소아청소년의 치과치료 시 시각적 탐색

Abstract

이 연구의 목적은 학령기 이전의 어린이, 학령기의 어린이, 그리고 청소년기 환자들의 치과 치료 중 시각적 탐색을 연구하는 것이다. 이 연구에서 시각적 탐색이란, 환자가 치료 중 눈을 뜨고 주변 환경을 관찰하는 것을 의미한다. 전체 치과 치료 중 눈을 뜨고 있는 시간의 비율을 측정하고 연령과의 상관관계를 조사하였다. 연세대학교 치과대학병원 소아치과에 내원하여 실란트 또는 수복 치료를 받은 Frankl 3,4 등급의 5 - 15세, 총 60명의 환자들을 조사하였다. 총 치료 시간과 치료 중 눈을 뜨고 있는 시간을 측정하였고, 평균 비율을 산출하여 비교하였다. 치과 치료 중 눈을 뜨고 있는 시간의 평균 비율은 5 - 6세는 90.3%, 7 - 9세는 87.5%, 10 - 12세는 83.6%, 13 - 15세는 35.3%로 나이가 증가할수록 감소하였다. 12세 이하 어린이들의 평균 눈을 뜨고 있는 시간의 비율은 13 - 15세 청소년들에 비해 통계학적으로 유의미하게 더 높았다. 소아치과 의사는 어린이 환자들이 치과 치료 중 눈을 뜨고 있는 비율이 높다는 점을 인지하여야 한다. 눈의 손상과 감염을 방지하기 위한 적극적인 눈 보호가 필요하며, 눈을 통해 소아환자의 감정을 감지하고, 불안과 공포를 경감시키기 위한 노력이 필요하다. The aim of this study is to investigate visual exploration of children and adolescents during dental treatment by examining time ratio of eye opening in various age groups. In this research, visual exploration is defined as the eye opening and observation of the surrounding dental environment. This study reviewed 5 - 15 year-old patients of Frankl rating 3 and 4, who visited the department of pediatric dentistry of Yonsei University Dental Hospital from March to October 2018. Restorative treatment or sealant application was done by one pediatric dentist. A trained observer used 2 stopwatches to measure total treatment time and time of open eye. Average ratios of eye opening were calculated and compared. 60 patients were investigated, with 15 patients in each age group. Average ratio of eye opening increased as age decreased. The average of 13 - 15 year-olds was significantly shorter than that of children younger than 12 years. This study indicated that children have higher rates of eye opening during dental treatment than adolescents.
