가정간호사 인력수급에 관한 연구 : 만성질환 노인환자를 중심으로

dc.description.abstract[한글] 서기 2000년후 가정간호사업은 보건의료사업중 주요한 사업으로 전망되고 있다. 우리나라 정부도 1989년 6월 가정간호제도를 도입하기로 방침을 확정하였고 1997년 1월 9일 의료법 시행규칙을 개정하여 분야별 전문간호사에 가정간호사를 추가하였다. 가정간호사업의 성공적 수행을 위하여 바람직한 인력의 수급 및 배분은 불가결한 요소이다. 우리 정부가 의지를 가지고 가정간호사업을 추진하고 있음에도 불구하고 가정간호사 인력 수급에 대한 연구는 찾아볼 수 없었다. 우리나라 21세기 국민보건의료의 중요한 역할을 담당할 성공적인 병원중심 가정간호사업을 위해 꼭 필요한 요소인 가정간호사 수급에 관한 연구를 시도하였다. 가정간호사 수요분석과 추계를 위하여 양적 분석만을 포함하였는데 국민이 실재로 이용하는 의료이용량에 기반을 두는 건강수요방법을 적용하였다. 의료이용추계는 병(의)원 의료이용에 국한하여 이에 영향을 미치는 요인중 연령을 감안하여 유승흠, 손명세외(1992)의 연구에서 제시된 연령별 연평균 의료이용 증가률을 적용하였는데 이는 간호사 인력수급에서 사용하지 알았던 새로운 의료수요추계방법이다. 가정간호사업대상자인 만성질환 노인환자는 65세이상 노인입원환자의 32%로 하여 가정간호사의 1일 평균 방문 횟수는 4회, 한 환자당 사업이 종결될 때까지의 가정방문횟수는 5.5회로 하여 가정간호사 수요를 추계하였다. 가정간 호사공급은 양, 질, 분포의 분석을 포함하는데 양적 분석과 추계는 1994년 배출예정 인원인 330명이 암으로 그대로 유지되는 기초추계방법을 적용하였다. 질에 대한 분석을 가정간호사가 전문간호사로서의 자질을 갖출 수 있는지 검토하기 위하여 가정간호사외 자격취득기준, 가정간호교육기관의 지원자격, 교과내용을 분석하였고 3개 가정간호교육기관 교육생의 일반적 특성을 분석하였다. 분포에 대한 분석은 가정간호사가 지역적 불균형을 초래하지 않고 가정간호사업을 수행할 수 있는지 검토하기 위하여 가정간호교육기관과 배출생의 지역적 분포를 분석하였다. 1) 가정간호사업에 의뢰되는 65세이상 만성질환 노인환자의 연평균 대상자수는 1995년 147,785명, 2000년 342,416명, 2005년 793,374명, 2010년 1,838,237명으로 노인인구가 증가함에 따라 2010년에는 1995년보다 12.4배이상 증가하고 가정간호사 수요는 1995년 782 명, 2000년 1,811명, 2005년 4,196명, 2010년 9,721명으로 추계되었다. 2) 가정간호사 교육기관은 1993년 12월 현재 9개의 교육기관 중 1개 교육기관이 교육을 더이상 실시하지 않아 8개 기관이며 6개의 교육기관에서 총 346명의 교육생이 배출되었다. 연도별 가정간호사 자격취득자는 1991년 37명, 1992년 113명, 1993년 196명이었다. 가정간호사의 공급추계는 1994년 8개 교육기관의 배출예정인원 330명이 앞으로 그대로 배출된다고 하면 1995년 1,006명, 2000년 2,656명, 2005년 4,306명, 2010년 5,956명이 가정간호사 자격증을 취득할 것으로 추계되었다. 3) 가정간호사의 질 분석에 있어 가정간호 교육기관의 지원자격은 간호사 면허소지자로 되어있고 1개 기관만이 임상경력 1년이상인 자를 추가하고 있었다. 가정간호 교과과정도 국립대학교에서 운영하는 가정간호 교육기판은 보건사회부 고시내용을 그대로 따르고 있었고 1개 기관은 규정시간수는 보건사회부 고시내용과 일치하나 내용이 지역사회간호에 대한 것은 빠져있고 병원환자간호를 위한 기초내용, 간호연구, 가정간호 운영관리에 관한 내용이 첨가되어있었다. 3개 가정간호 교육기관에서 교육을 받았거나 교육을 받고 있는 교육생의 일반적 특성은 다음과 같았다. 연령은 최저 21세에서 최고 57세였으며 교육생의 73.4%가 29세이하 이었다. 최종학력은 대학원을 포함하여 대학이상을 졸업한 사람이 41.5%로 전문대학을 졸업한 사람보다 적었다. 졸업년도는 1981년이후 졸업한 사람이 56.4%이었다. 근무경력은 전혀 근무경력이 없는 사람에서 최고 23년까지 였으며 근무경력이 5년이상이 되는 사람이 68.5%이었다. 근무지 이동횟수는 전혀 이동하지 않은 사람에서 최고 5회까지 이동하였으나 전혀 이동하지 않은 경우가 40.8%로 가장 많았고 1-2회 이동한 사람도 41.9%이었다. 현 근무지는 병원인 경우가 53.3%로 가장 많았고 직업이 없는 경우도 20.D%나 되었다. 결혼여부는 65.5%가 기혼자이었다. 4) 가정간호사가 지역적 불균형을 초래하지 않는지 검토하기 위하여 가정간호 교육기관과 배출생의 지역적 분포를 살펴본 결과 가정간호 교육기관은 강원도, 충청북도, 제주도에는 설립되어 있지 않았고 8개의 교육기관중 6개가 서울, 부산, 대구, 대전, 광주의 대도시에 설립되어 있었다. 따라서 가정간호사 배출생도 이 교육기관들에서 89%가 배출되었다. 5) 65세이상 만성질환 노인환자를 위한 가정간호사의 수요와 공급비교에 있어 1995년 이후 2005년까지 공급과잉이 예상되며 노인인구가 증가함에 따라 2006년부터 가정간호사의 공급부족이 예상되었다. 종합병원 병상수를 기준으로 하여 전체 병상에 대한 각 지역 병상수의 비율대로 수요가 있다고 전제한 지역별 가정간호사 수요와 공급의 비교에서 지역적 불균형이 초래될 것으로 예상되었다. 결론적으로 65세이상 만성질환 노인환자만을 가정간호사업 대상자로 설정할 경우 1994년이후 2005년까지 가정간호사 공급과잉현상을 보이다가 노인인구가 증가함에 따라 2006년이후 가정간호사가 부족할 전망이었다. 그러나 이 연구에서 감안하지 않은 65세미만의 만성질환자, 모든 연령층의 급성질환자뿐만 아니라 재활환자, 말기환자를 위한 가정간호사 수요까지 감안한다면 오히려 공급부족이 예견되었다. 가정간호사업실시 초기부터 모든 연령층을 대상으로 할 것이 아니라 병원 입원수요를 증가시키는 65세이상 만성질환 노인 환자에게 시작하여 대상자를 점진적으로 확대하여야 한다. 그리고 어느 시점에 모든 연령층을 대상으로 할 것인지 결정하고 이에 따른 가정간호사 공급을 늘이는 계획이 수립되어야 할 것이다. 