성, 연령 및 신장에 따른 후경골신경자극 감각유발전위의 변화

유발전위검사는 진찰만으로는 객관화하기 어려운 감각기능 장애를 정량화할 수 있다는 장점때문에 임상에서 많이 이용되는 전기생리학적 검사로서 후경골신경 유발전위검사는 말초신경인 후경골신경을 전기적으로 자극할 때 중추신경계에서 나타나는 전위의 변화를 Blgnalaverage하여서 관찰하는 것이다. 전기생리학적 검사의 임상적응에 있어서 흔히 문제가 되는 것은 기술이나 환경의 차이에 의하여 그 결과가 달라진다는 점인데 이를 해결하기 위해서는 각 검사실마다 일정한 검사방법을 많은 수의 정상인에서 적용하여서 정상

범위를 결정하고 이에 준하여 환자의 검사결과를 판정하여야 한다. 그러나 정상범위의 결정단계에서 문제가 되는 것이 환자의 성이나 신장, 연령 등 인위적으로는 조절할 수 없는 생리학적 인자에 의한 영향인데 현실적으로 이 여러 가지 생리적 인자별로 정상범위를

결정한다는 것은 불가능하다. 특히 후경골 신경유발전위의 전달경로는 말초신경계와 중추신경계를 포함하고 매우 길기 때문에 이런 생리적 인자에 의한 영향이 다른 유발전위에 비하여 현저할 것으로 추측된다. 따라서 정상범위를 결정하는 단계에서의 문제점을 해결하기 위한 많은 연구가 진행되어 온 것은 사실이지만 각 연구 보고마다 관심의 촛점이 다르고 통계 기법에 차이가 날 뿐 아니라 연구결과도 일정하지 않다는 문제점이 노출되었다. 더욱이 생리학적 인자는 독립적으로 검사 결과에 영향을 주기보다는 성과 신장, 신장과 연령사이의 상관관계 때문에 복합적으로 작용하리라고 추정되는데 현재까지 각각의 인자

에 의한 분석 연구보고는 있었지만 복합적인 영향에 대한 정착한 분석은 없는 것으로 생각된다. 이어 연구자는 특별한 신경학적 질환이 의심되지 않는 다양한 신장 및 연령의 20세 이상의 성인 123명 (남자 55명, 여자 68 명)에서 후경골신경 자극 유발전위검사를 시행하고 성. 연령 및 신장이 검사결과에 미치는 각각의 영향은 물론 복합적인 영향을 분석하여 다음과 같은 결과를 얻었다.

1. 각 전위파의 절대잠복기와 진폭 및 중추전달시간은 좌우간에 차이가 없었다.

2. 남자와 여자에서의 유발전위파를 t-검정으로 단순하게 비쿄한 결과 남자가 여자에 비하여 척수유발전위인 T12와 뇌유밭전위증 Pl, Nl 및 P2의 절대잠복기가 길었으며 T12의 진폭은 여자가 남자보다 컸다.

3. 단순회귀분석상 연형은 유발전위의 각 측정간에 다양하게 영향을 주었는데 대체적으로 나이가 많아지면 잠복기가 느려지고 진폭은 작아지는 경향을 나타내었다.

4. 신장이 유방전위의 측정간에 미치는 영향을 단순회귀방법으로 분석하면 진폭에는 큰 영향이 없고 잠복기는 척수 T12, 뇌유발전위의 Pl, Nl, P2의 순위로 영향을 주는 것으로 생각되었다.

5. 유의수준 0.05외 단계별 회귀분석으로 성, 연령 및 신장이 각 측정값에 미치는 영향을 평가하면 거의 모든 잠복기는 신장 힐 연형의 순위로 영향을 받고 진폭은 척수 T12 진폭에 대하여만 어느 정도 엉향력이 있을뿐 뇌유발전위에서는 연령만 통계적으로 의의가 있는 변수로 인정되었다. 중추전달시간은 연령 및 신장의 순위로 영향을 받으며 성별자체는 잠복기나 진폭에 모두 큰 영향이 없었다.

이상의 관찰결과로 후경골신경 SSEP는 신장과 연령에 의하여 복합적인 영향을 받는다는 사실을 확인할 수 있고 향후 임상에서 후경골신경 SSEP의 결과를 판정할 때는 이 두가지 인자의 영향력을 동시에 고려하여야 할 필요가 있다고 생각된다.



[영문]

Evoked potentials(EP) are kinds of electrephysiological study to gauge the senosry system, which usually cannot be measured on clinical examination, objectlvely and quantitatlvely. The limitations of the electrophysiogical study on clinical application include variations of results depending on not only technical and environmental factors but also various physiological factors such as sex, age and height. The effects of technical and environmental factors can be standardized in each laboratory, but not for physiological factors. The importance of

physiological factors to emphasized on the posterior tibial nerve evoked potential study, because of long travelling course including both peripheral and central nervous system. There are several reports on this problem, but with a lot of controversies probable due to different point of view and statistical analysis.

Furthermore, contrary to many authors, I think the effect of various physiological factors cannot be measured by simple regression analysis because of chose correlations between each others. To analyse the comp1ex effect of various physiological factors on posterior tibial nerve evoked potential study, 123

cases(male: 55.female: 68) of the age more than 20 years old and without definite neurological deficits are studied and analysed.

The results are as follows

1. Absolute latencies, amplitudes of each SSEP waves and T12-P1 central conduction the(CCT) between left and right side showed no signaificant diffierence.

2. Spinal T12 and cortical P1, N1, P2 latencies were significantly longer in males than in females and T12 amplitude was higher in females.

3. By simple regression analysis, age baa consistently correiated with many components of SSEP tilth general tendency of increasing latencies and decreasing amplitudes in old age.

4. The height was highiy correlated with absolute latencies of spina1 T12 and cortical P1, N1 and P2 in order, but litt1e effect on amplitudes.

5. On Stepwise multiple regression ana1ysis at the significant vague of 0.05, broth height and age are important variables for the latencies of posterir tibial SSEPs and only age for amplitudes. The gender itself has no measurable effect on

the results of SSEPs.

In conclusion. this study showed that both height and age are important physiological factors alterating the result of posterior tibial SSEP and the effect of these factors might be counted for clinical application of this study.
