청각은 본질적으로 문화 및 사회생황의 가장 중요한 의사소통의 매체로서 청력이 손실된 때는 여러가지의 많은 장애가 수반되게 된다. 따라서 청력손실의 예방과 정화한 진단, 치료 및 효율적인 대책을 위해서는 공인된 진단방법이 필요하며 이를 위한 여러가지 측정방법과 이에 따른 측정제기의 개발이 모색되어 왔다 .

현재 가장 널리 쓰여지고 있는 순음 및 어음청력검사와 Impedance청력검사는 서로간에 장단점 및 특성을 갖고 있으므로 청력검사의 목적 즉 청력장애의 유무 및 정도, 청력장애의 기능적 및 기질적 구분, 청력기관의 장애부위진단, 청력증진수술의 가능성 및 회복정도의 예측, 수술방법의 선택, 술후 청력개선의 정도판정, 보청기 및 특수교육의 적부판정등에 정확을 기하기 위해서는 이들 검사를 종합하여 상호간의 보완점과 관련성을 최대한도로 살려야 하며 그러기 위해서는 이들 검사 기준치간의 상관관계를 살펴볼 필요가 있다. 그러나 지금까지 우리나라에는 이들에 대한 부분적인 연구발표가 있었을 뿐 아직 체계적으로 이들 검사기준치간의 상관관계를 조사한 연구가 없는 실정이므로 이에 저자는 각종 청력검사의 기준치 및 그들의 상관관계를 살펴보고저 본 연구를 시행하였다. 연구대상으로는 20대의 건강한 의과대학 남학생 100명과 의과대학 및 간호대학 여학생 50명을 선정하였으며 그 결과 다음과 같은 성적을 얻었다.

1. 순음청력평균치 (PTA, Pure Tone Average ) 는 남자에서는 우측 귀에서 4±6.7 dB, 좌측 귀에서 3±7.7 dB였으며 여자에서는 우측 귀에서 4±7.4 dB,좌측 귀에서 2±6.7 dB였다.

2. 어음청취 역치 (SRT, speech Reception Threshold) 는 남자에서는 우측 귀에서 4±7.2 dB, 좌측 귀에서 3±8.1 dB였으며 여자에서는 우측 귀에서 3±6.5 dB, 좌측 귀에서 3±7.4 dB였다.

3. 최적쾌감치 (MCL, Most Comfortable Loudness) 는 남자에서는 우측 귀에서 45±15.4 dB, 좌측 귀에서 46±17.9 dB였으며 여자에서는 우측 귀에서 45±15.1 dB, 좌측 귀에서 44±16.6 dB였다.

4. 불쾌감역치 (UCL, Uncomfortable Loudness)는 약 반수에서만 측정이 가능하였으며 남자에서는 우측 귀에서 102 dB, 좌측 귀에서 102 dB였으며 여자에서는 우측 귀에서 103 dB, 좌측 귀에서 102 dB였다.

5. 어음판별역치 (PB Score, Phonetically Balanced Score)는 전부 90% 이상이었으며 100%인 경우가 전 예의 34%%로써 가장 많았고 남자에서는 우측 귀에서 98%, 좌측 귀에서 97%였으며 여자에서는 좌우측 귀 모두 97%였다.

6. 음강차판별능검사 (SISI test, Short Increment Sensitivity Indax test)에서는 0%를 나타sos 예가 45%로써 가장 많았고 80%이상을 나타낸 예는 한 예도 없었다.

7. 청각순응검사 (T.D. teat, Tone-Decay test ) 에서는 0 dB가 53%로 가장 많았고 20dB이상은 한 예도 없었다.

8. Impedance 청력검사에 의한 고실도(tympanogram)는 A형이 99.7%였고 B형이 0.3%였다.

9. 고실도의 극상치 (Peak Leve1)는 남자에서는 우측 귀에서 22.8±32.94 mmH^^2O, 좌측 귀에서 23.9±29.81 mmH^^2O 였으며 여자에서는 우측 귀에서 20.7±23.43 mmH^^2O, 좌측 귀에서 23.6±22.33 mmH^^2O 였다.

10. 정지응락(Static Compliance)은 남자에서는 우측 귀에서 0.6±0.54 cc,좌측 귀에서 0.6±0.53 cc였으며 여자에서는 우측귀에서 0.6±0.50 cc, 좌측 귀에서 0.6±0.53 cc였다.

