와우전도를 이용한 와우전위는 측정이 가능해짐에 따라 이를 이용해서 청각역치의 객관적 측정과 감음신경성 난청의 감별진단은 물론 청각생리 연구에 응용하고 있다.

저자는 감음신경성 난청의 객관적 감별진단 및 메니엘씨 병의 이신경학적인 객관적 진단에 SP(summating potential : 이하 SP로 약기함)와 AP(action potential : 이하 AP로 약기함)의 활용성을 알아보기 위하여 기니피그의 SP, SP/AP의 정상치를 구하고자 본 연구

를 실시하여 다음과 같은 결과를 얻었다.

1. AP의 역치

0.5KHz에서 53.93±12.41dB SPL, 1KHz에서 46.52±14.30dB SPL, 2KHz에서 36.30±8.01 dB SPL, 4KH2에서 22.17±13.26 dB SPL, 8KHB에서 17.73±7.30 dB SPL을 나타내어 주파수가 증가할수록 ANl(first negative component of action potential ; 이하 ANI으로 약기함) 역치는 감소하여 역비례 관계를 보였다.

2. 자극음의 강도에 따른 AP와 진폭과 잠복기의 변화

자극음의 강도와 주파수가 증가함에 따라 AP의 진폭은 증가하였으나, 잠복기는 감소하여 상호 역비례의 관계를 나타냈고 자극음의 구파수가 증가에 따른 진폭의 변화는 더 가파른(steep) 양상을 보였다.

3. 자극음의 변수(parameter)에 따른 SP, AP, SP/AP 의 변화

1) Interstimulus Interval(이하 ISI로 약기함)에 따른 변화

AP의 plateau는 80msec-320msec의 작극간 간격에서 유의하게 관찰되었다.

2) 자극음의 강도와 주파수에 따른 변화

자극음의 주파수와 강도가 증가함에 따라 SP, SF/AP는 증가하였다.

The electrocochleography which is measurable to cochlear potentials is a useful

method for the detection of hearing threshold, for the differential diagnosis of

sensorineural hearing loss and for the stuffy of auditory physiology.

I have measured the normal value of SP, SP/AP at the round window in 20 healthy

gunig pigs as a preliminary study for the differential diagnosis of sensorineural

hearing loss.

The results were as follows:

1. ANl threshold

ANI threshold was significantly decreased as the frequency of stimuli was

increased.: ANl thresholds were 53.93 ± 12.41 dB SPL at 0.5 KHz, 46.52 ± 14.30 dB

SPL at 1 KHz, 36.30 ± 8.01 dB SPL at 2 KHz, 22.17 ± 13.26 dB SPL at 4 KHz, 17.73

± 7.30 dB SPL at 8 KHz.

2. Amplitude and latency of AP according to the intensity of stimuli

As the intensities and frequencies of stimuli were increased, the amplitude of AP

was significantly increased. The reverse relationship was observed between

amplitude and latency of AP. As the frequency of stimuli was increased, the

amplitude-latency curve became steeper.

3. SP, AP, SP/AP according to the parameters of stimuli

1) ISI

The plateau of AP was noted at ISI between 80msec and 320msec.

2) The intensity and frequency of stimuli

The SP, SP/AP ratio were significantly increased as the intensity and frequency

of stimuli were increased.
