Abstract
Globally, the number of elderly patients is increasing, and they have a risk of polypharmacy compared to other populations. They are also exposed to risks of drug interaction by physiologic changes on absorption, distribution, metabolism and excretion with aging. As a result, the need of appropriate medication strategy for elderly patients is under the discussion. One of the medications with caution is benzodiazepine (BZD), and long-term use of BZD and long-acting BZD use in elderly patients are used as indicators of prescribing in primary care by OECD. Insomnia is one of indications that BZDs are commonly prescribed for, and as the aging itself affect to quality of sleep, elderly patients are vulnerable to sleep disorder with increased exposure to sleeping pills. This study was conducted to examine the prescribing patterns of benzodiazepine for elderly insomnia patients by specialty of physicians considering personal and clinic factors using the HIRA-Aged Patient Sample database (2002-2013). Total of 70,247 elderly patients, newly diagnosed with insomnia and prescribed BZD, were analyzed for the BZD prescription for a year after diagnosis. For general characteristics of study subjects, chi-square test was done on personal and hospital variables for BZD prescription patterns. To analyze the association between inappropriate BZD prescription and specialties of physician with variables, multilevel logistic regression was performed. Test results were presented as odds ratios with 95% confidence interval. 48.6% of study subjects were prescribed BZD for long-term or long-acting BZD. The most frequently prescribing specialty was internal medicine (IM, 41.8%) and the least prescribing specialty was psychiatry (9.4%). The proportion on inappropriate BZD prescription was 54.3% for orthopedic surgery (OS) followed by 50.1% for the others, 49.8% for IM, 43.5% for psychiatry and 40.3% for neurology. The difference of prescribing patterns was associated with age, income, type of health insurance and history of psychiatric illness before insomnia diagnosis. Compared to psychiatry, odds ratios for inappropriate BZD prescription of most of specialties were significantly higher except neurology (neurology OR=0.92, 95% CI=0.85-0.99; IM OR=1.25, 95% CI=1.17-1.33; the others OR=1.39, 95% CI=1.29-1.49; OS OR=1.41, 95% CI=1.30-1.53). In a subgroup analysis by specialties, odds ratio of OS was the highest in both genders (male OR=1.46, 95% CI=1.27-1.67; female OR=1.42, 95% CI=1.29-1.57). By across of age group, 70-74 group showed the highest OR for the others (OR=1.38, 95% CI=1.22-1.55), over 80 group showed the highest OR for internal medicine and orthopedic surgery (OR=1.59, 95% CI=1.19-2.13) and other age groups showed the highest OR for OS (65-69 OR=1.44, 95% CI=1.22-1.70; 75-79 OR=1.55, 95% CI=1.32-1.82; 80-84 OR=1.66, 95% CI=1.35-2.05). By the insurance type, all the type showed the highest OR for OS (Employee health insurance OR=1.37, 95% CI=1.24-1.52; Local-subscriber health insurance OR=1.52, 95% CI=1.32-1.75; Medical aid OR=1.82, 95% CI=1.41-2.34). Subgroup analysis of 5 specialties showed significant difference of BZD prescribing patterns among subgroups. Compared to psychiatry, other specialties showed a significantly lower OR for long-term prescription of BZD, and other specialties showed a higher OR for long-acting BZD. The biggest difference was seen in fraction of long-acting BZD ≥ 50% (IM OR=1.77, 95% CI 1.59-1.96; OS OR=2.44, 95% CI 2.17-2.75; the others OR=2.69, 95% CI 2.41-3.01). This study showed that prescribing patterns of BZD for newly diagnosed insomnia elderly patients were different among specialties of physicians. As many specialists are taking a role as a primary physician, additional study for the reason of prescribing difference seems to be needed.