11. 교차등골근반사 (Contralateral S.R., Contralateral Stapedial Reflex)역치는 남자 2.25%, 여자 1.75%에서 측정할 수 없었으며 측정이 가능하였던 예만을 대상으로 볼 때 남자의 우측과 좌측 귀의 역치는 각각 500 Hz에서는 89±15.9 dB, 89±13.8dB였고 1,000 Hz에서는 85±13.2 dB, 85±11.7 dB였고, 2,000Hz에서는 88±12.9 dB, 89±14.1dB였고 4,000 Hz에서는 92±15.8 dB, 91±16.8 dB였으며 여자의 우측과 좌측 귀에서는 자각 500 Hz에서는 90±15.5 dB, 89±12.0 dB였고 1,000 Hz에서는 86±13.4 dB, 85±11.2 dB였고 2,

000 Hz에서는 89±11.4 dB, 89±14.3 dB였고, 4,000 Hz에서는 93±15.6 dB, 91±l9.1 dB였다. 한편 비교차등골근반사 (Ipsilateral S.R.,Ipsilateral Stapedial Reflex) 역치는 남자의 우측과 좌측 귀에서 각각 1,000 Hz에서는 87±14.0 dB, 87±12.4 dB였고 2,000 Hz

에서는 90±13.6 dB, 88±14.0 dB였으며 여자의 우측과 좌측 귀에서는 각각 1,000 Hz에서는 89±14.5 dB, 87±12.6 dB였고 2,000 Hz에서는 91±14.9 dB, 90±14.0 dB였다.

12.이관기능검사에서 극상치의 차이는 +250 mmH^^2O 를 주었을 때 남자에서는 우측 귀에서 8±21.9 mmH^^2O, 좌측 귀에서 9±l7.3 mmH^^2O 였으며 여자에서는 우측 귀에서 9±22.3 mmH^^2O, 좌측 귀에서 9±17.6 mmH^^2O 였다. 한편 -25O mmH^^2O 를 주었을 때 남자

에서는 우측 귀에서 14±26.9 mmH^^2O, 좌측 귀에서 12±22.5 mmH^^2O 였으며 여자에서는 우측 귀에서 13±28.1 mmH^^2O,좌측 귀에서 13±20.9 mmH^^2O였다.

13. 어음청취역치와 순음청력평균치간의 차이 (SRT-PTA)는 남자에서는 우측 귀에서 -1±9.2 dB, 좌측 귀에서 0±10.0 dB였으며 여자에서는 우측 귀에서 -1±9.2 dB, 좌측 귀에서 0±10.4 dB였다.

14. 최적쾌감치와 어음청취역치간의 차이 (MCL-SRT)는 남자에서는 우측 귀에서 42±15.

1 dB, 좌측 귀에서 43±17.8 dB였으며 여자에서는 우측 귀에서 42±15.5 dB, 좌측 귀에서 42±18.1 dB였다.

15. 불쾌감역치와 어음청취역치간의 차이 (UCL-SRT, 즉 Dynamic rage)는 남자에서는 우측 귀에서 98 dB, 좌측 귀에서 99 dB였으며 여자에서는 우측 귀에서 99 dB, 좌측 귀에서 100 dB였다.

16. 등골근반사역 치와 그 주파수에서의 순음청력역치간의 차이 (S.R.Pure tone threshold)인 등골근반사감각역치는 교차반사의 경우 남자의 우측과 좌측 귀에서 각각 500 Hz에서는 84±17.7 dB, 84±15.9 dB였고 1,000 Hz에서는 87±14.4 dB, 87±13.9 dB였고 2,000 Hz에서는 80±13.7 dB, 84±16.8 dB였고 4,000 Hz에서는 87±18.6 dB, 87±21.3 dB였으며 여자의 우측과 좌측 귀에서는 각각 500 Hz에서는 85±17.8 dB, 85±14.4 dB였고 1,000 Hz에서는 87±13.8 dB, 90±14.2 dB였고 2,000 Hz에서는 80±11.8 dB, 85±15.5. dB였고 4,000 Hz에서는 88±19.2 dB, 88±21.6 dB였다. 한편 비교차반사의 경우 남자의 우측과 좌측 귀에서 각각 1,000 Hz에서는 89±16.3 dB, 89±18.0 dB였고 2,000 Hz에서는 82±15.9 dB, 83±18.9 dB였으며 여자의 우측과 좌측 귀에서는 각각 1,000 Hz에서는 90±l7.1 dB, 91±14.8 dB였고 2,000 Hz에서는 82±17.2 dB, 85±16.5 dB였다.



