우리나라 청장년의 가구형태와 우울과의 관련성: 국민건강영양조사 2016, 2018년 자료 분석

Abstract
Objective The purpose of this study is to analyze the relationship between household type and depression in Korean young and middleaged people through representative sample data analysis. While correcting the demographic and social factors and health-related factors in the study subjects according to gender aims to analyze the impact on the household form of depression. Methods This study used data from the National Health and Nutrition Examination Survey 2016 and 2018, and the subjects of the study were 9,829 adults aged 19-65 out of a total of 16,142 people. The variables for household type used in this study were 'single-person household', 'first-generation household (couple, other)', 'secondgeneration household (one child)', 'second-generation household (2 children or more)', and 'Other'. Depression was measured using the Patient Health Questionnair-9, we define that in this case more than 10 points depressed. Multiple logistic regression was performed to confirm the association between household type and depression. Results As a result of comparative analysis of depression by gender in this study, 309 subjects confirmed that depression was significant with a PHQ-9 score of 10 or higher, 3.86% of the total study subjects, 104 men in 2.9%, and 2.9% in women. There were 205 people, showing a gender difference of 4.7%. The odds ratio of depression was not significant for both male and female groups based on 'second-generation households (2 or more children)'. In the case of men, in the case of demographic variables, 'off-theshelf' was 'off-the-shelf' in middle occupation, 'high' in terms of 'stress' to 'low' among health-related factors, and 'Yes' in unmet medical care (medical). ' was statistically significant as 'no', and 'no' as 'good' in subjective health status was statistically significant. In the case of women, among demographic variables, 'under middle school graduation' based on 'university graduate or higher' in education level, among health-related factors, 'current smoker' on the basis of 'non-smoker' and 'stress' It was statistically significant at 'high' on the basis of 'low', 'no' on the basis of 'yes' in unmet medical care (dental), and 'no' on the basis of 'good' in subjective health status. The variables that could be confirmed to be significant in both men and women were stress and subjective health status variables in health-related factors. Conclusion In the analysis according to gender, it was confirmed that the relationship with depression according to household type was not significant. This indicates that the relationship with depression can be confirmed differently depending on the subdivided household type, and the relationship between household type and depression may not be significant as various independent