정신분열증의 양성, 음성증상에 따른 추체외로증상의 비대칭성

본 연구는 정신분열중환자에서 항정신병 약물 투여후에 나타나는 추체외로 증상이 좌, 우측 체부에서 비대칭성을 보이는지를 밝히고 양성증상과 음성증상이 신체좌, 우의 추체외로증상에 대하여 어떤 관련성이 있는지를 밝히는데 목적이 있다.

30명의 정신분열중 환자를 대상으로 최소한 일주일 이상 항정신병 약물을 투여받지 알은 상태, 항정신병약물을 투여한 1주일후 그리고, 3주일후등 세차례에 걸쳐 정신분열증의 양성증상과 음성증상 및 추체외로증상의 정도를 평가하였다. 추체외로중상은 좌, 우측

체부에 대해 각각 평가되어 좌, 우측에 차이가 있는지 그리고 양성 및, 음성증상의 심한 정도가 좌, 우측 체부에서 관찰된 추체외로증상의 정도와 어떤 상관성이 있는지를 통계분석 하였다.

그 결과는 다음과 같다.

1, 항정신병 약물을 투여한 1주일 및 3주일후 모두에서 신체의 좌, 우측에 나타나는 추체외로 중상점수에 있어 우측이 약간 높았으나 통계적으로 유의한 차이는 아니었다.

2, 항정신병 약물을 투여한 1주일후에는 양성중상 점수나 음성중상 점수모두에 있어 신체의 좌, 우측체부 각각의 추체외로증상 점수와 유의한 상관관계가 없었다. 그러나 항정신병 약물을 투여한 3주일후에는 양성증상 점수가 높을수록 오른쪽 추체외로중상 점수만이

유의하게 높아졌다. 반면음성중상 점수가 높을수록 왼쪽, 오른쪽 모두에서 추체외로중상 점수가 높아짐이 발견되었다

이와 같은 연구결과는 정신분열증의 음성증상은 추체외로증상에 있어 양측 대뇌반구 모두의 병적인 과정과 관련된 반면 양성증상의 경우 비대칭적인 장애와 관련이 있음을 시사한다.

This study was designed to test a hypothesis that positive symptoms is related with the asymmetry in extrapyramidal symptoms caused by antipsychotic medication, while negative symptom is not. Thirty schizophrenic patients were examined for evaluation of positive, negative symptom with PANSS and extrapyramidal symptoms in right and left extremities after one and three weeks treatment with antipsychotic drugs.

Results were as fellows :

1. The extrapyramidal symptoms of right side of the body were a little severer than those of left side in both evaluation time, however the difference was not statistically significant.

2. One week after neuroleptic medication, there were no correlations between positive or negative symptom scores and extrapyramidal symptom scores in each side of body

3. Three weeks after neuroleptics medication, positive symptom scores were significantly correlated with the scores of the right side extrapyramidal symptoms but not with those of the left side, The negative symptom scores were significantly correlated with the scores of both the right and the left sides,

These findings suggest that negative symptoms of schizophrenia are related with the pathologic process of both brain hemispheres,while positive symptoms are related with asymmetrical pathologic process in hemispheres.