[영문]

Currently in the otologic field, there are various methods of audiometric examinations such as pure and speech and impedance. A few studies has been done in these fields in Korea.

The purpose of this paper is to establish norms and the relationship of norms of various audiometric tests, so I have performed the tests in 100 male medical students and 50 female medical and nursing students in healthy physical condition and the following results were obtained.

1. The mean of the PTA(Pure Tone Average) was 4dB on the male right, 3dB on the male left, 4d8 on the female right, 2dB on the female left and the mean of the SRT(Speech Reception Threshold) was 4dB, 3dB, 3dB, 3dB, respectively.

2. The mean of the MCL(Most Confrotable Loudness) was 45dB on the male right, 46dB on the male left, 45dB on the female right, 44dB on the female left.

3. The mean of the UCL(Uncomfortable Loudness) was 102dB on the male right, 102dB on the male left, 103dB on the female right, 102dB on the female left. We had trouble in estimating the UCL in about half the cases as we could not estimate the UCL with our conventional audiometer.

4. All cases showed over 90% in the PB Score and 100% was the most common level.

5. SISI(Short Increment Sensitivity Index) test was done at 1,000 and 4,000Hz.

The most common level was O% and above 80% was not seen.

6. TD(Tone Decay) test was done at 2,000 and 4,000 Hz. The most common level was 0dB and above 20dB was not seen.

7. Tympanogram was A type with one exception which was 8 type on the male left.

8. The mean of the tympanogram peak level was 22.8 mmH^^2O on the male right, 23.9 mmH^^2O on the male left, 20.7 mmH^^2O on the female right, 23.6 mmH^^2O on the female left and the mean of static compliance was 0.6cc on all sides.

9. The mean of the contralateral stapedial reflex threshold at 500, 1,000, 2,000, 4,000 Hz was 89dB, 85dB, 88dB, 92dB on the male right, 89dB, 85dB, 89dB. 91dB on the male left, 90dB, 86dB, 89dB, 93dB on the female right, 89dB, 85dB, 89dB, 91dB on the female left. Depending on the tested frequencies, the stapedial reflex was not generated in 2.25% on the male, 1.75% on the female.

10. The mean of the ipsilatetal stapedial reflex threshold at 1,000, 2,000Hz was 87dB, 90dB on the male right, 87dB, 88dB on the male left, 89dB, 91dB on the female right, 87dB, 90dB on the female left. Depending on the tested frequencies, the stapedial reflex was not generated in 0.75% on the male, 0.5% on the female.

11. Eustachian tube function using the impedance audiometer malfunctioned in 7.5% of the cases. The mean difference between the peak level in the resting state and the peak level at inflation (+250mmH^^2O ), deflation (-250mmH^^2O ) was 8mmH^^2O,

14mmH^^2O on the male right, 9mmH^^2O, 12mmH^^2O on the male left, 9mmH^^2O, 13mmH^^2O on the female right, 9mmH^^2O, 13mmH^^2O on the female left.

12. The mean difference between the SRT and the PTA was -1dB on the male right, 0dB on the male left, -1dB on the female right, OdB on the female left.

13. The mean difference between the MCL and the SRT was 42dB on the male right, 43dB on the male left. 42dB on the female right, 42dB on the female left.

14. The mean dynamic range was 98dB on the male right, 99dB on the male left, 99dB on the female right, 100dB on the female left.

15. The mean of the contralateral stapedial reflex threshold(Sensation Level) at 500, 1,000, 2,000, 4,000Hz was 84dB, 87dB, 80dB, 87dB on the male right, 84dB, 87dB, 84dB, 87dB on the male left, 85dB, 87dB, 80dB, 88dB on the female right, 85dB, 90dB, 85dB, 88dB on the female left.

16. The mean of the ipsilateral stapedial reflex threshold(Sensition Level) at 1,000, 2,000Hz was 89dB, 82dB on the male right, 89dB, 83dB on the male left, 90dB, 82dB on the female right, 91dB, 85dB on the female left.
