Down 증후군에 있어서 선천성심장기형에 대한 임상적 분석

Down증후군에서 선천성심장기형이 잘 동반된다는 것은 잘 알려진 사실이다. 이 질환에서 심장기형은 다양하게 나타나는데 대부분의 경우 급성 혹은 만성적으로 생명에 위협을 주게된다. 외국에서 발표자에 따라 선천성 심장기형의 발생율 및 기형의 양상이 다르게

보고되고 있는데 우리나라에서는 아직 연구되고 있지 못한 실정이다. 저자들은 1980년 1월부터 1986년 6월까지 만6년6개월동안 연세대학교 의과대학부속 세브란스병원에 입원하여 Down증후군으로 진단받은 186명과 그중 선천성심장기형이 동반되었던 67명에 대하여

임상적 분석을 하였다. 남녀의 성별비는 남아가 1.6:1로 호발하였으며 Down증후군에서 선천성심장기형의 발생율은 36.2%이었고 남아에서는 32.1%, 여아에서는 42.3%의 발생율을 보였다. 동반된 심장기형의 병형별 분포는 심실중격결손이 53.7%로 가장 높았고, 심내먁상결손이 20.9%, 동맥관개존증이 10.4%, 2차공 심방중격결손이 10.4% 그리고 활로사징 4.5%의 순이었다. 또한 공존하고 있는 두가지이상의 심장기형이 동반된 경우은 9예(13.4%)에서 보였으며 그중 동맥관개존증이 6예(8.9%)에서 관찰되어 가장 높은 빈도를 보였다.

비청색증성 심장기형이 동반된 환아중 9예에서 심도자술을 시행하였는데 그중 8예에서 폐동맥혈압이 증가되어있는 소견을 보였다.

It is recognized that Down syndrome is commonly associated with congenital heart disease. A wide variety of cardiovascular malformations have been identified in these patient, and the majority of malformations are acutely or chronically life-threatening. But, clinical evaluation of congenital heart disease with Down syndrome in Korea have not previously studied and the purpose of this study has been to obtain the clinical data in Korea. We evaluated 67 patients with congenital heart disease in Down syndrome at Yonsei University Medical College during 6 and

half years from Jan. 1980 to June, 1986.

Results are summarized as follows;

1) The male to female sex ratio of the patients was 1.6;1.

2) The incidence of congenital heart disease among Down syndrome was 36.2%, that of male, 32.1% and female, 42.3%.

3. The most common lesions were Ventricular septal defect (53.7%), Patent ductus arteriosus(10.4%), Secundum atrial septal defect(10.4%) and Tetralogy of Fallot(4.5%).

4) Co-existing cardiac malformations were seen 9 patients(13.4%). Patent ductus arteriosus was the most commonly associated cardiac malformation and was present in 6 patients(8.9%).

5) Cardiac catherization of 9 patients with Acyanotic congenital heart disease in Down syndrome showed pulmonary hypertension to be present in 8 patients.
