PURPOSE: We analyzed the discrepancies of the infundibular septal malalignment in anteriorly malaligned ventricular septal defect and in the tetralogy of Fallot (TOF) and clarified the clinical significance of the differences.
METHODS: Between March 1993 and August 1996, we conducted a retrospective study on 26 children with anteriorly malaligned ventricular septal defect(AMVSD) and 31 children with TOF.
RESULTS: The angle between anteriorly deviated infundibular septum and interventricular septum and the peak velocity of the right ventricular outflow tract (RVOT) were greater in TOF than in AMVSD, but the length of the anteriorly deviated infundibular septum was longer in isolated VSD than in TOF. The postoperative RVOT peak velocity was found to be related to the degree of anteriorly deviated infundibular septum in isolated VSD, but there was no relationship between the degree of anteriorly deviated septum and Qp/Qs, Rp/Rs, and preoperative RVOT peak velocity, respectively.
CONCLUSION: Comparing the anteriorly malaligned VSD and tetralogy of Fallot, we concluded that there were differences in angles between the anteriorly deviated infundibular septum and the interventricular septum, in lengths of the anteriorly deviated infundibular septum, and in the peak velocities of the right ventricular outflow tract. The degree of the anteriorly deviated infundibular septum had no influence on the right ventricular outflow tract.
