 세균분포비와 SBI는 두방법 모두 유의성있는 변화를 보였으며 Spirochetes는 치은연하소파술을 시행한 측에서 더욱 많이 감소하였다.

3. Ⅲ군에서 비운동성 세균분포비의 증가와 SBI의 감소는 치은연하소파술을 행한 측이 많은 변화를 보였고, 치은염지수와 치태지수는 두방법 유사하였다.

The purpose of this study was to compare the treatment effects between subgingival curettage and Keyes method to gingivitis and periodontitis. 22 volunteers were selected to participate in this study and divided into 3 groups.

GroupⅠ: 8 gingivitis with probing depth 3mm or less,

GroupⅡ: 8 periodontitis with probing depth 3-5mm,

GroupⅢ: 6 periodontitis with probing depth 5mm or more.

1 week after having done scaling, subgingival curettage was done one side and Keyes method the other side. After 1, 2, 3, 4 week, treatment effects; gingival index (GI), plaque index(PI), sulcusbleeding index(SBI) and dark field microscopic bacterial study were recorded.

The result of this study were as follows:

1) In Group Ⅰ, no statistically significant differences in the changes of microflora and S B I were found between two methods, while there were more reduced of PI and GI in Keyes method sites than the subgingival curettage sites.

2) In Group Ⅱ, significant changes in nonmotile bacterial percentages and SBI. were found on both methods. But, spirochetes were more reduced in subgingival curettage sites than Keyes method sites.

3) In Group Ⅲ, significant increase in nonmotile bacterial percentage and reduction of SBI. were recorded in subgingival curettage sites, but there were no differences in GI and PI.
