진료업무수행평가절차의 기준을 적용한 자궁적출환자진료의 질 평가

진료과정에 대한 질 평가를 하기 위하여 1983년 4월부터 1984년 3월까지 1년간 서울시

내에 위치한 한 대학부속병원에서 복식자궁적출술을 받은 환자 114명을 대상으로 진료업

무수행평가절차의 기준을 적용하여 분석하였다.

수술이 타당하지 못하다고 평가된 것은 난소낭종으로 진단된 3예(2.6%)였고 수술의 당

위성을 인정받지 못한 것은 병리학적 진단상 병변이 없는 것으로 판명된 13예(11.4%)였다

.

수술전 1년 이내 팹 스미어를 시행하지 않은 것은 13예(11.4%)였으며 복식부분자궁절제

술을 시행한 것은 (0.9%)였다.

퇴원시 상태는 전례가 양호하였으며 사망한 예는 없었다.

수술전 재원기간이 1일 이하이면서 총재원기간이 9일이하인 것은 65예(57.0%)였다.

입원시의 혈색소가 10gm 이상으로서 수술전 수혈이 실시된 것은 2예(1.8%)였다.

수술기록은 100% 갖추어져 있었으며 수술후 합병증 발생율은 5예(4.4%)였다.

이상 10가지의 요소를 진료업무수행평가절차의 기준을 적용하여 평가한 결과, 평균 92.

6%의 높은 평가기준 충족율을 보여 우리나라 대학부속병원에서의 진료 수준으로서 만족스

러운 결과였다.

[영문]

In order to evaluate medical care procedure. a total of 114 inpatients medical

records for abdominal hysterectomy(excluding radical hysterectomy) discharged

during April 1983 through March 1984 from a university hospital in Seoul were

studied.

The major findings were as follows:

1) Not justified indication of surgical operation cases was 2.6%; all ovarian

cysts.

2) Not justified pathological diagnoses were 11.4%; all no pathological

diagnosis.

3) Not done Papanicolaou smear within one year was 11.4%.

4) There was one case of subtotal abdominal hysterectomy.

5) All discharge status was within standard limit.

6) The mortality was nil.

7) Preoperative stay more than one day and total stay more than nine days was

34.2%.

8) Preoperative transfusion except hemoglobin lefts than 10gm was 1.8%.

9) Operation report was all recorded.

10) Postoperative complication was occured in 4.4%.

11) The mean agreeable rate of the criteria was 92.6%, It is concluded that the

medical care criteria agreed satisfactorily to the Performance Evaluation Procedure.
