초음파를 이용한 태아성장표준치에 관한 연구

최근 초음파를 이용한 태아성장지표들의 측정은 임신주령의 예측과 태아체중의 산출에

가장 정확하고 간편한 방법으로 평가되고 있다.

저자는 1983년 3월부터 1987년 6월까지 만 4년 4개월간 연세의료원 산부인과에 내원하

여 임신 11-42주 사이 초음파 촬영을 시행한 임산부를 대상으로 294회의 두정둔부길이, 1

972회의 아두대횡경, 1142회의 대퇴장경 및 968회의 복위를 측정하여 다음과 같은 결과를

얻었다.

1. 두정둔부길이의 성장은 임신 7-12주 사이에 주당 평균 6.9mm이었으며, 임신 9주 이

후 급격히 증가하는 양상과 함께 표준편차도 주령에 비례하여 증가하였다. 평균 두정둔부

길이는 임신 12주에 외국의 기준치들과 큰 차이를 보였다.

2. 아두대횡경의 성장은 임신 11-42주 사이에 주당 평균 2.4mm이었으며, 임신 27주 이

후에 둔화되었다. 평균아두대횡경치는 임신 36주 이후 외국의 기준치들에 비해 약 2-5mm

정도 적었다.

3. 태아대퇴장경의 성장은 임신 11-42주 사이에 주당 평균 2.1mm이었으며, 임신 32주

이후에 둔화되었다. 평균대퇴장경치는 임신 18주 이후 외국의 기준치들에 비해 약2-5mm

정도 적었다.

4. 태아복위의 성장은 임신 11-42주 사이에 주당 평균 9.5mm이었으며, 아두대횡경이나

태아대퇴장경에서와 같이 임신말기에 성장의 둔화는 보이지 않았다. 평균태아복위치는 임

신 34주 이후 외국의 기준치들보다 약2-3.5mm 정도 적었다.

Ultrasonography was performed on pregnant women between 11th and 42nd week of

gestation as antenatal examinations at the Department of Obstetrics and Gynecology,

Yonsei University College of Medicine from May, 1983 to June, 1987.

This study was undertaken to measure the mean values of crown-rump length,

biparietal diameter, femur length and abdominal circumference in each week of

gestation and to compare them with other Korean and foreign normograms of these

parameters.

The results obtained were as follows;

1. The mean growth rate of crown-rump length between 7th and 12th week of

gestation was 6.9mm per week and abruptly increased with standard deviation after

9th week as gestation advanced. The mean crown-rump length at 12th week

significantly differed from that in foreign normogram.

2. The mean growth rate of biparietal diameter between 11th and 42nd week of

gestation was 2.4mm per week and was decreased after 27th week. The mean biparietal

diameters after 35th week were 2 to 5mm smaller than those I foreign normograms of

same gestations.

3. The mean growth rate of femur length between 11th and 42nd week of gestation

was 2.1mm per week and was decreased after 32nd week. The mean femur lengths after

18th week were 2 to 5mm smaller than those in foreign normograms of same

gestations.

4. The mean growth rate of abdominal circumference between 11th and 42nd week of

gestation was 9.5mm per week but there was no flattening of growth observed in

biparietal diameters and femur lengths after mid pregnancy. The mean abdominal

circumferences were 2-3.5cm smaller than those in foreign normograms.
