Genomic architecture of invasive mucinous adenocarcinoma of the lung with multiple pulmonary lesions

Abstract

Invasive mucinous adenocarcinoma (IMA) of the lung frequently presents with diffuse pneumonic-type features or multifocal lesions, which are regarded as a pattern of intrapulmonary metastases. However, the genomics of IMA have not been studied. The purpose of this study was to determine the genomic architecture of IMA manifesting in the lung through multiple pulmonary lesions by multi-region whole exome sequencing. Whole exome sequencing was performed on samples taken from 24 regions in seven patients with multifocal lung IMAs. Early initiating driver events, such as KRAS or NKX2-1 mutations, are clonal mutations and were present in all multifocal IMAs in each patient. In two patients, adenocarcinomas were synchronously located in the bilateral lower lobes and had different mutation types, suggesting that they were double primaries. The tumor mutation burden of multifocal IMAs was low (mean: 1.01/mega base), but the Jaccard similarity coefficient suggested intra-tumor heterogeneity. Mutational signature analysis found six mutational signatures, of which signature 1, an age-related signature, was the most common. In conclusion, genomic evidence found in this study indicated the clonal spread of diffuse pneumonic-type or multifocal IMAs. IMAs synchronously located in the bilateral lower lobes can be double primaries, which indicates that these tumors should be carefully staged. IMAs exhibited a heterogeneous genomic landscape despite the low somatic mutation burden. Further studies are warranted to determine the clinical significance of the genomic characteristics of IMAs in expanded cohorts.

폐의 침습성 점액선암은 영상의학적 소견상 흔히 폐렴과 유사한 다발성 병변의 형태로 나타나며, 이는 다발성 원발암이 아닌, 폐내의 전이성 병변으로 간주된다. 그러나 아직 이를 뒷받침 하는 유전자 분석은 시도된 바가 없다. 따라서 이 연구에서는 차세대 염기서열분석 (next generation sequencing, NGS)을 통해 다발성 폐 평변 형태로 나타나는 폐의 침습성 점액선암의 전장 유전체 분석 (whole exome sequencing, WES)을 시도하여 본 종양의 성질을 분석하고자 한다. 분석을 위해 다발성 폐 병변 형태로 나타난 총 7명의 페의 원발성 침습성 전액선암 환자의 24개의 구역에서 전장 유전체 분석을 시행하였다. 분석 결과, 암 유발 돌연변이 (driver mutation)중 KRAS, NKX2-1등의 유전자의 변이는 모든 종양의 구역에서 나타나는 클론성 변이 (clonal mutation)로 확인되었다. 폐의 양측 하엽에서 동시에 침습성 점액선암이 발생한 두 환자의 경우에는, 서로 다른 종류의 암 유발 돌연변이가 관측되었으며, 이는 다발성 원발암의 가능성을 배제할 수 없는 소견이다. 측정한 종양 조직 변이 부담 (tumor mutation burden)은 모든 구역에서 낮게 측정되었으며, 그 중위값은 일백만 염기서열당 1.01개였다 (1.19/megabase). 그러나, 계통수 분석 (phylogenetic tree analysis) 및 자카드 유사도 계수 (Jaccard similarity coefficient) 분석 결과, 높은 정도의 종양내 이질성 (intra-tumor heterogeneity)이 관찰되었다. 암유전체의 돌연변이 패턴분석 (mutational signature analysis)에서는 1번 패턴이 대부분의 구역에서 관찰되었는데, 이는 노화와 연관된 돌연변이 패턴이다 (age-related signature). 결론적으로 이 연구를 통해서 다발성 폐병변 형태로 나타나는 폐의 침습성 점액암은, 클론성 변이를 통해 전파된다는 것에 대한 유전적 증거를 제시할 수 있었다. 다만, 동시에 폐의 양측 하엽에 병발한 침습성 점액선암의 경우는 다발성 원발암의 가능성을 완전히 배제할 수는 없으므로 폐암의 병기 설정에 주의가 요구된다. 폐의 침습성 점액선 암의 경우 낮은 종양 조직 변이 부담에도 불구하고 높은 수준의 종양내 이질성이 관찰되었다. 이러한 특징적인 종양의 형태를 보다 정확하게 규명하기 위해서, 대규모의 환자 코호트 연구가 추후 필요할 것으로 사료된다.
