dc.description.abstractPurpose: To evaluate the positive predictive values (PPV) of breast lesions with suspicious or malignant fine needle aspiration (FNA) cytology. Materials and Methods: From April 2001 to July 2008, 24 patients (age: 18-60, mean: 42.69 yrs) who had previously been diagnosed with suspicious FNA cytology were included in our study. Six cases were diagnosed as suspicious for malignancy, 8 as atypia, and 10 as malignancy on the outside FNA cytology. We compared the ultrasonographic(US) findings with the final pathologic results. Results: Lesions on the breast ultrasound measured from 5-40 mm (mean: 15.6 mm). Among the 24 cases, 6 were diagnosed as category 3, 12 as category 4, and 6 as category 5. Ten (41.7%) of the 24 lesions were confirmed as malignancy. One (16.7%) of the 6 cases diagnosed as suspicious for malignancy on outside FNA was confirmed as malignant. Three (37.5%) of 8 cases with atypia on FNA and 6 (60.0%) of 10 cases with malignant FNA were confirmed as malignant. All 6 cases diagnosed as category 3 were confirmed as benign lesions, 3 cases (25.0%) of the 12 category 4 lesions were confirmed as malignancy (33.3%), and all 6 cases diagnosed as category 5 were confirmed as malignancy (100%). Conclusion: Fine needle aspiration cytology of the breast has a positive predictive value of 41.7%, which brings caution in the patient’s diagnosis and further treatment. If the imaging findings are not highly suggestive of malignancy, additional biopsy is recommended.-