이상의 결과 및 결론을 기반으로 다음과 같은 제언을 한다. (1) 가정간호사업대상자를 병원입원수요를 증진시키고 있는 65세이상 만성질환 노인환자에게 시작하여 점진적으로 모든 연령층의 급만성 질환자로 확대해나가는 것이 요구된다. (2)가정간호사업대상자를 확대하는 시점을 결정하고 이에따른 가정간호사 공급을 늘이면서 지역별 수요와 공급의 불균형을 해소하는 가정간호사 인력계획이 수립되어야 할 것이다. (3)시행하고자 하는 가정간호사업의 방향과 전문간호사로서 적절한 가정간호사 인력을 준비시킬 수 있는 교과내용과 교육기간에 대한 재검토가 요구된다. (4) 병원중심의 가정간호사업을 실시할 경우 80%이상의 병원이 시부에 집중분포하므로 이 사업의 혜택을 받을 수 있는 군부지역 주민의 가정간호사업을 위해 보건진료원율 활용하는 방안이 모색되어야 한다. (5)가정간호사업에 필요한 가정간호사 인력의 정확한 산정을 위해 이용할 수 있는 자료의 장기적이고 체계적인 축적이 요구된다. Supply and Demand of Home Health Care Nurses for the Chronically Ⅲ Elders Ko, Ⅱ Sun Dept. of Nursing , The Graduate School, Yonsei University Home health care nursing will be an important area of health care delivery system in the year 2,000. The Korean government adopted the home health care nursing in 1989 and included home health care nursing as one of the areas of professional nursing in 1990. In order to have a successful home health care program, it is necessary to supply enough home health care nurses. There are no researches in supply and demand of home health care nurses in Korea. The purpose of this study is to analyze and estimate supply and demand of hospital based home health care nurses. Analysis of demand was done in quantitative aspect of home health care nursing. Quanitative analysis was done with health demand method based on utilization of health care service and productivity of nurses. Age factor wart considered in estimation of health care service utilization. Based on the selected number of patients, the clients of the home health care nurses are elderly people with chronic degenerative diseases. This study assumed that 32% of elders age over 65 are the clients of home health care service. Assumptions relates to the Productivity of home health care nurses are: 1) each home health care nurse can make 4 home visits a day; 2) each client needs average 5.5 home visits. Analysis of supply of home health care nurses was done in quantitative, qualitative, and distribational aspects. Quantitative analysis wart bases on current status of the training facilities for home health care nurses and the assumption of 330 home health care nurses will be trained every year. Qualitative analysis included: 1) licence criteria for home healthcare nurses ; 2) evaluation of the training facilities far home health nurses; 3) comparison of the existing curriculum and the one from the Department of Health and Social Affairs; 4) analysis of characteristics of the trainees in three training centers for the home health care nurses. In order to examine the balance of geographical distribution of home health care nurses, local origins of the training facilities and the trainees were analyzed. Summary of findings : 1. The number of the elderly clients referred to home healthcare nurses will increase dramatically as elderly population increases. The estimanted number of clients are 147,785 in 1995, 342,416 in 2000. 