계속적 외래 복강관류 (CAPD) 환자에서 복막을 통한 Tobramycin의 약 역학적 연구

Abstract
[한글]계속적 외래 복강관류(Continuous Ambulatory Peritoneal Dialysis : CAPL)는 넓은 복막면에 관류액이 계속 접촉하여 용질과 수분교환에 효과적이지만 중독한 합병증으로 복막염이 수반될 수 있다.

본 관찰은 CAPD 환자에서 발생하는 복막염과 전신감염을 항균제 투여로 치료하는데 있어서 특히 tobyamycin의 독성을 최소한도로 줄일 수 있는 투여경로와 용량결정에 중점을 두었다.

대상은 복막염이 동반되거나 동반되지 않은 CAPD 환자 20명을 선택하여 계속적 주입 또는 간헐적 주입으로 복강내 tobramycin을 투여하면서 약역학적 관찰에 치중하여 다음과 같은 결과를 얻었다.

1. 복강관류를 통하여 tobramycin 10mg을 관류액 2ℓ에 계속적으로 관류할 때 6시간후 관류액내 tobramycin 농도는 복막염이 있을 때 1.81±0.22㎍/ml였고, 복막염이 없을 때 2.52±0.20 ㎍/ml였으며 6시간 복막을 통한 toramycin 흡수율은 각각 66.2±2.4, 47.7±5.

4% 복막염이 있을 때 tobramycin의 흡수울은 유의하게 증가하였다.

2. 복막염 합병유무에 불구하고 관류액 2ℓ에 obramycin의 농도는 유의한 차이가 없고, 또한 추적검사 기간동안 혈청 tobramycin 농도는 일정하게 유지되었다.

3. 복강관류를 통하여 tobrmycin 1.8mg/kg을 투여하면 복막염의 유무에 따라 6시간후 2ℓ의 관류액내의 tobramycin 농도는 각각 2.52±0.18 ㎍/ml 및 4.31±0.29 ㎍/ml이고 6시간동안 복막을 통한 tobramycin의 흡수율은 각각 70.8±2.5, 48.2±4.8%로 복막염이 있을

때 유의하게 증가하였다.

4. 복막염 합병유무에 불구하고 관류액애 tobramycin 1.0 mg/kg의 간헐적 투여로 6시간후 혈청 tobramycin 농동 (최고 혈청농도)에 큰 차이가 없고, 48시간후의 혈청 tobramycin 농도(최저 혈청농도)도 차이가 없었다.

5. 복강 관류액 2ℓ에 tobramycin을 계속적으로 10mg 혹은 간헐적으로 1.0mg/kg으로 투여하고 혈청농도를 13일까지 추적한 바 계속 항균농도를 유지함을 관찰하였다.

이상 성적으로 보아 간헐적 투여에 있어서 tobramycin 초기용량은 1.8mg/kg, 유지용량은 48시간마다 1.0 mg/kg이 적정용량으로 생각되며, 계속적 투여에서는 매 교환시 tobramycin 2.02∼0.3 mg/kg이 필요하다고 생각된다. 또 CAPD 환자에서 복막염 이외의 감염증을 동반한 환자에서는 계속적 투여가 적합하다고 생각된다.



[영문]To establish guide-lines for methods and dosages of antibiotics in the treatment of CAPL-associated peritonitis by continuous method or pulse method, into 10 patients 96 without and 4 with peritonitis) TBM 10 mg or 0.2 mg/kg was administered every 6 hours mixed in 2ℓ of dialysate (continuous method )and the other 10 patients (6 without and 4 with peritonitis) was received TBM 1.8 mg/kg as loading dose followed by 1.0 mg/kg every 48 hours (pulse method)

Dialysate and serum samples were obtained before and 0.25, 0.5, 1, 2, 4, and 6 hours after TBM administration and thereafter serum samples were obtained every 24 hours up to 13days after first dose of TBM. All samples were frozen at -70 C until TBM concentrations were measured by radioimmunoassay.

The results are as follows.

1. By continuous method, dialysate TBM concentrations at 6 hours after administration was significantly lower in patients with peritonitis (1.8±0.22ug/ml) than in patients without peritonitis (2.52±0.20 ug/ml), and transperitoneal TBM absorption for 6 hours was significantly higher in patients with peritonitis (66.2±2.4%) than in patients without peritonitis (47.7±5.4%)

2. By pulse method, dialysate TBM concentration at 6 hours after administration was significantly lower in patients with peritonitis (2.52±0.118 ug/ml) than in patients without peritonitis (4.31±0.29 ug/ml) and transperitoneal TBM absorpiton

for 6 hours was significatly higher in patients with peritonitis (70.8±2.5%) than in patients without pertionitis (48.2±4.8%)

3. Adequate and similar serum levels of TBM were achieved regardless of peritonitis by both methods of intraperitoneal TBM administration.

These results suggest that the continuous method 0.2-0,3 mg/kg of tobramycin intraperitoneally every 6 hours, is useful for the treatment of CAPD associated peritonitis and that the pulse method with 1.8 mg/kg as a loading dose followed by 1.0mg/kg every 48 hours may be adequate for the treatment of infections other than peritonitis in continuous ambulatory peritoneal dialysis (CAPD) patients.
