Abstract
본 연구는 국내 의료기관의 고가 의료장비 보유 수준과 환자경험평가 결과의 관련성을 알아보고자 진행되었다. 연구대상을 추출하기 위해 건강보험심사평가원에서 시행한 제3차 환자경험평가 결과와 건강보험심사평가원의 2021년 의료기관 상세 현황, 의료장비 상세 현황 자료를 활용하였다. 이를 통해 2021년에 고가 의료장비인 CT, MRI, PET를 모두 보유했거나, CT와 MRI만 보유한 의료기관 320곳을 대상으로 선정하였다. 독립변수는 의료기관의 고가 의료장비(CT, MRI, PET) 보유 수준과 의료기관의 특성(설립연도, 설립지역, 설립형태, 의과대학 소속 여부, 병상 수, 전문의 수)으로 선정하였다. 종속변수는 환자경험평가 영역인 전반적 평가, 간호사 영역, 의사 영역, 투약 및 치료과정, 병원환경, 환자권리 보장의 6개 영역으로, 각 의료기관의 영역별 평균 점수를 사용하였다. 의료기관의 고가 의료장비 보유 수준과 환자경험평가 결과의 관련성을 파악하기 위해 다중 회귀분석(Multiple Regression Model)을 시행하였다. 연구 결과 CT, MRI, PET를 모두 보유한 경우가 CT와 MRI만을 보유한 경우에 비해 전반적 평가 영역과 의사 영역, 병원환경 영역, 환자권리 보장 영역에서 환자경험평가 평균 점수가 높았다. 병원환경 영역이 2.22점 높았고, 전반적 평가 영역이 2.15점, 환자권리 보장 영역이 1.77점, 의사 영역이 1.54점 순서였다. 따라서 의료기관의 기술적 요인이 의료기관을 찾는 환자가 느끼는 의료의 질에 대한 만족도와 관련성이 있다고 확인된 바, 의료기관에서는 환자경험을 높이기 위해 이러한 부분도 고려할 필요가 있을 것이다.
This study was conducted to investigate association between domestic medical institutions’ retention of high-cost medical equipment and patient experience evaluation results. We used the results of the 3rd Patient Experience Assessment, conducted by the Health Insurance Review & Assessment Service(HIRA), and the data of HIRA's 2021 Detailed Status of Medical Institutions and Detailed Status of Medical Equipment to extract research subjects. As a result, 320 medical institutions that owned all of the high-cost medical equipment(CT, MRI, and PET) or only CT and MRI in 2021 were selected as subjects. The independent variables were the retention of high-cost medical equipment (CT, MRI, PET) and the characteristics of the medical institution (year of establishment, region of establishment, type of establishment, affiliation with a medical school, number of beds, number of specialists). The dependent variable was the average score of each hospital's six areas of patient experience evaluation: overall evaluation, nurses category, doctors category, medication and treatment process, hospital environment, and patient rights guarantee. A multiple regression model was conducted to determine association between medical institutions’ retention of high-cost medical equipment and patient experience evaluation results. The study found that the part with CT, MRI, and PET had higher average patient experience rating scores than only with CT and MRI in overall evaluation, doctors category, hospital environment, and patient rights guatantee. The hospital environment category was 2.22 points higher, followed by the overall evaluation category at 2.15 points, the patient rights guarantee category at 1.77 points, and the doctors category at 1.54 points. Therefore, it has been shown to be related to patients' satisfaction with the quality of care they receive, it's important for medical institutions to consider these factors in order to improve patient experience.