전세계적으로 노인 인구는 증가 추세이며, 다른 환자들보다 많은 약제를 처방받는다. 또한 노화로 인해 약물의 흡수, 분포, 대사 및 배설에 영향을 받아 약물 상호작용의 위험이 높다. 따라서 노인 환자에게 적절한 약물치료의 필요성이 대두되고 있다. 대상이 되는 약제 중 하나가 벤조다이아제핀(Benzodiazepine, BZD)계 약물로, 경제개발협력기구(Organization for Economic Cooperation and Development, OECD)의 Health Care Quality Indicator(HCQI) project에서도 1차 의료 약제 처방의 지표 중 하나로 노인 환자에서 BZD계 약물의 장기 처방률과 장기작용 BZD계 약물 처방률을 사용하고 있다. 불면증은 노인 환자들이 BZD계 약물을 처방받는 대표적인 질환이며, 노화 과정 자체로 수면 장애에 취약하기 때문에 그에 대한 약물 복용도 빈번할 것임을 짐작할 수 있다. 이 연구는 처방 진료과목에 따른 노인 불면증 환자의 BZD 처방 행태를 개인과 요양기관의 특성을 고려하여 분석하고자 시행되었다. 국민건강보험공단의 노인코호트DB(2002년~2013년)을 이용하여, 불면증을 새롭게 진단받은 노인환자가 진단 후 1년 간 처방받은 BZD계 약물을 분석하였다. 통계분석은 먼저 대상자의 일반적 특성을 파악하기 위해 BZD 처방 행태에 따른 개인 수준 및 요양기관 수준 변수에 대하여 카이제곱 검정을 시행하였다. 그리고 약물을 처방한 진료과목을 정신과, 내과, 신경과, 정형외과, 그 외 진료과 5개로 구분하여 부적절한 BZD 처방행태와의 관련성을 개인 수준 및 요양기관 수준의 변수를 고려하여 파악하기 위해 다수준 로지스틱 회귀분석(multilevel logistic regression)을 시행하였다. 분석결과는 오즈비(Odds Ratio, OR)와 95% 신뢰구간(95% Confidence Interval, 95% CI)을 산출하였다. 전체 연구대상자인 70,247명 중 48.6%(34,138명)는 BZD계 약물을 장기처방 받거나 또는 장기작용 BZD을 처방받았다. 가장 많이 처방한 과는 내과(41.8%, n=29,398명)였고, 가장 적게 처방한 과는 정신과(9.4%, n=6,625명)이었다. 각 진료과의 부적절한 BZD 처방 행태 비율은 정형외과 54.3%(4,700명), 그 외 진료과 50.1%(8,262명), 내과 49.8%(14,631명), 정신과 43.5%(2,881명), 신경과 40.3%(3,664명) 순이었다. 연령, 소득수준, 의료보장 유형, 불면증 진단 전 정신질환의 이력 여부에 따라 BZD 처방 행태에 차이를 보였다. 정신과를 기준으로 각 과의 부적절한 BZD계 약물 처방의 OR는 신경과 0.92 (95% CI=0.85-0.99)로 약간 감소하였으나, 내과 1.25 (95% CI=1.17-1.33), 그 외 진료과 1.39 (95% CI=1.29-1.49), 정형외과 1.41 (95% CI=1.30-1.53)로 유의하게 높았다. 진료과목별 하위그룹 분석 시 남성과 여성 모두 정형외과에서 부적절한 BZD 처방의 OR이 가장 컸다(남성 OR=1.46, 95% CI=1.27-1.67; 여성 OR=1.42, 95% CI=1.29-1.57). 연령별로 보면 70-74세는 그 외 진료과에서 OR이 1.38 (95% CI=1.22-1.55)로 가장 컸고, 80세 이상은 내과와 정형외과의 OR이 1.59 (95% CI=1.19-2.13)로 가장 높았으며, 그 외 대부분의 연령군은 정형외과에서의 OR이 가장 컸다(65-69세 OR=1.44, 95% CI=1.22-1.70; 75-79세 OR=1.55, 95% CI=1.32-1.82; 80-84세 OR=1.66, 95% CI=1.35-2.05). 그리고 모든 의료보장 유형에서도 정형외과의 OR이 가장 높았다(직장가입자 OR=1.37, 95% CI=1.24-1.52: 지역가입자 OR=1.52, 95% CI=1.32-1.75; 의료급여 OR=1.82, 95% CI=1.41-2.34). BZD 처방 행태를 5개의 하위그룹으로 나눠 분석한 결과, 5개 진료과의 처방 행태의 차이가 유의하였다. BZD을 장기처방하는 경우는 정신과에 비해 다른 진료과의 OR이 유의하게 낮았고, 대신 다른 진료과에서는 장기작용 BZD 처방의 OR이 높았다. 장기작용 BZD의 처방 분율이 50%이상인 경우, 가장 큰 차이를 보였는데, 내과의 OR은 1.77 (95% CI 1.59-1.96), 정형외과 2.44 (95% CI 2.17-2.75), 그 외 진료과 2.69 ( 95% CI 2.41-3.01)였다. 이 연구를 통하여 처음 불면증을 진단받고 BZD을 처방받은 노인환자에서 약물을 처방받은 진료과목이 정신과, 신경과, 내과, 정형외과, 그 외 진료과에 따라 처방 행태에 차이가 있음을 확인하였다. 다수의 각 과목의 전문의가 1차 의료의 역할을 하고 있는 한국의 의료상황을 고려할 때, 진료과목 별 처방행태에 차이를 보이는 원인에 대한 추가적인 연구가 필요할 것으로 사료된다.
