Abstract
[한글] 악교정수술전후의 안모의 형태적변화 뿐만 아니라 하악골 운동 양상 및 기능상의 변화를 측정하기 위하여 하악골전돌을 주소로 악교정수술을 요하는 19명(남자13명, 여자 6명)의 골격성Ⅲ급 부정교합환자의 술전과 술후 2개월 및 술후 8개월에 측모두방사선 사진을 각각 촬영하고 E.M.G. 및 M.K.G.를 이용하여 연구를 시행한 결과 다음과 같은 결론을 얻었다. 1. 칙대개폐구속도는 술전 (개구 349.7mm/sec, 폐구 313.1mm/sec)에 비해 술후 2개월 (개구 232.mm/sec, 폐구 206.9mm/sec)에는 감소되었다가 술후 8개월 (개구 280.9mm/sec, 폐구 319.1mm/sec)에는 점차 증가되는 경향을 보였다. 2. 종말치아접촉시 최대속도는 술전 (61.7mm/sec)에 비해 술후 2개월 (72mm/sec), 술후 8개월 (105.7mm/sec)에는 점차 증가되었다. 3. 수직안정공간은 정상군과 실험군 술전 간에는 유의차가 있었으나 정상군과 술후 8개월간에는 유의차가 없었다. 4. 최대개구량과 최대전후방운동량 및 측방변위량은 정상군과 실험군(술전, 슬후 8개월) 간에는 모두 유의차가 없었다. 5. 연하양상은 술전에 성숙연하가 58 %이었는데 술후 2개월에 90%에서 성숙연하를 보였고, 술후 8개월에는 그 비율이 63%가 되었다. 6. 각 근육의 근활성도를 비교한 결과 안정위에서는 정상군과 실험군간에 유의차가 없었으며, cotton roll clenching시에는 정상군과 실험군 술전에는 유의차를 보였으나, 정상군과 술후 8개월에는 유의차가 없었다.
[영문] The purpose of this study was to investigate the changes of mandibular movement and muscle activity following orthognathic surgcry in patients with mandibular prognathiam. Lateral cephalogram, M.K.G., E.M.G. recordings were obtained immediately before surgery, and 2 mouths and 8 months after surgery. Among the patients who received orthognathic surgery, 19 (13 men,6 women) micro selceted for this stuffy. Statistical analysis for each time interfal differences micro performed with the SPSS package. The results micro as follows: 1. Comparcd with talc pre-operative group (opening 349.7 mm/sec, closing 313.1 mm/sec), the mean values of the maximum opening and closing velocity in the skeletal Class Ⅲ surgery group were significantly decreased in the 2 months cost-operative (opening 232.9 mm/sec, closing 206.9 mm/sec), but the values tended to increase in the 8 months post-operative group (opening 280.9 mm/sec, closing 319.1 mm/sec). 2. Compared with the pre-operative group (61.7 mm/sec), the maximum velocity of the terminal tooth contact increased in the 2 months (72 mm/sec) and 8 mouths (105.7 mm/sec) post-operative groups. 3. In the mean value of vertical freeway space, there was significant difference between the normal group and the pre-operative group, but not between the normal group and the 8 months post-operative group. 4. In the mean values of the maximum opening, the maximum anterio-posterior movement from centric-occlusion, and the lateral deviation from centric occlusion, there was no si후ificant difference between the normal group and the surgery group (the pro-operative and the 8 months post-operative groups). 5. The mature swallowing pattern was 58% in the pre-operative group, but 90% in the 2 mouths post-operative group, and 63% in the 8 months post-operative group. 6. In the comparison of muscle activity, there was no significant difference between the normal group and the surgery group during the rest position. However, during cotton roll clenching, there was significant difference between the normal group and the pre-operative group, but not between the normal group and the 8 months post-operative group.
