만성질환자는 질병의 조절은 가능하나 평생동안 치료와 자가 간호를 계속해야 하는 행동적인 난문제를 갖고 있기 때문에 환자로서 지켜야 할 행위를 이행하지 않을 경우 회복이 지연되거나 질병이 악화되며, 여러가지 신체적, 정신적, 사회적인 문제들을 일평생 지니고 살아야 한다.

특히 만성관절염은 오랜기간의 통증과 활동의 제한으로 무능력하게 되며 심하게는 관절의 변형까지 초래하는 만성질환으로 가족이나 주위사람에게 완전히 의존적이 되며, 전반적인 삶에 부정적인 영향을 미치게 된다.

본 연구는 만성관절염환자를 대상으로 치료지시이행을 증진시키고 만족스러운 삶을 유지할 수 있도록 돕는 간호중재 전략을 위한 기초자료를 제공하고자 건강과 관련된 중요한 변수인 가족 지지와 치료지시이행 및 삶의 만족간의 관계를 규명하기 위하여 시도된 서

술적 상관관계 연구이다.

연구방법은 1987년 4월27일부터 1987년 5월 12일까지 13일동안 서울시 소재 일 대학부속병원 정형외과 외래에서 만성관절염으로 진단 받고 통원치료 받도록 지시된 환자를 모집단으로 하여 선정기준에 의해 유의표출한 92명을 연구대상으로 하였으며, 질문지를 통하여 자료를 수집하였다.

연구도구는 강이 개발한 가족지지 측정도구, 본 연구자가 작성한 치료지시이행 측정도구, 그리고 Wood, Wylie & Sheafer 가 작성한 삶의 만족 측정 도구 (Life Satisfaction Index-Z)를 사용하였다.

자료의 분석을 위해 일반적 특성은 실수, 백분율, 평균, 표준편차를 구하였고, 일반적 특성과 제 변수들과의 유의성 검정은 ANOVA와 t-test를 이용하여 분석하였다. 가설검정은 피어슨적율상관계수(Pearson Correlation Coefficient)를 이용하여 분석하였으며 추가분석으로 삶의 만족에 영향을 미치는 요인을 확인하기 위해 단계적 중회귀분석 (Stepwise Multiple Regression)을 이용하였다.

본 연구의 결과는 다음과 같다.

제1가설: "만성관절염환자의 가족지지정도가 높을수록 치료지시 이행 정도는 높을 것이다."는 지지되었다(r=.2604, p=.006).

제2가설: "만성관절염환자의 치료지시이행정도가 높을수록 삶의 만족정도는 높을 것이다."는 기각되었다(r=.1663, p=.057).

제3가설: "만성관절염환자의 가족지지정도가 높을수록 삶의 만족정도는 높을 것이다."는 지지되었다(r=.3914, p=.001).

2. 일반적 특성과 삶의 만족과의 관계에서 침범부위수가 많은 사람, 증상호소수가 많은 사람이 삶의 만족이 낮은 편이었다

3. 삶의 만족에 영향을 미치는 예측인자를 확인하기 위한 단계적 중회귀분석 결과 가장 중요한 예측인자는 가족지지였고 다음이 침범부위수, 치료지시이행, 통증정도의 순이었으며 이 4개의 주요변수에 의해 30.8%의 설명이 가능하였다.

결론적으로 만성관절염환자에게 있어서 가족지지는 치료지시이행과 삶의 만족을 증진시킬 수 있는 주요변수임이 확인되었다





A Study on the Relationships between Family Support, Compliance and Life

Satisfaction in Chronic Arthritis

This descriptive-correlational study was undertaken in order to examine if there

were relationships between family support, compliance and life satisfaction in

chronic arthritis patients.

The sampling method was a non-probability, purposive sampling technique. The

participants of this study were 92 volunteer adults.

1) who have been diagnosed as having chronic arthritis and

2) who were at the medical out patient clinic of one university hospital in Seoul

between April 27th to May 12th 1987.

The instruments used for this study were the Family Support Scale developed by

Kang Hyun Sook, a Compliance Scale developed by the researcher and the

Life-Satisfaction Index-Z developed by Wood et. al.

Analysis of data was done using t-test, ANOVA, Pearson Correlation Coefficient

and Stepwise Multiple Regression Analysis.

The results of study were as follows;

Hypothesis Ⅰ; "The higher the family support, the higher the compliance" was

accepted. (r=.2604, p<.01)

Hypothesis Ⅱ; "The higher the compliance, the higher the life satisfaction" was

rejected. (r=.1663, p>.05)

Hypothesis Ⅲ; "The higher the family support, the higher the life satisfaction"

was accepted. (r=.3914, p<.01)

In addition, by using Stepwise Multiple Regression Analysis it was determined

that the main influencing factors on the life satisfaction were family support, the

number of inflammatory joints, compliance and pain level. These 4 main variables

made it possible to explain 30.8% of the variance in life satisfaction.

In conclusion, this study revealed that family support is an important factor

that can improve compliance and life satisfaction in chronic arthritis patients.
