Abstract
[한글]요검사(urinalysis)는 신장 및 비뇨생식기계의 질병의 진단과 치료에 다양한 임상적인 정보를 제공해주며, 신장과 직접 관련이 없는 대사성질환등의 생체 변화를 예측케 해주는 검사 종목이다.

저자는 1990년 4월 1일부터 4월 30일 사이에 전북대학병원에 내원한 입원환자 및 외래환자중 물리화학적 요검사 성적이 양성(positive) 으로 나온 검체와 음성(negative)의 결과를 보인 환자 113명(남자 69명, 여자44명)을 대상으로 하여 시험지봉을 이용하여 여러 가지 조건 변화에 따른 요의 물리화학적 검사(physicochemical tests) 성적의 추이를 시험지봉 검사에 의하여 관찰하였다.

보관 온도에 따른 검사 성적의 변화는 F-test로 분석하였고, 시험지봉의 보관 온도 조건에 따른 차이가 가검뇨의 보관 온도 조건에 따른 방치시간별 차이는 짝비교 검정(paired t-test)을 하여 다음과 같은 결과를 얻었다.

1. 가검뇨의 온도 차이(4℃, 20℃, 37℃)에 다른 물리화학적 검사 결과는 유의한 차이를 보이지 않았다.

2. 시험지봉의 보관 온도 조건(4℃와 20℃)에 따른 물리화학적 검사 결과는 적혈구에서만 유의한 차이를 보였으며 (P<0.01), 그 외의 검사 성적들은 유의한 차이를 보이지 않았다.

3. 가검뇨의 보관 온도 변화에 따른 방치 시간별 물리화학적 검사 결과는 다음과 같다.

1) 백혈구는 보관온도에 따른 방치 시간별 변화가 거의 없었으며, 유의한 차이를 보이지 않았다.

2) Nitrite는 실온보관에서 12시간부터 크게 증가하는 경향을 보였으며, 검체의 실온보관과 냉장보관에 따른 유의한 차이를 보였다(P<0.05, P<0.001, P<0.0001).

3) pH, protein, glucose는 검체 보관 온도에 따른 방치 시간별 농도 변화는 거의 없었으며 유의한 차이를 보이지 않았다.

4) Urobilinogen은 실온보관에서 6시간부터 서서히 감소하는 경향을 보였으며, 6시간부터 48시간 사이의 농도간에 유의한 차이를 보였다(P<0.05, P<0.01).

5) Bilirubin은 실온 보관 검체가 12시간 이후로 감소하는 경향을 보였으며, 2224시간치와 48시간치에서 유의한 차이를 보였다(P<0.05).

6) 적혈구는 실온 보관 검체가 2시간부터 감소하는 경향을 보였으며, 2시간치부터 48시간치 사이에 유의한 차이를 보였다(P<0.01, P<0.001, P<0.0001).

이상과 같은 실험 결과를 볼 때 검사에 사용하는 시험지봉의 보관조건은 20℃ 내외의 실온에 보관하는 것이 가장 타당하다고 생각되며, 통상뇨의 물리화학적 검사를 위한 검체의 보관은 실온에 보관할 경우 2시간을 넘지 말것이며, 그이상 검사가 지연될 경우에는

반드시 냉장 온도(4℃)에 보관함으로서 그 신뢰도를 높일 수 있으리라 생각된다.



[영문]Urinalysis is screening laboratory test for the diagnosis and management of renal or lower urinary tract diseases as well as the detection of metabolic or systemic diseases not directly related to the kidney.

Thks study was done to evaluate the physicochemical tests of urine using test strip under various conditions. Urine specimens were collected from the 113 in- and out- patients(male; 69, female; 44) in chonbuk National University Hospital during

April 1 to April 30, 1990. These specimens had been examined in which physicochemical tests had been negative and positive result.

The physicochemical tests with the specimen temperature as the variable were analyzed using a F-test. The differences with the test strip storage temperature and the differences with the specimen storage temperature over time were both analyzed using a paired t-test.

The results were as follows:

1. The results of physicochemical tests by specimen temperature(4℃, 20℃, 37℃) showed no significant difference.

2. In the results of physicochemical tests by test strip storage temperature(4℃and 20℃), only erythrocytes showed significant difference(P<0.01).

3. The results of the physicochemical tests by specimen storage temperature over time were as follows;

1) Leukocytes did not change significantly with specimen storage temperature over time.

2) Nitrites showed a tendency to increase after 12 hrs of storage time at room temperature, and there was a significant difference between specimen storage temperature(P<0.05, P<0.001, P<.0001).

3) pH, protein, and glucose did not change significantly using specimen storage temperature over time. There was no significant difference between specimen storage temperature.

4) Urobilinogen showed a tendency to slowly decrease after 6 hrs of storage time at room temperature and a significant difference between specimen storage temperature from 6 hrs to 48 hrs of storage time(P<0.05, P<0.01).

5) Bilirubin showed a tendency to decrease after 12 hrs of storage time at room temperature and a significant difference between specimen storage temperalure from 24 hrs to 48 hrs of storage time(P<0.05).

6) Erythrocytes showed a tendency to decrease after 2 hrs of storage time at room temperature and a significant between specimen storage temperature from 2 hrs to 48 hrs of storage time(P<0.01, P<0.001, P<0.0001).

In view of these experimental results, It could be concluded that the storage conditions of test strips should be at room temperature, near 20℃. Specimens stored at room temperature must be tested within 2 hrs, and if not, the specimens should be refirgerated at 4℃.