variables are excluded. In addition, in the case of stress, unmet medical care (surgery), unmet medical care (dental), and subjective health status, the close association with depression was confirmed regardless of gender. Compared to the elderly, young adults who are relatively healthy and have low medical service demand and use may also be related to depression due to health-related factors. This suggests that single-person households tend to feel isolated from family and social networks, and unmet medical care may occur due to reasons such as having no one to turn to for help when sick, and subjective health status may change.

이 연구는 대표성을 갖는 표본자료 분석으로 우리나라 청장년층의 가구형태와 우울과의 관련성에 대하여 성별에 따른 연구대상자의 인구사회적 요인과 건강관련 요인을 보정하면서 가구형태가 우울에 미치는 영향에 대해 분석하는 것을 목적으로 한다. 이 연구는 국민건강영양조사 2016, 2018년 자료를 이용하였으며, 연구 분석의 대상자는 총 16,142명 중 만 19-65세의 성인 9,829명을 대상으로 우울증 환자 364명과 결측치 1,463명을 제외한 8,002명을 연구 대상으로 선정했다. 본 연구에서 사용한 가구형태에 대한 변수는 ‘1인가구’, ‘1세대 가구(부부, 기타)’, ‘2세대 가구(자녀 1명)’, ‘2세대 가구(자녀 2명 이상)’, ‘기타’로 총 5범주로 나눴다. 우울은 Patient Health Questionnair-9를 이용하여 측정하였으며, 10점 이상일 경우 우울이 있다고 정의하였다. 가구형태와 우울과의 연관성을 확인하기 위해 다중회귀분석(Multiple logistic regression)을 수행하였다. 청장년의 가구형태와 우울과의 관련성에 대한 본 연구에서는 제 7차 국민건강영양조사 자료 중 2016년, 2018년 자료를 이용하여 만 19세 이상 만 65세 이하 청장년에서 가구형태와 우울과의 관련성을 성별에 따라 분석하였다. 인구사회적 및 건강관련 요인을 보정한 모형 3을 중심으로, 선행 연구의 결과와 비교하여 정리한 내용은 다음과 같다. 본 연구에서 우울을 성별로 비교 분석한 결과, PHQ-9 10점 이상으로 우울에 유의한 것으로 확인된 대상자는 309명으로, 전체 연구 대상자의 3.86%로, 남성에서 104명으로 2.9%, 여성에서 205명으로 4.7%로 성별의 차이를 보였다.‘2세대 가구(자녀 2명 이상)’을 기준으로 남성과 여성 두 군 모두에게 우울의 오즈비는 유의하지 않았다. 남성의 경우, 인구사회적 변수에서 중 직업에서 ‘사무직’을 기준으로 하여 ‘무직’에서 3.35(95% CI, 1.68-6.68)로 우울의 오즈비가 유의함을 확인할 수 있었다. 건강관련 요인 중에서는 ‘스트레스’에서 ‘낮음’을 기준으로 하여 ‘높음’에서 10.56(95% CI, 6.05-18.45), 미충족의료(의과)에서 ‘예’를 기준으로 하여 ‘아니오’에서 0.32(95% CI 0.19-0.56), 주관적 건강상태에서 ‘좋음’을 기준으로 ‘아니오’에서 7.75(95% CI 3.32-18.08) 으로 통계적으로 유의하였다. 그 외에는 통계적으로 유의하지 않았다. 여성의 경우, 인구사회적 변수 중 교육수준에서 ‘대졸 이상’을 기준으로 하여 ‘중졸 이하’에서 2.07(95% CI, 1.16-3.72)로 우울의 오즈비가 유의함을 확인할 수 있었다. 건강관련 요인 중에서는 흡연에서 ‘비흡연자’를 기준으로 하여 ‘현재 흡연자’에서 2.47(95% CI, 1.58-3.86), ‘스트레스’에서 ‘낮음’을 기준으로 하여 ‘높음’에서 11.62(95% CI, 7.71-17.52), 미충족의료(치과)에서 ‘예’를 기준으로 하여 ‘아니오’에서 0.65(95% CI 0.47-0.91), 주관적 건강상태에서 ‘좋음’을 기준으로 ‘아니오’에서 4.61(95% CI 2.69-7.91) 으로 통계적으로 유의하였다. 그 외에는 통계적으로 유의하지 않았다. 남성과 여성 모두에서 유의함을 확인할 수 있었던 변수는 건강관련 요인에서 스트레스, 주관적 건강상태 변수였다. 성별에 따른 분석에서 가구형태에 따른 우울과의 관련성 결과값이 다변수 분석에서 동일하게 관련성이 유의하지 않음을 확인하였으며 이는 세분화된 가구형태에 따라 우울과의 관련성이 다르게 확인될 수 있으며 다양한 독립변수를 제함에 따라 가구형태와 우울과의 관련성이 유의하지 않음을 나타낸다. 또한 스트레스, 미충족의료(의과), 미충족의료(치과), 주관적 건강상태의 경우 우울과의 밀접한 연관성을 성별과 관계없이 모두 확인할 수 있었으며, 노인층에 비해 상대적으로 건강하며 의료 서비스 요구도 및 이용도가 낮은 청년, 중년, 장년층에서도 건강관련 요인으로 인해 우울과의 관련성이 있을 수 있음을 확인하였다. 이는 1인가구의 경우 가족 및 사회적 관계망에서 고립감을 느끼기 쉽고, 아플 때 도움을 요청할 사람이 없는 등의 이유로 미충족 의료가 발생할 수 있으며 이로 인한 주관적 건강상태도 변화할 수 있음을 시사한다. 향후 가구형태와 우울과의 연관성을 보다 심도깊게 분석 및 연구를 진행하여 앞으로 지속적으로 증가하게 될 추가적인 1인가구의 정신건강 보건 정책 기틀 마련을 위한 후속 연구가 필요하다.
