오늘날 우리 사회는 각종 산업의 발달과 더불어 대기와 하수오염 및 소음과 각종공해가 문제화되고 있다. 특히, 반자연적 물리환경 아래서의 소음장애야말로 위급한 문제라고 하겠다. 그러므로 국방이라는 확고한 목표를 가진 군대의 한 구성원인 0군 조종사들의 경우, 항상 항공기소음에 노출되므로, 이들 소음으로 인해 청각장애는 물론 심리적인 긴장 내지는 불안을 일으켜 건강의 위해도가 증가되고 있는 실정이다.

본 연구는 0군 조종사들의 청력변화 및 청력상실을 소음성 난청의 발생 요인으로 분석하고, 청력보호구 사용 정도를 파악함으로써 소음성난청에 대한 예방책으로 군 보건 교육의 지표 및 주기적인 청력검사의 실시를 재인식시키고자 하는데 그 목적이 있다.

연구대상은 본 항공의학연구원 이비인후과를 방문한 0군 조종사 108명이었으며, 1986년 9월4일부터 10월 31일에 걸쳐 자료수집을 하였다.

연구도구는 본 연구자가 문헌고찰을 하여 지도교수의 자문을 받아서 작성한 질문지로 문항수는 총 35개 문항이었다.

자료분석은 백분율, X**2 -test, ANOVA의 통계방법으로 분석하였다.

본 연구결과를 요약하면 다음과 같다.

1. 연령별로 분석한 청력의 변화는 연령이 증가할수록 청력이 떨어짐을 보여 주었다(P < .05).

총비행시간별로 분석한 청력의 변화에서는 총비행시간이 많아질수록 청력이 떨어짐을 보여 주었다(P< .05).

계급별로 살펴본 청력의 변화는 계급이 많아질수록 청력이 떨어짐을 보였다(P< .05).

2. 순음청력 검사상 소음성난청의 발생율은 9명 ( 8.3%)이고, 이중 전형적인 C^^5 - dip은 5명 ( 4.6%)이며, 고음역난청자는명 4명(3.7%) 이였다.

기종별 소음성난청의 발생율은 유의한 차이가 없었다.(P> .05).

청력보호구 사용에 관한 교육유무별 소음성난청의 발생률은 교육을 받지 않은 군에서 높은 발생율은 보였으나 통계적으로 유의한 차이가 없었다 (P> .05).

신체 및 청기에 관한 자각증세별로는 청기에 관한 자각증세인 이명이 36명 (33%)이고 그 기간은 1년 미만 이었으며, 그중 소음성난청의 발생율은 2명 (5.6%)이었다.

3. 청력보호구 사용 정도에서는 계급이 높을수록 청력보호구 사용정도가 낮았다 (P< .05).

청력에 관한 보건교육을 받은 유무에서는 보건교육을 받은 군에서 청력보호구 사용정도가 높게 나왔으나 유의한 차이가 없었다(P> .05).

결론적으로 조종사들은 항상, 항공기소음에 노출되어 있는 그들의 근무환경으로 인해 소음성난청을 유발할 수 있다. 그러므로 조종사들의 청력보존을 위해, 청력보호구의 사용과 정규적인 보건교육 및 신체검사의 필요성을 지역사회의 한 일원인 보건간호원으로서

강조하고 싶으며, 국가적인 차원에서도 관심과 협조가 요구된다.





A STUDY ON THE ETIOLOGICAL CLASSIFICATION OF AIRCRAFT NOISE INDUCED HEARING LOSS

-Centered on the ROKAF pilots-

Nowadays, the air and water pollution is a serious problem in our society along

with the various industrial development. Especially, the noise problem under the

antinatural circumstances is an urgent problem.

In case of pilots as members of military personnel with the aims of national

defense, they are always exposed to the aircraft noise resulting in not only the

hearing losses but also the psychological stresses and anxieties so affecting their

health seriously.

The purpose of this report is to provide the index of the military health

education and to stress the importance of the periodical hearing test to prevent

the noise induced hearing losses, by analyzing the changes and the losses of the

hearing ability of pilots in terms of their etiology, and by estimating the degree

of using the apparatus for the protection of hearing.

The subjects in this study were based oi a convenient sample of 108 air force

pilots visiting ENT OPD from Sep. 4 to Oct. 31, 1986.

The questionnaire was made up of 35 items drawn from reviewing the literatures

directed by the professor.

The statistical analysis of data was done by use of percentile, x^^2 -test and

ANOVA test.

The findings of this study were as follows:

1. The incidence of noise-induced hearing loss was increased with increasing age

(P (.05), total flight time (P < .05),ranks (P < .05).

2. The incidence of noise-induced hearing Loss was 9 persons (8.3%); They typical

C^^5-dip was 5 persons (4.6%) and hightone loss 4 persons (3.7%) by pure tone

audiometry.

The incidence of hearing loss was no meaningful difference among types of

aircrafts (P > .05).

The incidence of hearing loss in the groups uneducated to don the protecting

apparatus was slightly higher than that of the groups educated, but was no

meaningful difference (P > .05).
