요로전환술을 받은 남성 방광암 환자의 증상 클러스터와 건강관련 삶의 질

dc.description.abstractBladder cancer is the seventh most common malignancy in the world and patients experience various symptoms. The simultaneous appearance of two or more of these symptoms is called‘Symptom Cluster’, and its importance is increasing in practice as it requires an integrated approach and intervention of symptoms that are related to each other, rather than to a single symptom. Although many studies have reported that distressing symptoms affect quality of life in patients with various cancers, only a few studies have been done with bladder cancers patients. This study aimed to identify the symptom clusters and their relative impact on quality of life in male bladder cancer patients with urinary diversion. This study was a descriptive and cross sectional study. Participants were male bladder cancer patients who underwent radical cystectomy and urinary diversion more than 6 months after the primary diagnosis. Data were collected using self-reported questionnaires from inpatients and outpatients at the department of urology, Y university hospital in Seoul, Korea from September 11th, 2019 to November 30th, 2019. The collected data were analyzed with descriptive statistics, frequency analysis, independent sample t-test, analysis of variance (One Way ANOVA), correlation analysis, factor analysis, stepwise multiple regressions, Cronbach’s alpha using SPSS 25.0 program. The results of this study are as follow: 1. A total of 120 patients participated in this study. The mean age was 71.31(±8.88) and 109(90.8%) of them had spouses and caregivers and caregivers. The most common stage of cancer diagnosis was stage Ⅲ and Ⅳ(60.0%). Seventy-nine out of 120 patients (65.7%) received an ICUD, 29 out of 120 patients (24.2%) received a Ureterocutaneostomy and 12 out of 120 patients (10.0%) received a Neobladder. The average time from surgery to participate the study was 54.34(±57.97) months. 2. The mean score for HRQoL was 96.39(±26.90) out of 168. The mean scores for physical, emotional, social, functional well-being and radical cystectomy-specific concerns were 21.13(±5.13), 15.95(±5.79), 14.96 (±5.02), 14.28(±7.07) and 30.08(±10.11) respectively. At the 100 point scale, the radical cystectomy-specific concerns were the lowest. 3. The mean score of symptom experience of this study was 2.48(±0.63) on the a scale of 4. The most severe symptoms was erection dysfunction(0.28±0.81), inability to enjoy life(1.56±1.29) and lower content with quality of life(1.62±1.32). The most mild symptoms was nausea(3.58±0.85), difficulty controlling urine(3.42±1.16), and dizziness(3.36±0.96). 4. Three symptom clusters were identified: Fatigue-weight loss cluster, Emotional distress cluster, and Appetite-sleep disturbance-QoL cluster. The most severe symptom cluster was Appetite-sleep disturbance-QoL Cluster(1.90±1.03), followed by Emotional distress cluster(2.49±1.04), and Fatigue-weight loss cluster(2.69±1.61). 