근관치료에 대한 인식도 및 의료보험에 관한 설문조사

환자와 치과의사들이 자연치아가 다른 어떠한 인공 수복물보다 기능적으로 더욱 효과적임을 인식함에 따라 치수 침범된 치아에 대한 보존노력이 격증되어 근관치료학의 중요성이 점점 비중이 높아지게 되었다. 근관치료는 눈으로 직접 확인할 수 있는 보존영역의 와동형성 내지 지대치 형성과는 달리 치근단 협착부를 방사선사진으로 간접적으로 인지하여 치아 개개에 대한 지식과 경험에 의한 촉지로써 시술해야 하므로 어려운 점이 많다.

그러나, 현행 의료보험제도는 치과의료의 특성인 시간적, 기술적, 예술적 및 재료공학적 측면은 반영되지 않은 채, 행정적 편의와 보험재정(보호) 차원의 일방적 운영으로 불합리하고 적정한 진료에도 많은 문제점들이 도출되고 있는 실정이다.

이에 저자는 환자와 치과의사의 근관치료에 대한 인식도 및 지식정도를 측정하고 현행 근관치료의료보험에 대한 문제점을 지적하여 양질의 진료를 위한 대안을 제시하고자, 대한치과의사협회의 연구비를 대한치과보존학회을 통해 지원받아 1986년 1년간 치과치료을

받은 경험이 있는 환자 1,300명과 치과의사 1,500명을 대상으로 환자에게는 근관치료에 대한 인식도를, 치과의사에게는 근관치료에 대한 인식도 및 의료보험제도에서의 근관치료에 대한 의견조사를 질문지를 이용 조사하여 다음과 같은 결론을 얻었다.

1. 근관치료에 대한 환자측의 인식

근관치료에 대한 정확한 지식은 없었으나 치아보존의 중요성은 인식하고 있었으며 (85.0%) 처치는 대부분 치과의사의 결정을 따랐다(85.2%).

2. 근관치료에 대한 치과의사의 인식

근관치료의 중요성은 인식하고 있었으나(85.7%) 시술 자체에 부담감을 가지고 있으며 치료결과에 확신이 없었다(79.9%).

3. 근관치료의료보험에 대하여 다수의 치과의사가 시술 내용에 비하여 보험수가가 매우 낮다고 지적하였으며(91.6% ) 현실화 되었을 경우에는 근관치료의 적극적 권유를 포함하여 성실한 치료가 기대될 수 있다고 전망하였다(73.1%).

This survey was purposed to provide some information of recognition and health insurance coverage of endodontic treatments with questionaire which was consist of 29 items in 1986, and analyzedb of 1,300 patients who had dental problems and of 1,500 private practitioners in Korea.

The results were as follow :

1. Patient's recognition for endodontic treatments :

Most of patients didn't have an accurate information of endodontic treaments. On the other hand, they recognized the importance of their own teeth(85.0%), and patients were followed dentist's recommandation and took a treatment(85.2%).

2. Dentist's recognition for endodontic treatments :

Most dentists understood the importance of endodontic treatments(85.7%), however they considered endodontic treatments were considerbly troublesome, and their abilities were not enough to treat confidently(79.9%).

3. Most of dentists complainted to relatively low insurance oost in spite of difficulties of endodontic treatments(91.6%), and if the cost of endodontic treatment will be up reasonably then, they seemed to agree with active performances and suggestions for patients(73.1%).
