당뇨병은 만성 대사성 질환으로 질병의 조절은 가능하나 완치되지 않으므로 합병증을 예방하고 건강한 삶을 영위하기 위해서는 의료인의 지도하에 환자 자신이 질병관리를 할 수 있는 방법을 배우고 지속적으로 자가간호를 수행함이 필수적이라고 할 수 있다. 본 연

구에서는 학령기 아동 및 청소년의 행동에 중요한 영향을 미치는 자아존중감과 자가간호와의 관계를 규명함으로써 학령기 및 청소년기 당뇨환자의 건강관리지도 및 교육에 도움을 주기 위해 시도된 서술적 상관관계 연구이다.

연구방법으로는 1993년 10월 12일부터 11월 10까지 서울시에 위치한 2개 종합병원에서 통원치료를 받고 있는 학령기 및 청소년기 인슐린 의존성 당뇨환자 54명에게 우편을 이용한 질문지법을 실시하였다.

연구도구는 Coopersmith(1967)가 개발하고 김귀인, 장도현(1984)에 의해 번역된 자아존중감 측정도구를 사용하였으며 자가간호는 박오장(1984)이 개발한 당뇨환자 이행행위 측정도구를 수정 보완하여 사용하였다.

수집된 자료는 SPSS/PC+를 이용하여 전산통계 처리하여 실수, 백분률, 최소값, 최대값, 평균, 표준편차, t-test, ANOVA, 피어슨 적률상관관계 등의 통계방법으로 분석했다.

본 연구의 결과는 다음과 같다.

1. 대상자가 인지하는 자아존중감 점수범위는 27-75점으로 최소값이 33, 최대값은 74, 평균 54.57, 표준편차 7.50이었다.

2. 대상자가 인지하는 자가간호 점수범위는 14-70점으로 최소값이 37, 최대값은 69, 평균 52.80. 표준편차 7.58이었다.

3. 본 연구의 가설 "학령기 및 청소년기 당뇨환자들의 자아존중감이 높을수록 자가간호정도는 높을 것이다"는 유의하게 상관관계가 있는 것으로 나타나 가설이 지지되었다. (r=.5275, p<.001)

4. 대상자의 일반적 특성에 따른 자아존중감 정도에는 유의한 차이가 없었다.

5. 대상자의 일반적 특성에 따라 자가간호 정도는 연령에 따라 유의한 차이가 있는 것으로 나타났다. 8-13세군이 14세-16세군, I7-19세군보다 자가간호를 잘하는 것으로 나타났다. (F=3.41, p<.05)

6. 대상자의 자가간호정도에서는 내용별로 인슐린주사가 평균평점이 4.29로 가장 잘되었고, 혈당검사와 위생적인 생활이 각각 평균평점 3.63으로 가장 안된 것으로 나타났다.

본 연구결과를 통해 학령기 및 청소년기 당뇨환자의 자아존중감과 자가간호 사이에는 높은 상관관계가 있는 것을 알 수 있으므로 자아존중감을 높일 수 있는 교육방안이 개발되어야 하고 당뇨교육 프로그램에 혈당측정, 위생적인 생활 및 식품교환표에 의한 식이요

법에 관한 내용이 강화되어야함을 제언한다.

Because diabetes is a chronic metabolic disease which means that the disease is

not curable even it is controllable, it is necessary for patients to learn

management of the disease and implement self-care under the direction of health

care providers.

This study was conducted to identify the correlation between self-esteem and

self-care of young diabetics and to help educate them about health management.

For this study, questionnaires were mailed between October 12, 1993 and November

10, to 54 insulin-dependent young diabetics who were being treated in two general

hospitals located in Seoul in outpatient clinics.

Two management tools were used for this study. The first one was developed by

Coopersmith (1967) and translated by Kwi-in Kim and Do-hyun Chang (1984). For

Self-care, modified diabetic behavior questionnaire of Oh Chang Park was used.

With the collected data, 1 used SPSS to analyze the data using error, ratio,

maximum, minimum, mean, standard deviation, t-test, ANOVA, and Pearson correlation.

The result of the Study is as follows:

1. As for the degree of self-esteem perceived by respondents, within the range of

25 to 75, the minimum value was 33, the maximum was 74, the mean was 54.57, and the

standard deviation was 7.5.

2. Concerning the degree of self-care perceived by respondents, within the range

of 14 to 70, the minimum value was 37, maximum was 69, mean was 52.80, and the

standard deviation was 7.58.

3. The hypothesis of this study "The higher the decree of self-esteem of young

diabetics, the higher the degree of self-care" was supported. The study showed

meaningful correlation between these two characteristics.

4. It was not shown that the degree of self-esteem was statistically.

5. It was shown that the degree of self-care was statistical significant when

compareed to the general characteristics of respondents, such as age and grade.

6. For the decree of self-care by factors, the insulin injection was the highest

scored item on the questionnaire with the mean of 4.29.

The bloodsugar level score and personal hygiene score were the lowest with the

mean of 3.63.

This study demonstrated that there exists a high correlation between the

self-esteem and self-care of young diabetics. In the future, we need determine the

variables which influence self-esteem.