793,374 in 2005, and 1,838,237 in 2010. This means that the number of elders in need of home health care in the real 2010 will be 12 times that of 1995. Demand of home health care nurses are estimated as 782 in 1995, 1,811 in 2000, 4,190 in 2005, and 9,721 in 2010. 2. There are 8 home health care nurse training centers as of December, 1993. Total number of certified home health care nurses are 346. If we assume that the same number(330) of home health care nurses will he trained annually from 1994, it will be 1,006 in 1995, 2,056 in 2000, 4,806 in 2005, and 5,950 in 2010. 3. There are differences in the qualification criteria for the applicants among different home health nurse training centers. While only registered nurses can be trained as home health care nurses, one center required at least one year of clincal experience prior to the training. Also, curriculum are different among training centers. 4. About 73% of the trainees were below 29 years of age and 42%, of the trainees were college graduates. About 69% have more than 5 years of experiences in nursing. One half of the trainees current working place was hospital and 66% were married. 5. Six out of these 8 centers are located in Seoul. 89% of certified home health care nurses were trained in training centers located in Seoul, Pusan, Taegu, Taejun, and Kwanju. 6. If this trend of supply and demand continues, there will be an over-supply of home health cafe nurses from 1995 till 2005. However, as elderly population grows. there will be a shortage of home health care nurses in 2006. In conclusion, there will be a shortage of home health care nurses starting from 2006. This shortage will be even more dramatic when all other patients who could be benefited by the home health care services are included. For example, patients with acute health problems, patients in need of rehabilitative care and terminal patients who are not included in this study may be able to use the home health care services. However, the initial home health care services should be focused on the care of the elderly clients, then the home health care services should be expanded to other age groups or other age groups of care. According to these results and conclusion. the following implications can be made. 1) Initial home health care service should be focused on paticular age groups such as elderly clinents with chronic illness, it should be gradually expanded to other types and age groups. 2) It is necessary to set a policy guided me for home health care expansion. supply of home health nurses should be planned according to this policy guideline. 3) It is necessary to evaluate and reexamine the existing training program. 4) It is necessary to investigate possibility of utilizing existing health care service workers for home health care services in the rural area, the community health practitioners can be trained to provide home health care services in the rural area. 5) A long-term and systmatic data collection is necessary to better estimate and plan home health care services and supply enough home health care nurses.
