양악 전돌증 환자에서 소구치 발치를 통한 교정치료시 입술 주위 연조직 변화에 관한 연구

안모의 심미성은 교정치료를 받고자 하는 중요한 동기중의 하나이며, 또한 교정치료의 궁극적 목적 중의 하나이다. 따라서, 진단과 치료 계획의 수립시부터 안모의 심미성에 대한고려가 필요하며, 교정 치료에 의한 측모의 변화를 예측 하는 것이 치료 계획을 수립하

는데 필수적이라 할 수 있겠다. 본 연구에서는 성장의 효과를 완전히 배제하였을 때 교정 치료에 의한 연조직의 변화양상을 알아보기 위하여 입술의 돌출을 주소로 내원하여, 4개의 소구치를 발거한후 전치의 후방 견인을 통하여 입술의 돌출도 감소를 도모한 18세 이

상의 환자중, 총생의 양이 6mm를 넘지 않고, 1 to facial Plane이 7mm이상이며, Angle씨 제I급 구치관계를 보이는 여자 환자 87명의 치료 전후 측모 두부 방사선 계측 사진을 비교 분석하여 다음과 같은 결론을 얻었다.

1. 치아의 후방이동( 상악 ; 5.78mm 하악 ; 4,78mm )으로 인한 입술의 두께 변화는 각 부위에 따라 그 변화양상이 달랐으며, 홍순부( Redportion of lip)에서는 많은 량의 두께증가( 2.78mm )를, 그 이외의 부분(Ls,B)에서는 적은 량의 두께의 감소(0.65-0.7mm)를 보였다.

2. 치아의 후방이동으로 인하여 홍순부의 길이는 감소(1.3mm)하였다.

3. 입술의 피부부분( Cutaneous portion of lip )의 길이는 치아의 후방 이동에 따라 중간(2.9mm)하였다.

Facial esthetics is one of the most important goal of the orthodontic treatment

and main concern of many patients. Facial esthetics should be considered in

orthodontic diagnosis and treatment planning, Prediction of soft tissue profile

changes after orthodontic tooth movement should be considered as well. The purpose

of this study was to find out the effect of orthodontic treatment on lip profile in

adult patient. The pre- and post treament cephalometric reontgenograms of 87 female

adult with bimaxillary protrusion were used to analyze lip profile change. All

subjects were treated with four bicuspids extraction

Obtained results were as follows .

1. Lip thickness changes after incisor retraction showed different patters

according to areas of the lip, The thickness of the red lip area showed 2.78 mm

increase in average. In contrast the thickness of the cutaneous area showed 0.65 -

0.7 mm decrease according to the different cutaneous areas.

2. The length of the red lip area decreased(1.3mm) after incisor retraction.

3. The length of ire cutaneous lip area increased(2.9mm) after incisor retraction