5. In the stepwise multiple regressions, the factors to affect HRQoL were Appetite-sleep disturbance QoL cluster(β=14.366, p<.001), followed by Fatigue-weight loss cluster(β=5.454, p<.001), Emotional distress cluster(β=7.036, p<.001), age(β=-.351, p= .016) and education level(β=5.730, p= .027). The regression model accounted for 77.8% of the variance (F=79.236, p<.001, R2= .778). The results showed that male bladder cancer patients with urinary diversion experienced various symptoms at the same time, and the identified symptom clusters affected the overall health related quality of life. Therefore, nursing practice requires a broad understanding of symptom clusters experienced by bladder cancer patients and nursing intervention strategy should be developed based on the effectively management of symptom clusters rather than focusing on a single symptom. 방광암은 전 세계 암 유병률 중 7위를 기록하고 있으며. 환자들은 다양한 증상들을 경험하고 있다. 두 개 이상의 증상이 동시에 나타나는 것을 증상 클러스터(Symptom Cluster)라고 하는데, 환자들이 호소하는 단일 증상에 대한 접근보다는 서로 연관되어있는 증상들의 통합적인 접근과 중재가 요구됨에 따라 실무에서는 그 중요성이 증가하고 있다. 증상 클러스터는 삶의 질에 부정적인 영향을 미치는 것으로 보고되고 있으나, 방광암 환자의 증상 클러스터 연구는 부족한 실정이다. 따라서 방광암 환자들의 증상 클러스터를 규명하고 효과적으로 중재하기 위한 기초자료를 제공하기 위해 본 연구를 수행하였다. 본 연구는 횡단적-서술적 조사 연구이며, 연구 대상자는 방광암으로 진단받고 요로전환술 후 6개월이 경과한 남성환자이다. 자료수집은 2019년 9월 11일부터 2019년 11월 30일까지 서울시에 소재한 Y 대학교 부속병원 비뇨의학과 외래 및 병동에서 질문지법을 통해 이루어졌다. 수집된 자료는 기술통계, 빈도분석, 독립 표본 t검정, 분산분석(One Way ANOVA), 상관 분석, 요인분석, 단계적 다중회귀분석(Stepwise multiple regressions), Cronbach’s alpha를 사용하였고 SPSS 25.0 프로그램을 이용하였다. 연구의 결과는 다음과 같다. 1. 본 연구 대상자는 총 120명이었다. 평균 연령은 71.31(±8.88)세이며, 109명(90.8%)의 대상자가 배우자가 있으며 돌봄 제공자가 있다고 응답했다. 암 진단 병기는 stage Ⅲ, Ⅳ가 72명(60.0%)으로 많았고, 요로전환술 타입으로는 회장도 관요로전환술이 79명(65.7%), 요관 피부 문합술이 29명(24.2%), 정위성방광대치술이 12명(10.0%)의 순으로 많았다. 수술 후 경과 시기는 평균 54.34(±57.97)개월이었다. 2. 본 연구 대상자의 건강관련 삶의 질은 168점 만점에 평균 96.39(±26.90)점 이었고 100점으로 환산했을 때 57.38점 정도의 수준이었다. 하부 영역별 삶의 질 평균 점수는 신체적 건강 21.13(±5.13)/28점, 정서적 건강 15.95(±5.79)/24 점, 사회적/가족적 건강 14.96(±5.02)/28점, 기능적 건강 14.28(±7.07)/28점, 추가적인 문제들(radical cystectomy-specific) 30.08(±10.11)/60점 순으로 높았다. 3. 본 연구 대상자의 증상 경험은 4점 만점에 평균 2.48(±0.63)점이었다. 가장 정도가 심한 증상으로는 발기부전(0.28±0.81), 삶을 즐기기 어려움(1.56±1.29), 삶의 질에 대한 만족도 저하(1.62±1.32) 순으로 낮았고 가장 경미한 증상으로는 오심(3.58±0.85), 소변조절 어려움(3.42±1.16), 현기증(3.36±0.96)으로 높았다. 4. 본 연구 대상자의 클러스터는 총 3개로 규명되었으며 제 1요인은 피로-체중감소 클러스터(Fatigue-weight loss cluster), 제 2요인은 정서적 디스트레스 클러스터(Emotional distress cluster), 제 3요인은 식욕-수면장애-삶의 질 클러스터(Appetite-sleep disturbance-QoL cluster)로 확인되었다. 가장 정도가 심한 클러스터로는 식욕-수면장애-삶의 질 클러스터(1.90±1.03), 정서적 디스트레스 클러스터(2.49±1.04), 피로-체중감소 클러스터(2.69±1.61) 순 이었다. 5. 본 연구의 단계적 회귀분석 결과 건강관련 삶에 질에 미치는 영향 요인으로는 식욕-수면장애-삶의 질 클러스터(β=14.366, p<.001), 피로-체중감소 클러스터(β=5.454, p<.001), 정서적 디스트레스 클러스터(β=7.036, p<.001), 연령(β=-.351, p=.016), 교육 수준(β=5.730, p=.027) 이였고 식욕-수면장애-삶의 질 클러스터가 가장 큰 영향을 미쳤다. 모형의 전체 설명력은 77.8%였다(F=79.236, p<.001, R2=.778). 본 연구 결과에서는 요로전환술을 받은 남성 방광암 환자들이 다양한 증상들을 동시에 경험하며, 규명된 증상 클러스터는 전반적인 건강관련 삶의 질에 영향을 미치는 것으로 나타났다. 따라서 간호 실무에서는 요로전환술을 받은 방광암 환자들이 경험하는 증상 클러스터에 대한 폭넓은 이해력이 요구되며, 단일 증상에 초점을 맞추기 보다는 증상 클러스터를 효과적으로 관리할 수 있는 중재 전략을 개발해야 할 것이다.-
