본 논문은 연세대학교 의과대학 외과학 교실에서 1958년 1월 부터 1975년 12월까지 만 18년간 수술가료하였던 15세이하의 소아 284명의 급성충수염 환자에 대하여 발병율, 천공율, 임상소견, 치료경과와 합병증 등에 관해서 비천공군과 천공군간의 차이를 통계적으로

비교분석하여 다음과 같은 결론을 얻을 수 있었다.

1. 발병은 3세이전엔 드물고, 6세 이후 점차 증가하는 경향을 보였고, 봄과 가을에 비교적 많았으며, 남여의 비율은 1.6:1 이었다.

2. 천공군에서 의의가 있었던 것은 증상과 이학적 소견 및 검사소견 등이 일반적으로 현저히 중했으며 발병기간 수술후 재원기간 및 퇴원후의 내원치료 등의 이병기간이 길었고 사망률 은 없었으나 합병증이 많았다.

3. 병저의 진행이 성인에 비해 빠르며 의사표시불충분과 해부조직학적 차이 등으로 인해 높은 천공율 (34.5%)을 보였으며 천공율은 연령과 반비례하여 6세 이전에는 71%, 6세이후에는 30%이었다.

4. 진단은 적절한 진정 및 수면상태에서 시행하는 것이 좋으며 진단이 뚜렷치 않은 경우엔 입원시켜 주기적 관찰을 하는 것이 유리한데 이로 인하여 합병증과 이병율을 감소시킬 수 있었으며 사망율은 없었다.

An analysis of 284 cases of acute appendicitis in children under the age of 15

was carried out, which were treated at the Department of Surgery of Yonsei

University during the past 18 years period form Jan. 1958 to Dec. 1975.

Authers, having reviewed and analysed statistically the clinical features,

diagnosis, treatment, complications and morbidity applied to these cases, obtained

the results as followings.

1. The disease is rare under the age of 6, and showed progressive increase from 6

to 15 years old. Ratio between male and female was 1.6 : 1.

2. Most clinical features, laboratory findings, complications and morbidity were

more serious in the perforated group than non-perforated one and the differences

were significant statistically between two groups.

3. The disease in more fulminant in the children than adults and the over-all

perforation rate was 34.5% in the children, comparing with 15 to 25% of adult

appendiceal perforation and the perforation rate under the age of 6 was very high,

4. Early diagnosis, aided by moderate sedation and frequent examinations, can not

be over-emphasized, and the prompt and adequate treatments are essential to prevent

serious complications and the morbidity in the children with acute appendicitis.
