Abstract

Various techniques have been used in reconstruction of the soft tissue defect accompanying fingertip iruurias:local flap such as the bilateral volar V-Y advancement flap, regional flaps such as the cross-finger flap, palmar flap, neurovascular island flap and groin flap as an example of a distant flap. To say nothing of the unique anatomical characteristics of the skin and subcutaneous tissue covering the volar aspect of the fingertip, the previous methods had the problem of finger length shortening, sensory abnormalities and significant donor site morbidities Furthermore, in case of distal phalangeal bone exposure accompanying soft tissue defect of more than 2.5cm. , the above mentioned techniques have limitations in reconstruction and restoring the function of the fingers, frequently leading to significant donor site deficit at the same time. Authors have used the inner sole area which bears distinct anatomical characteristics similar to the volar side of the fingertips, as an instep free flap, in four cases of fingertip reconstruction. We have obtained satisfactory results and no functional deficit was found at the donor site. The cases are here presented along with a review of the current literatures.
