Three-dimensional evaluation of a virtual setup considering the roots and alveolar bone in molar distalization cases

dc.description.abstractWe aimed to evaluate the parallesim and symmetry of the roots and the dehiscences or fenestrations of virtual teeth setup using roots isolated from cone beam computed tomography (CBCT) images. We selected 16 patients with a plan for non-extraction orthodontic treatment with molar distalization. After fabricating the composite teeth by merging the roots isolated from CBCT and crowns obtained from the intraoral scan, we sequentially performed three setups: crown setup considering only the crowns, root setup-1 considering root alignment, and root setup-2 considering the roots and surrounding alveolar bones. The left-right symmetry and parallelism of the roots and the amount of root exposure were evaluated among the setups. Each setup model was printed using a three-dimensional (3-D) printer, and the American Board of Orthodontics Objective Grading System (ABO OGS) scores were compared among the setups. The inclination of the maxillary incisors and the angulation of the mandibular incisors showed better symmetry in the root setups-1 and -2 compared to crown setup (p <0.05). In terms of root parallelism, the mean angulation differences between adjacent teeth in the root setups–1 and –2 were significantly smaller than in the crown setup, except for some posterior teeth (p <0.05). In both the crown setup and root setup-1, the frequency of root exposure of posterior teeth was higher on the buccal side in the maxilla and on the lingual side in the mandible. There was no significant difference in ABO OGS scores among the setups. In non-extraction cases involving molar distalization, a virtual teeth setup aligning the composite teeth within the alveolar bone could improve the symmetry and parallelism of the roots and reduce the risk of root exposure when compared to conventional crown setup while not compromising the quality of occlusion. 본 연구는 CBCT에서 분리한 치근을 이용한 가상 셋업의 치근 평행성 및 대칭성과 치근노출을 평가하는 것을 목표로 하였다. 비발치 구치부 후방이동으로 교정치료 받을 예정인 16명의 환자들을 대상으로, CBCT에서 분리한 치아의 치근부와 구강 내 스캔에서 얻은 치관부를 합성하여 복합 치아를 만든 뒤 치관부만 고려한 crown setup, 치근을 함께 고려한 root setup-1, 마지막으로 치근과 치조골을 함께 고려한 root setup 2 등 세가지 방식의 셋업을 차례로 진행하였다. 각 셋업의 치근 좌우 대칭성 및 평행성과 치근 노출량 및 노출빈도를 평가하였다. 각 셋업 모델은 3-D 프린터로 출력하여 American Board of Orthodontics Objective Grading System(ABO OGS) 점수를 셋업간 비교하였다. 상악 절치 inclination 및 하악 절치 angulation은 crown setup과 비교해 root setup-1, -2가 더 대칭적이었다(p <0.05). 치근 평행성에 있어서, root setup-1, -2의 인접 치아 간 평균 각도 차이는 일부 구치부 치아를 제외하고 crown setup 보다 유의하게 작았다(p <0.05). Crown setup 및 root setup-1에서, 구치부 치아의 치근노출 빈도는 상악은 협측에서, 하악은 설측에서 높았다. 셋업 간에 ABO OGS 점수에는 유의한 차이가 없었다. 구치부 후방이동을 포함하는 비발치 케이스에서 치조골 내에서 복합 치아를 배열하는 가상 치아 셋업은 기존 치관셋업에 비해 교합의 질을 손상시키지 않으면서 치근의 대칭성과 평행성을 개선하고 치근 노출 위험을 줄일 수 있다.-
