Intravenous thrombolysis using tissue plasminogen activator is an approved treatment of acute ischemic stroke. However, it has limitation
of short therapeutic time window and hemorrhagic complications. This review provides an update of current treatment approaches in acute ischemic stroke within and beyond the current therapeutic 3 hour time window, including novel thrombolytic agents, mismatch based thrombolysis, combined intravenous and intra-arterial therapies, and mechanical thrombectomy.
