모성사망의 3대원인의 하나인 임신성 고혈압성 질환은 모체뿐 아니라 자궁태반기능저하를 일으켜 주산기 이환율 및 사망율을 높이는 중요한 질환으로서 끊임없는 연구에도 불구하고 아직까지도 그 발병원인 및 기전이 확실하지 않으므로 이에 대한 연구가 더욱 필요함은 주지의 사실이다. 임신성 고혈압성 질환이 일단 발병할 경우 모체와 태아에 치명적인 결과를 줄 수 있으므로 가장 중요한 것은 이에 대한 적절한 예방이 우선적이며 또 이미 발병된 경우에는 적절한 치료를 시행하여야 하나 아직까지의 치료는 확실한 원인에 기초를 두어 치료를 한 것이 아니라 지금까지 알려진 추정된 병인을 바탕으로 대중적인 요법으로 치료를 하여 왔으며(김등, 1978) 오래전부터 임신성 고혈압성 질환의 원인 및 발병기전이 연구되어 왔다. 1953년 Mckay등은 임신성 고혈압성 질환이 범발성혈관내응고병증(disseminated intravasocular coagulation)에 의하여 유발된다고 보고한 이래 많은 논란이 되어 봤으며 최근에는 범발성혈관내응고병증이 임신성 고혈압성 질환의 원인이라기

보다는 결과로 사료된다고 보고하고 있다(Pritchard et al., 1976) .이렇듯 임신성 고혈압성 질환의 원인 및 병인(pathogenesis)에 대하여서는 아직까지 불확실하며 여러가지 추정된 이론이 있으나 앞으로 산과영역에서 해결해야할 큰 과제라 하겠다. 더욱이 임신성고

혈압성질환을 가진 한국인산모에서 일차적이건 이차적이건 혈액 응고계의 변화가 어떠한지는 아직까지 알려진 것이 없었다. 이에 저자는 1982년 4월 1일부터 1982년10월31일까지 만 7개월간연세대학교 의과대학부속 세브란스병원 산부인파에서 분만한 임신중독증 중증

산모 35명, 임신자간증 산모 10명 및 합병증이 없었던 정상 임산부 10명을 대상으로 추적연구로서 진통직전이나 진통중에, 모체의 혈소판치, 혈장 피브리노젠(fibrinogen)치, 혈청 FDP(Fibrin Degradation Product) 치 및 프로트롬빈 (prothrombin)치를 측정 분석하여 다음과 같은 결론을 얻었다.

1. 연령별 분포는 임신중독증 중증 산모에서는 25 - 29세군이 60.0%로 가장 많았으며 임신자간증 산모의 경우 20 ∼ 27세군이 50.0%로 양군 모두 20 - 29세군에 대부분이 포함되었고 35세이상은 전예에서 단 1예가 있었다.

2. 분만횟수별 분포는 초산부가 임신중독증 중증 산모의 68.5%,임신자간증 산모의 80.0%로 가장 많았다.

3. 임신주수별 분포는 임신중독증 중증 및 임신자간증 산모 양군 모두 37 ∼ 40주가 각각 62.9%, 50.0%로 대부분을 차지하였다.

4. 혈소판치가 15만/㎖이하로 감소된 경우는 임신중독증 중증산모의 11.5%, 임신자간증 산모의 40.0%였다.

5. 혈장 피브리노젠치는 156 ∼ 538 ㎎/100㎖로 상당히 다양하였고 150mg/ 100㎖이하인 경우는 전 환자에서 1예도 없었다.

6. 혈청 FDP가 10㎍/㎖의 양성율을 나타낸 빈도는 임신중독증 중증 산모의 37.1%, 임신자간증 산모의 30.0%였다.

7. 프로트롬빈치는 양군 모두 정상범위로 9.2 ∼ 10.6 sec 이내였다.

8. 혈소판의 평균치는 임신중독증 중증산모가 235.4±83.7(×10**3/㎖), 임신자간증 산모 185.0±51.1(×10**3/㎖)로서 대조군의 248.6±58.8과 큰 차이가 없었다.혈장 피브리노젠치와 프로트롬빈치의 평균치역시 정상범위인 308.6±76.3㎎/100㎖, 10.1±0.6 sec로 대조군과 통계적으로 의의있는 차이를 볼 수 없었다.

9. 임신중독증 중증 산모에서 혈소판치 가 20만/㎖ 미만일 때 혈청 FDP치가 10㎍/㎖이상을 나타내는 경우가 58.3%였다

Hypertensive disorders in pregnancy remain the major cause of maternal and

perinatal morbidity and mortality. In sprite of the widespread advances In the

treatment of the disease in recent years, the etiology and pathogenesis were not

definetely known. Much earlier, Schmorl (1893) postulated that coagulation was the

factor in the production of the characteristic lesions of fatal eclampsia. Also

Page (1972) postulated that preeclampsia and eclampsia are both the consequence of

DIC.

But the lack of evidence for DIC in many women with hypertenstive disorders in

pregnancy indicated that changes In the coagulation mechanism, when present, were

the result rather than the cause. In order to study the coagulation changes of

hypertensive patients in pregnancy, a prospective study was performed. The platelet

counts, plasma fibrinogen, serum FDP & prothrombin time were checked out in 35

preeclampsia severeform, 1O eclamptic patients and 10 normal pregnant women, who

were delivered between April 1, 1982 and October 31, 1982 at the Yonsel University

Medical Center.

The results were as follows :

1. 60.0% of the Patients in preeclampsia severe form were in the 25-29, and 50.0%

in eclamptic patients were in the 20-24 year old.

2. 68.5% of the preeclamptic patients & 80.0% of the eclamptic women were the

primtgravida.

3. The 37-40wks of gestational weeks of preeclampsia & eclampsia patients were

62.9% and 50.05 respectively.

4. Platelet levels were decreased below the 150(×10**3/㎖) in 11.5% of the

preeclampsia and 40.0% of the eclamptic patients.

5. The plasma fibrin levels were much variable and also no case was found under

the values of 150㎎/1OO㎖.

6. 37.1% of preeclampsia and 30.0% of eclampsia had the values over 1O㎍/㎖ in

Serum FDP.

7. In mast of patients of both group prothrombin time were within normal

(9.2-10.6sec).

8. Mean platelet count, plasma fibrinogen and prothrombin time were within

normal.

9. 58.3% of the patients with the platelet values below the 200 (×10**3㎖) had

serum FDP above 10㎍/㎖
