하악전돌증의 악교정수술후 안면연조직 변화에 관한 연구

하악 전돌증의 수술 후 안면 경조직의 변화 정도에 따른 안면 연조직의 변화 정도를 알기 위해 연세대학교 치과대학 부속병원에서 수술하였던 하악 전돌증 환자 11명(남 6명, 여 5명)을 대상으로 수술 전과 후의 두부 방사선 규격 사진상에서 경조직 및 연조직의 변화를 관찰함으로써 다음과 같은 결과를 얻었다.

1. 수술후 경조직 대 연조직의 수평 변화 비율은 하악점(Pogonion) 부위에서 1 : 0.97, 하순구 (mentolabial sulcus)에서 1 : 0.97, 및 하순 부위에서 1 : 0.99로서 연조직이 경조직에 비해 다소 적게 변화하는 양상을 보였다.

2. 경조직 하악점 (Pogonion)이 후방으로 1mm수평 변화함에 따라 연조직 하악점 (Pogs) 0.97mm, 연조직 B point (Bs) 0.90mm 및 하순 최첨점 (Li) 0.65mm 후방 변화하여, 상부로 갈수록 적게 변화하는 양상을 보였다.

3. 수술후 하순구 (mentolabial sulcus)의 깊이와 상순 비교 돌출도는 거의 변화가 없었고 비순각은 증가, 하순 비교 돌출도는 다소 감소하는 경향을 보였으나, 하악이 후방 변화하는 양과는 유의한 상관관계를 관찰할 수 없었다.

4. 수술후 안면 하부의 경조직 수직고경 (ANS -Gn)은 평균 약 2.58mm, 연조직 수직고경(Sn-Mes)는 평균 약 4.65mm 감소하여 악 1 : 1.80의 비율로 감소하는 경향이 있으나, 수직고경의 변화간의 상관계수는 0.28로서 낮은 상관관계를 보였다.

The purpose of this study is to evaluate the changes of the soft tissue relative to the inderlying skeletal and dental elements before and after orthognathic surgery for the correction of the prognathic mandible, and the influence of the orthognathic surgery quantitatively on the various regions of the facial profile.

11 patients were selected (6 men and 5 women), who had received orthognathic surgery for the correction of their prognathic mandible, and the mean age was 20.5 years.

Lateral cephalometric head films were taken before and after surgery. The results were obtained as follows:

1. The ratio of horizontal changes of hard tissue to soft tissue at pogonion, mentolabial sulcus, and lower lip were almost 1:1.

2. 1 mm of change at Peg resulted in 0.97 mm at Pogs, 0.90 mm at Bs, and 0.65 mm at Li.

3. There were little changes at the depth of mentolabial sulcus and relative upper lip protrusion, but nasolabial angle was increased (average 0.18°) and relative lower lip protrusion was decreased (average -1.45 mm). There was no significant correlation with the changes of hard tissue landmarks.

4. The lower anterior facial height of hard tissue (ANS-Gn) and soft tissue (Sn-Mes) was decreased, but there was no significant correlation between the changes.
