치과대학 부속병원의 외래 및 입원환자의 내원실태 및 그 특성에 대한 임상적 연구는 비교적 활발하나 응급환자에 대한 연구조사는 드문 실정이다. 응급환자의 주종을 이루는 악안면 외상 및 급성감염, 치조와 출혈, 악관절 탈구등에 대한 임상적 연구에서 악안면 외상, 특히 안면골 골절자체에 대해서는 국내, 외에서 손상원인, 빈도 및 치료에 대해 활발하게 이루어져 왔으나, 그외의 질환에 대해서는 미흡한 실정이며 특히 야간 응급환자에 대한 조사는 드물어 이에 저자는 1980년 1월에서 1984년 12월까지 만 5년간 연세대학교
치과병원에 내원한 야간 응급환자 1,029명을 내원한 주소(chief complaint)에 따라 외상환자군, 급성 치성감염환자군, 치조와 출혈환자군, 하악관절 탈구환자군, 난발치 환자군 및 기타환자군의 6군으로 나누고 각군에 대해 지역, 발생빈도. 손상내용 및 처치내용등에 관한 임상적 연구를 시행하여 다음과 같은 결론을 얻었다.
1. 총내원 응급환자의 연도별 분포는 1980년부터 1982년 까지는 급격히 증가하는 양상을 보였고 연평균 증가율은 22%였으며, 월별빈도는 5월이 113예(11%), 10월이 110예(10.7%)순으로 높았으며 연령별 분포는 생후 8개월에서 80세사이로 다양하나 20대가 333예(32.
4%)로 가장 많았으며 남녀비는 2.3 : 1이었고 주소에 따른 빈도는 외상(76.3%), 치조와 출혈(7.5%), 급성 치성감염(7.3%) 순이었으며, 지역별 발생빈도는 병원인접지역인 서대문구, 은평구, 마포구가 402예 (39.0%)였다. 또한 1차진료기관 경유환자는 279예(27.1%)였다.
2. 외상군에서의 연령 및 성별빈도는 20대가 245예(31.2%)로 가장 많았으며 남녀비는 2.6 : 1이었고 원인으로는 추락(43.2%), 폭력(27.6%), 교통사고(21.9%)순이었고, 특히 12세 이하 소아에서는 추락이 주원인이었다. 또한 연조직 손상은 상, 하순 및 치은에 호발
하였고, 단순열창(85.4%)이 빈발하였으며, 치아 및 치조골 손상은 주로 상악전치부(67.6%)에 발생하였고 치아탈구(48.4%)가 가장 많았다. 안면골 골절은 하악골(76.1%), 관골 및 관골궁(14.4%), 상악골(8.0%)순으로 호발하였고, 하악골 골절은 정중부(50.5%) 및 우각부(20.6%)에서 빈발하였으며 안면골 골절의 처치중 관혈적 정복술은 77예(42.3%), 비관혈적 정복술은 56예(30.8%) 였으며 49예(26.9%)는 검진만을 받았다.
3. 급성 치성 감염군에서는 급성 봉와직염이 41예(54.7%)로 빈발하였고 원인치아로는 하악 제3대구치(32%)가 가장 높은 빈도를 보였다.
4. 치조와 출혈군의 출혈원인은 주로 치과술식후에 발생한 이차적출혈(79.2%)이었고 처치는 압박지혈(47.8%)이 가장 많았다. 난발치군 18예는 모두 1차진료기관에서 발치도중 내원하였으며 이중 하악 제3대구치가 15예(83.3%)로 대부분이었다.
5. 악관절 탈구군 27예중 20대가 11예(40.7%)로 빈발하였으며 기타군에서는 급성치수염(70.2%)이 빈발하였고 원인치아는 제1대구치(33.3%)가 가장 많았다.
[영문]
This is a retrospective study on nighttime emergency patients.
The study was based on a series of 1,029 patients treated as nighttime emergency patients at dental hospital, Yonsei medical center from Jan. 1, 1980, to Dec. 31, 1984.
The obtained results were as follows:
1. Yearly increased emergency patients rate was 22% and the highest monthly distribution showed in October. The age distribution ranged from 8 months to 80 years with the peak incidence occurring in the 20- to 30- year age group (32.4%)
and the ratio of male to female was 2.3 : 1.
Trauma (76.3%) was the most frequent cause in nighttime emergency and dentoalveolar hemorrhage (7.5%) and acute odontogenic infection (7.3%) were next in order of frequency.
According to the analysis by the districts, 39.5% of the total patients resided in Seo Dai Mun ku, Ma Po ku, and Eun Pyung ku near this hospital. 72.9% of the total patients visited directly to the emergency room.
2. In the trauma group, peak incidence occurred in the 20- to 30- year group (31.2%) and the ratio of male to female was 2.6 : 1.
Falls(43.2%) was the most frequent cause, followed by fight(27.6%) and traffic accident(21.9%).
Frequent sites of soft tissue injury were upper and lower lip and gingiva. Simple laceration(85.4%) was the most common type of soft tissue injury. As to the teeth and alveolar injury, the most common site was the upper anterior region(69.6%).
Luxation of tooth(48.4%) was the most common type.
The most common site of facial bone fractures was mandible(76.1%), zygoma complex(14.4%), maxilla(8.0%) in order. Frequent fracture sites of mandible were symphysis(50.0%) and angle region(20.6%). 77 cases(42.3%) were treated by open
reduction, 56 cases(30.8%) were treated by closed reduction and 49 cases(26.9%) were examined only.
3. In the acute odontogenic infection group, acute cellulitis(54.7%) was the most common for the nighttime emergency call and the infection of lower 3rd molar(32%) was the most common etiologic factor.
4. In the dentoalveolar hemorrhage group, a major cause was postoperative hemorrhage(72.9%) and hemostasis was obtained by pressure dressing mainly(46.5%).
All 18 cases of the complicated exodontics group were transferred from local dental clinics for the failure of removal of fractured roots and condemned tooth was the lower 3rd molar(83.3%) mostly.
5. In the TMJ dislocation group, the peak incidence showed in the 20- to 30- year group(40.7%) and acute pulpitis(70.2%) was the most common and its frequent site was 1st molar(33.3%) in other group.
