16, 16Dimethyl-Trans-△²-PGE₁-Methyl Ester 질정에 의한 초기 및 중기임신중절에 관한 연구

Prostaglandin E^^1의 유도체인 16, 16-Dimethyl-Trans-△2-PGE6^^1 Methyl Ester(16DM-PGE^^1)질정을 사용하여 초기임신 21예, 중기 임신 20예에 임신중절을 시행하였다.

임신중절 성공율(완전유산 및 불완전유산)은 초기임신중절군 21예중 완전유산이 10예(47. 6%), 불완전유산이 6예(28.6%)로의 성공율을 나타냈으며 중기임신중절군은 20예중 18예에서 완전유산 2예에서 불완전 유산이 되어 100%의 성공율을 보였다.

복부진통의 시작은 초기임신중절군에서는 평균 1시간 51분(Mean±SE : 1.85hr ± 0.2hr)부터, 질출혈은 평균 4시간 19분(Mean±SE ; 4.3hr ± 0.4hr)부터 시작되었고 중기임신중절군은 복부진통이 평균 59분(Mean±SE ; 59min ± 4.2min)부터, 중기임신중절에 소요

된 시간은 평균 13시간 54분(Mean ± SE ; 13.9hr ± 1.8hr)이었다.

초기와 중기임신중절군에 사용한 16DM-PGE^^1 질정의 부작용으로는 초기임신중절군 21에중 6예928.6%)에서 오심, 구토, 토사, 흉부동통 및 체온상승등이 나타났으며 39℃이상의 체온상승을 보인 1예를 제외하고는 모두 경한 부작용을 보였다. 중기임신중절군은 총 20예중 11예(55%)에서 구토, 토사, 상복부통증 및 페온상승등이 나타났고 심한 부작용은 4예에서 나타났다.

첫 질정 삽입 24시간 후 자궁경관 개대효과는 임산부 21예 모두 Hegar No.10이 저항없이 통과하여, 1.0㎝이상 자궁경관 개대가 되었음을 알 수 있었고 자궁경부의 연화로 쉽게 소파수술을 시행할 수 있었다. 또 초산부 10예에서 첫질정 삽입 3시간 후 자궁경관 개대

효과는 8예에서 8㎜이상 개대되었으며 나머지 2예에서도 5㎜이상 자궁경관의 개대와 자궁경부 연화가 되었다.

vaginal suppositories for early trimester termination and

midtrimester termination

The present study is a clinical analysis of 41 cases which were terminated

pregnancy with 16, 16-Dimethy1-Trans-△**2-PGE^^1 Methy1 Ester vaginal suppository

at the Dept. of OB. & GYN. of Severance Hospital, Won Ju Christian Hospital and

Cheil Hospital from May. 1982 to Oct. 1982. 21 cases of these 41 cases were early

trimester termination and 20 cases were mid-trimester termination.

1. In early trimester termination, 10 cases(47.6%) of 2L cases had complete

abortions and 6 cases (28.6%) had incomplete abortion and in mid-trimester

termination, 18 cases (90%) had complete abortions and 6 cases (28.6%) had

mfd-trimester termination, 18 cases (90%)had complete abortion and 2 cases (10%)

had incomplete abortion. So the success rate 16DM-PGE^^1 vaginal suppository was

76.2% in early trimester and 100% in mid-trimester.

2. The onset of uterine contraction following the initial administration of

16DM-PGE^^1 vaginal suppository was 1 hour 51minutes (Mean±SE; 59mins ± 4.2 min)

in mid-trimester termination.

3. The time interval required for expulsion of conceptive products, whether

complete or incomplete, was 13 hours 54 minutes in mid-trimester termination group.

4. Following 24 hrs after first insertion of vaginal suppository, the cervix was

dilatated to 1.0㎝ or more and marked cervical softening was noted in early

trimester termination group. And in 10 nulliparous women of early trimester

termination group, the cervical dilatation of 8㎜ or more was observed in 8

patients and in the remaining 2 cases, the cervixes were dilatated to 6㎜ and 7㎜

respectively.

Cervical softening appeared in all cases.

5. There were side effects observed in 6 cases (28.6%) during early trimester

termination and in 11 cases (55%) during midtrimester termination with 16DM-PGE**1

vaginal suppository. Severe side effect was appeared in only one cases during early

trimester termination and 4 cases (20%) during mid-trimester termination.
