임상간호사가 인식한 환자안전역량, 심리적 안전감, 간호관리자의 안전 변혁적리더십이 근접오류 보고의도에 미치는 영향

Abstract

본 연구는 임상간호사가 인식한 환자안전역량, 심리적 안전감, 간호관리자의 안전 변혁적리더십이 근접오류 보고의도에 미치는 영향을 확인하기 위하여 시도된 횡단적 서술적 조사연구이다. 본 연구는 순천향대학교 부천병원 임상연구심의위원회 승인 후 서울 및 경인 소재의 상급종합병원 3곳에서 환자 간호에 직접 참여하는 임상간호사를 대상으로 수행되었다. 연구자는 자료 수집대상 부서에 방문하여 참여의사를 밝힌 간호사만을 대상으로 설문지를 배부하였으며, 총 자료수집은 2024년 7월 17일부터 2024년 7월 26일까지 진행되었다. 설문내용은 총 69문항으로 구성된 자가 보고식 지면 설문지를 활용하였고, 최종 225부의 자료를 분석에 사용하였다. 수집한 자료는 SPSS WIN 26.0 프로그램을 사용하여 기술통계, one-way ANOVA, Pearson’s Correlation coefficient, Multiple linear regression을 이용하여 분석하였다. 본 연구의 주요 결과는 아래와 같다. 본 연구에서 근접오류 보고의도와 주요 변수들 간의 상관관계를 분석한 결과, 환자안전역량은 심리적 안전감(r=0.452. p<.001)과 유의한 정(+)의 상관관계를 보였으며, 간호관리자의 안전 변혁적리더십(r=0.589. p<.001), 근접오류 보고의도(r=0.353. p<.001)와 유의한 정(+)의 상관관계를 보였다. 또한 심리적 안전감은 간호관리자의 안전 변혁적리더십(r=0.547. p<.001), 근접오류 보고의도(r=0.303. p <.001)와 유의한 정(+)의 상관관계를 보였다. 또한 간호관리자의 안전 변혁적리더십과 근접오류 보고의도(r=0.242. p <.001)는 유의한 정(+)의 상관관계를 보였다. 임상간호사의 근접오류 보고의도에 영향을 미치는 요인을 확인하기 위해 다중회귀분석을 실시한 결과 총 경력(β=0.180, p<.001), 1년이내 보고관련 교육 이수경험(유)(β=0.137, p<.05), 환전안전역량(β=0.188, p<.05), 심리적 안전감(β=0.185, p<.05)이 정(+)의 영향을 미치는 것으로 나타났다. 즉 임상간호사의 총 임상경력이 높을수록 근접오류 보고의도가 유의하게 증가하고, 1년 이내 보고관련 교육이수 경험이 있는 임상간호사일수록 근접오류 보고의도가 높아짐을 알 수 있다. 또한 임상간호사가 인지하는 환자안전역량이 높을수록 근접오류 보고의도가 높아지는 것을 알 수 있으며, 심리적 안전감이 높을수록 근접오류 보고의도가 유의하게 증가하는 것으로 나타났다. 하지만 간호관리자의 안전 변혁적리더십은 유의한 영향을 미치지 않는 것으로 나타났다. 간호사의 근접오류 보고의도를 향상시키기 위해서는 환자안전역량을 강화할 수 있는 체계적인 교육 프로그램과 조직 차원의 정책적 지원이 필요하다. 특히, 심리적 안전감이 근접오류 보고의도에 중요한 영향을 미치는 만큼, 처벌 없는 보고 문화 조성, 간호사 간 신뢰 구축, 관리자와의 개방적 소통 등 다양한 방안을 통해 심리적 안전감을 높이는 노력이 요구된다. 본 연구는 근접오류 보고의도를 향상시키기 위한 전략 수립에 있어 개인적 접근뿐만 아니라 심리적 안전감을 조성하는 조직적 환경의 중요성과 이를 이끄는 간호관리자의 역할을 강조했다는 점에서 의의가 있다.

This cross-sectional descriptive study was conducted to identify the effects of clinical nurses' perceived patient safety competency, psychological safety, and nursing unit managers' safety leadership on there near-miss reporting intentions. After obtaining approval from the Institutional Review Board of Soonchunhyang University Bucheon Hospital, Clinical nurses directly involved in patient care from three tertiary hospitals in the Seoul-Inchon regions were recruited. Data collection was conducted from July 17 to July 26, 2024. Questionnaires were distributed only to nurses who voluntarily agreed to participate, and a total of 225 completed responses were used for the final analysis. To identify factors influencing near-miss reporting intention, multiple regression analysis was conducted. The regression model was statistically significant (F=10.046, p<.001) with an adjusted explanatory power of 16.8%. Factors positively influencing near-miss reporting intention included nursing experience(year) (β=0.180, p<.001), Completion of near miss reporting training within the past year (β=0.137, p<.05), patient safety competency (β=0.188, p<.05), and psychological safety (β=0.185, p<.05). Specifically, nurses with more clinical experience and those who had completed near miss reporting training within the past year showed significantly higher near-miss reporting intentions. Moreover, higher levels of patient safety competency and psychological safety were associated with increased near-miss reporting intention. However, nursing unit managers' safety leadership did not have a significant effect. These findings confirm that patient safety competency and psychological safety are key factors influencing clinical nurses' intention to report near-miss. Given the significant role of psychological safety, efforts should focus on fostering a non-punitive reporting culture, building trust among nurses, and promoting open communication, in order to encourage nurses to report near-misses.
