불인통에 대한 경피적 고주파 척수측색로 절단술

최근 의학의 발전으로 악성종양 및 만성질환의 생존율이 크게 향상되게 되고 따라서 이들 환자들의 생존기간동안의 통증치료가 관심의 대상이 되고 있다.

경피적 고주파 척수측색로 절단술은 1965년 Rosomoff에 의해 시도된 이 후 비교적 간단하고 안전하게 시술할 수 있는 불인통 치료의 대표적인 수술적요법으로 인정 받았음에도 불구하고 새로운 진통제의 개발과 다양한 수술요법 등의 발전으로 그 시술 회수가 점차 감소하는 추세이다.

이에 저자는 1973년 1월부터 1993년 3월까지 연세대학교 부속 세브란스 병원에 불인통으로 인하여 경피적 고주파 척수측색로 절단술을 받기위해 입원한 185명에 대하여 총 215회의 시술후 치료결과를 분석하여 다음과 같은 결과를 얻었다.

1. 전체 185명의 환자중 악성종양에 의한 불인통은 161명으로 이 중 폐암이 53예(32.9%)로 가장 많았으며 만성질환에 의한 불인통 환자는 24명으로 이중 환상지통과 절단부위통증이 각각 6예(25%)로 가장 많았다.

2. 통증의 부위별 분포는 상하지에 국한된 경우가 93예(50.2%)로 가장 높은 빈도를 보였으며, 다음으로 흉부(21.6%), 복부 및 회음부(21.2%)의 순이었다.

3. 전체 215회의 시술직후의 결과는 만족(Good)이 79.6%, 보통(Fair)이 11.6%로 전체적으로 90% 이상에서 시술직후 통증의 완화에 도움이 되었으며 악성종양에 의한 불인통 환자의 경우가 만성질환에 의한 불인통 환자의 경우보다 결과가 좋았다.

4. 통증의 부위에 따른 치료결과는 악성종양에 의한 불인통의 경우 상하지에 통증이 있었던 환자들에게서 만족이 89.0%로 가장 좋은 결과를 보였으나 만성질환에 의한 불인통 환자에서는 예수가 적어 부위별 차이점을 발견할 수는 없었다.

5. 수술후 합병증은 운동장애와 배뇨장애가 46예(21.4%), 및 41예(19.0%)로 높은 빈도를 보였으나 대부분 일시적이었고 3주이내에 호전되었으며, 영구적인 장애를 남긴 경우는 각각 6예(2.8%)와 5예(2.3%)였으나 경한 정도였다.

6. 폐기능검사상 수술후에 전반적으로 폐의 기능이 저하되는 소견을 보였으나 위험한 정도는 아니었으며, 수술후 15예(7.0%)에서 호흡곤란이 있었으나 모두 경비캐뉼라를 통한 산소흡입만으로 호전되었다.

7. 악성종양에 의한 불인통 환자에서 수술후 통증이 재발되었다고 판단되었던 경우는 7예(3.8%)로 매우 낮았으며, 만성질환에 의한 불인통 환자에서는 5년후에 모두 통증이 재발되기는 하였으나 상당기간 통증의 완화에 도움이 되었다.

이상의 결과로 보아 경피적 고주파 척수측색로 절단술은 악성종양에 의한 불인통 환자의 단기간동안의 통증완화에는 매우 좋은 결과를 보였으며, 만성질환에 의한 불인통 환자에게도 상당기간 만족할만한 통증완화를 얻을 수 있었다.

비록 최근 이 술식의 시술 회수가 감소하고 있기는 하나 이 술식이 국소마취하에 비교적 간단하게 시술할수 있으며 안전하고 환자에게 경제적 부담을 적게 줄수 있다는 점으로 볼때 아직도 일부 불인통 환자에게 제통목적으로 사용할 수 있는 매우 효과적인 방법으로 생각되어진다.

[영문]

Intractable pain has been a continuing problem in neurosurgery and many methods have been employed in its treatment.

Since percutaneous cordotomy was introduced by Mullan et al. in 1963, and modified by Rosomoff et al. in 1965, this method had been used as one of the most popular surgical one to control intractable pain. However, this method has been less frequently applied recently because of the development of new potent analgesics and other surgical methods.

The authors analyzed 215 procedures on 185 patients, who suffered from intractable pain and treated at Yonsei Medical Center from January 1973 to March 1993.

The results were summarized as follows :

1. Malignant disease comprised of 161 cases and the lung Ca. was the predominant primary lesion(32.9%).

2. The most common site of intractable pain was extremities(50.2%). and followed thoracic area(21.6%), and abdomen and perineum(21.2%).

3. Immediate postoperative results showed 79.8% Good, 11.8% Fair, and 3.7% Poor. Overall favorable(Good and Fair) outcome was achieved over the 90% of series, and the outcome of patients with malignancy was better than benign one.

4. Intractable pain confined to extremities showed the best response to therapy(89.0%) in patients with malignancy, butt in patients with benigin chronic disease, there was no different outcome related to pain site.

5. Postoperative complications were motor weakness(21.4%), and bladder dysfunction(19.0%), but most of them were transient and disappeared within 3 weeks. Permanent disability was very low as 2.6%.

6. 15 patients(7.0%) had mild respiratory difficulty after operation, but all of them were improved without any specific treatment except oxygen inhalation.

7. Recurrence of pain in patients with malignancy was very low(3.8%) until discharge, but in patients with benign chronic disease, 13 patients who were able to follow were all recurred after 5 years, however this procedure was considered to be valuable for releiving the pain in some relative period.

In conclusion, percutaneous radiofrequency cervical cordotomy allows easy, effecient, less expensive and low risk relief of intractable pain.

Recently, although the number of the procedures diminished, this method was still useful in patients with intractable pain due to malignancy who requires short-term relief of pain and in some selected cases with benign chronic disease.
