한국산 화분 추출액으로 시행한 알레르기 피부반응검사에 관한 연구

317명의 알레르기성 비염 및 천식 환자를 대상으로 소나무(Pinus spp. 이후 Pine이라함

), 참나무(quercus spp. 이후 Oak라함) 포풀라(Populus spp. 이후 popular라함), 오리나

무(Alnus spp. 이후 alder라함), 쑥(Artemisia spp. 이후 sagebrush라함), 돼지풀(Ambros

ia spp. 이후 ragweed라함, 일명 두드러기쑥이라함) 잔디(Zoysia Japonica 이후 grass라

함)의 한국산 화분 추출액으로 피부단자시험(prick test)을 시행하고 양성을 보인예에서

RAST(Radioallergosorbent test)를 시행하여 다음과 같은 결과를 얻었다.

1. 317예의 환자중 남자가 1248예 여자가 169예였고 연령분포는 5세에서 82세까지로 평

균 연령은 34.5세였다.

2. 한국산 화분추출액 7종에 대한 피부단자시험상 양성인 예는 73예로 23.0%였고 이들

중 30세이상 40세미만에 속하는 연령군에서 36.2%로 가장 높은 양성율을 보여 각 연령군

의 양성율 간에는 의미있는 차이(P〈0.05)가 있었으나 남녀별 차이는 없었다(0.05〈P〈0.

1).

3. 한국산 화분 추출액 7종에 대한 각각의 감작정도를 보면 쑥이 14.2%, 두드러기쑥이

10.4%, 잔디가 5.0%, 참나무가 4.1%, 오리나무가 3.8% 포풀라가 1.9%, 소나무가 0%로 쑥

과 두드러기쑥이 가장 높았다.

4. 출생월에 따른 쑥과 두드러기쑥에 대한 양성반응율과 환자의 출생월간에 유의한 연

관성은 없었다.(0.05〈P〈0.1)

5. 알레르기성 질환별 화분 감작정도를 보면 알레르기성 비염이 25.0%, 기관지 천식이

21.5%, 기관지 천식과 알레르기 비염이 동반된 경우가 22.6%로 질환간에 유의한 차이는

없었다.

6. Bencard사제 화분 7종에 대한 감작정도를 보면 쑥이 16.7% 두드러기쑥이 8.8%, 참나

무가 5.4%, 포풀라가 5.0%, 잔디가 4.4%, 오리나무가 4.1%, 소나무가 1.9%로 쑥과 두드러

기쑥이 가장 높았다.

7. 한국산과 Bencard사제 화분 추출액으로 시행한 피부단자시험의 일치율을 보면 소나

무가 95.9%, 참나무가 93.6%, 오리나무가 96.3%, 포풀라가 93.4%, 두드러기쑥이 87.1%,

쑥이 88.1%, 잔디가 93.1%였다.

8. 한국산과 Bencard사제 쑥 화분으로 시행한 피부단자시험의 RAST에 대한 일치율(agre

ement)은 각각 70.0%, 63.1%로 한국산이 높았으며 두드러기쑥 화분의 경우 각각 59.0%, 6

5.7%로 Bencard사제가 높았다.

9. 한국산과 Bencard사제로 시행한 피부단자시험에 대한 RAST의 음성 추정율(negative

probability)과 양성 추정율(positive probability)을 보면 쑥의 경우 음성 추정율은 한

국산이 90.5%, Bencard사제가 89.2% 였고 양성 추정율은 한국산이 42.9% Bencard사제가 3

6.9%였다. 두드러기쑥의 경우 음성 추정율은 한국산이 82.4% Bencard사제가 81.3%, 양성

추정율은 한국산이 37.0%, Bencard사제가 41.4%였다.

이상의 결과로 알레르기성 비염 및 천식 환자의 한국산 화분 7종에 대한 감작율은 23.0

%였고 쑥과 두드러기쑥이 주요 화분으로 생각되었다.

[영문]

To evaluate the incidence of sensitization to Korean pollen, we performed prick

skin tests with 7 home-made and Bencard's pollen extracts-pine, oak, alder,

popular, ragweed, sagebrush, grass-in 317 Koreans with respiratory allergic

diseases. The patients with positive skin tests to the pollen extracts were tested

by RAST for specific IgE. The results of the skin test and the RAST were compared.

The following results were obtained.

1) 73 patients (23%) showed positive reactions to one or more home made

allergenic extracts. Skin test positivity was not different between male and

female. But there was significantly higher positivity in the age group of fourth

decade.(P〈0.05)

2) Home-made sagebrush showed positive skin reactions in 14.2% of total patients,

ragweed in 10.4%, grass in 5.0%, oak in 4.1%, popular in 1.9%. There was no

positive case to home-made pine extract.

3) There was no difference in skin test positivity among rhinitis, bronchial

asthma, and combined cases.

4) There was no relationship between birth season and skin test positivity to

pollen.

5) Bencard's sagebrush showed positive skin reactions in 16.7%, of total

patients, ragweed in 8.8%, oak in 5.4%, popular in 4.4%, alder in 4.1%, pine in

1.9%.

6) the agreement between the results of skin test with home-made and Bencard's

allergenic extracts were 95.9% in pine, 93.6% in oak, 96.3% in alder, 93.5% in

popular, 87.1 in ragweed, 88.1% in sagebrush, 93.1% in grass.

7) The agreement of skin tests with home-made and Bencard's sagebrush extract

with RAST were 70.0%, 63.1% respectively, and those of ragweed were 59.0%, 65.7%

respectively.

8) the positive and negative probabilities of RAST according to skin test with

home-made sagebrush extract were 42.9% and 90.5%, and those with Bencard's

sagebrush extract were 36.9% and 89.2%.

The positive and negative probabilities of RAST according to skin test with

home-made ragweed extract were 40.8% and 82.4%, and those with Bencard's ragweed

extract were 41.5% and 81.3%.
