Effect of intentional restriction of venous return on tissue oxygenation in a porcine model of acute limb ischemia

Abstract
A sufficient oxygen supply to ischemic limb tissue is the most important requirement for wound healing and limb salvage. We investigated whether partial venous occlusion in the common iliac vein (CIV) causes a further increase of venous oxygenation in a porcine model of acute hindlimb ischemia. In 7 pigs, the model of acute hindlimb ischemia was created with embolization of the common iliac artery. Oxygen saturation was evaluated at baseline (T0), just after the arterial embolization (T1), at 10 minutes (T2), at 20 minutes (T3), and at 40 minutes (T4). Next, an intentional partial venous occlusion was achieved by inflating the vascular balloon at the level of the right CIV. Then, blood sampling was repeated at 5 minutes (T5), at 15 minutes (T6), and at 25 minutes (T7). The arterial oxygen saturation in the right SFA was similar during all phases. In contrast, after arterial embolization, an immediate reduction of venous oxygen saturation was observed (from 85.57 ± 1.72 at T0 to 71.86 ± 7.58 at T4). After the partial venous occlusion, the venous oxygen saturations (T5-T7) were significantly increased, again. The venous oxygen saturations evaluated in the hindlimb ischemia with partial venous occlusion and in the control limb were significantly over time. Venous oxygen saturations in the experimental limbs were higher than those in the control limbs (79.71 ± 4.78 vs 60.00 ± 4.24 at T7, p-value <0.001). Partial venous occlusion results in an increase of venous oxygen saturation in the ischemic limb, while significant changes in venous oxygen saturation are not observed in the control limb. An explanation for this may be that the oxygen consumption in the limb tissue is increased because it gets congested with the partial venous occlusion in the right CIV.

하지 허혈 조직에 충분한 산소의 공급은 상처 회복 (wound healing)과 사지 구제 (limb salvage)에 중요하다. 본 연구는 돼지 급성 하지허혈 모형 (acute limb ischemia)에서 인위적 총장골정맥 (common iliac vein)의 부분 폐쇄 (partial occlusion)를 통한 정맥 환류 (venous return) 제한이 하지 허혈 조직 산소포화도 (tissue oxygenation)에 미치는 영향에 대하여 조사하였다. 실험은 총 7마리 돼지를 사용하였다. 급성 하지허혈 모형을 만들기 위하여 혈관 폐색장치 (vascular plug)를 이용하여 색전술을 통한 우측 총장골동맥 (common iliac artery)의 통한 완전 폐쇄를 유도하였다. 조직 산소 포화도 측정은 우측 표피대퇴동맥 (superficial femoral artery)과 동측 총장골정맥에서 채취한 혈액으로 혈액가스검사 (blood gas analysis)를 시행하였다. 순서는 처음 (T0)과, 동맥 색전술 직 후 (T1), 동맥 색전술 10분 후 (T2), 20분 후 (T3), 40분 후 (T4), 이어서 인위적으로 혈관내 풍선 확장술 (intravascular balloon inflation)로 총장골정맥을 부분 폐쇄를 시켜 정맥 환류 제한을 유도한 다음 5분 후 (T5), 15분 후 (T6), 마지막으로 25분 후 (T7)으로, 총 8번을 평가하였다. 결과 상, 표피대퇴동맥에서 측정된 동맥혈의 조직 산소포화도는 모든 단계에서 큰 변화가 없었다. 반면에, 총장골동맥 색전술 후 총장골정맥의 조직 산소포화도는 의미 있게 감소하였다 (T0 단계 85.57 ± 1.72 %에서 T4 단계 71.86 ± 7.58% 로 감소). 흥미롭게도 총장골정맥을 부분 폐쇄한 후에는, 정맥혈 조직 산소포화도가 다시 증가하였다 (T5 단계 74.14 ± 9.56 %에서 T7 단계 79.71 ± 4.78% 로 증가). 한편, 좌측 하지는 총장골정맥을 폐쇄하지 않은 급성 하지허혈 대조군 (control limb)으로 설정하여, 우측 실험군 (experimental limb)과 조직 산소포화도의 차이를 분석하였는데, 정맥혈 조직 산소포화도는 시간이 지남에 따라 실험군에서 대조군보다 의미있게 높았다 (T7 단계에서 실험군은 79.71 ± 4.78 %, 대조군은 60.00 ± 4.24 %). 위 결과들을 종합할 때, 급성 하지허혈 모형에서 총장골정맥의 부분 폐쇄는 정맥혈의 일시적인 울혈 및 압력 상승을 유도하여, 조직산소포화도를 증가시키고, 허혈 조직이 산소를 이용할 수 있는 시간을 늘려, 상처 회복에 도움이 될 수 있는 가능성을 보여주었다. 향후, 위와 같은 기전의 유용성을 검증하는 후속 연구가 필요하다.
